Evaluating chaplaincy utilization in a suicide-risk-stratified Veterans Affairs patient sample
Although chaplaincy is a key part of the Department of Veterans Affairs' (VA) healthcare services, few studies have investigated how classified suicide risk impacts this service utilization. Filling this gap, this study evaluates utilization patterns in a suicide-risk-stratified national sample of VA patients who died by suicide (cases), as well as suicide risk-matched patients (controls) who did not die by suicide in 2017-2018. Given overdispers…
Rural–urban disparities in access to interventional psychiatry treatments among U.S. Department of Veterans Affairs patients
Objective: While tele-mental health has improved access to standard medication management and psychotherapy for rural Veterans, implementation of interventional psychiatry treatments will require in-person care, potentially leading to the exacerbation of rural-urban disparities in access to mental health care. We studied the availability of interventional psychiatry treatments, delivered clinically or in the context of a research trial, to rural …
Using natural language processing to inform targeted rural and urban Hispanic U.S. Department of Veterans Affairs suicide prediction models
Objective: Rural Hispanic Veterans experience elevated suicide rates when compared to urban counterparts. Group differences remain poorly understood. This study evaluates a rurality-stratified sample of Hispanic Veterans Affairs (VA)-patients, leveraging unstructured electronic health record (EHR) data to refine population-specific suicide risk prediction metrics. Method: The study utilized a rural and urban Hispanic VA-patient dataset, including…
Mental health care quality across the Covid-19 pandemic in rural versus urban patients
This study reports on the quality of mental healthcare delivered to rural and urban Department of Veterans Affairs (VA) patients across COVID-19 pandemic. This is a retrospective study using VA medical records between 3/10/2019 and 9/22/2022. This study divided the COVID-19 pandemic into Eras based on significant historical events. Quality metrics represented core mental health services. Delivery modalities included on-site, telephone, and video.…
Adult friendship and suicidality
Despite the role friendship can play in the physical and mental health of adults, little is known about its relationship with suicidality. To address this gap, we conducted a scoping review on adult friendship and suicidality in Medline, PsycInfo, and the Cochrane Library from inception to April 9, 2024. We used a combination of keywords and MeSH terms to find studies reporting on the themes of suicidality and adult friendship. We summarized key …
Examination of increasing suicide rates among rural Hispanic VA patients
PURPOSE: Death by suicide is increasing more rapidly among Hispanics than non-Hispanics who use United States Department of Veterans Affairs (VA) health services, and the increase is most rapid among those living in rural areas. Our goal was to identify characteristics of rural Hispanic VA patients that contribute to this emerging disparity. METHODS: We linked electronic medical records from the VA, personnel data from the US Department of Defens…

The role of race in rural‐urban suicide disparities
PURPOSE: To assess the role that race-ethnicity plays in modifying the observed rural-urban disparity in suicide among Veteran Health Administration (VHA) users. METHODS: We performed a retrospective cohort study of 10,737,864 VHA users between 2003 and 2017, using cross-linked VHA medical records and National Death Index mortality data to assess longitudinal race-stratified rural-urban differences in age- and sex-adjusted annual suicide rates. W…
Influence of contextual factors on death by suicide in rural and urban settings
PURPOSE: To examine the association between contextual factors, represented by geographic and community health variables, and suicide among rural and urban Department of Veterans Affairs health care users (VA users). METHODS: We performed a retrospective cohort study of 12,700,847 VA users between 2003 and 2017. We assigned contextual factors based on individuals' home address at the ZIP Code (area deprivation), county (sunlight exposure, altitud…
Evaluating policies to improve access to mental health services in rural areas
PURPOSE: The United States Department of Veterans Affairs (VA) has gradually implemented policies to increase access to mental health care outside of VA medical centers. Most notably, this included requirements to offer mental health services at VA-administered community-based clinics in 2008 and increased access to VA-paid care in the community beginning in 2014. Our objective was to understand how mental health service use patterns changed for …
A pandemic of Body, Mind, and Spirit
Death by suicide is a serious problem in the United States and rural communities are disproportionally affected by these tragedies.1 According to the National Vital Statistics System (NVSS), suicide rates in rural communities have risen significantly over the past few decades.1 The gap between the suicide rates in rural and urban communities is broadening and suicide rates are increasing at a faster rate in rural communities than they are in urba…
Timing of evidence-based psychotherapy for posttraumatic stress disorder initiation among Iraq and Afghanistan war veterans in the Veterans Health Administration
Most veterans in our study did not initiate CPT/PE. Generally, clinical complexity variables increased likelihood of initiating CPT/PE and initiating CPT/PE more than 1 year after first mental health visit. Additional research is needed to understand whether CPT/PE delay results from receipt of alternative intervention due to clinical complexity variables. Learning what matters for patients
Sedentary lifestyle, poor dietary behaviors and metabolic alterations associated with psychiatric medications contribute to poor health and high rates of obesity among individuals with serious mental illness (SMI). Interventions that increase engagement in physical exercise, dietary modifications, lifestyle changes and preventive health care can provide health benefits across the lifespan. These interventions have led to substantial physical impr…
