Redlining, Urban Heat, and Climate Hazards
Structural inequities, including historical redlining, have created compounding climate-related vulnerabilities in marginalized communities, with potential implications for pediatric cardiometabolic diseases (CMD). However, the joint effects of redlining, climate risk, and urban heat on pediatric CMD remain poorly understood. Using New York State hospital administrative data (2018–2022), we examined associations between neighborhood redlining, cl…
National and State-Level Trends in Mental Health and Substance Use Disorder Services at Federally Qualified Health Centers, 2012–2019
Provision of Behavioral Health Services at Community Health Centers and Hospital Emergency Department Visits
BACKGROUND: Numerous US patients seek the hospital emergency department (ED) for behavioral health care. Community Health Centers (CHCs) offer a potential channel for redirecting many to a more patient-centered, lower cost setting. OBJECTIVE: The aim of this study was to identify unique market areas serviced by CHCs and to examine whether CHCs are effective in offsetting behavioral health ED visits. RESEARCH DESIGN: We identified CHC-year specifi…
Changes in Performance Measures and Service Volume at US Federally Qualified Health Centers During the Covid-19 Pandemic
In this cohort study of US FQHCs, nearly all quality measures declined during the first year of the COVID-19 pandemic, with most declines persisting through 2021. Similarly, most visit types declined in 2020; 60% of these remained below prepandemic levels in 2021. By contrast, mental health and substance use visits increased in both years. The pandemic led to forgone care and likely exacerbated behavioral health needs. As such, FQHCs need sustain…
Spatial availability of federally qualified health centers and disparities in health services utilization in medically underserved areas
Association Between Medicaid Managed Care Coverage of Substance Use Services and Treatment Utilization
The results of this cross-sectional study suggest that carving out coverage of substance use services and financing them through FFS coverage may be associated with overall increases in treatment utilization but with heterogeneous associations across states and treatment types
A database of US state policies to mitigate Covid-19 and its economic consequences
CUSP is the most comprehensive publicly available policy database of health, social, and economic policies in response to the COVID-19 pandemic in the US. CUSP documents widespread variation in state policy decisions and implementation dates across the US and serves as a freely available and valuable resource to policymakers and researchers
Association of Medicaid Expansion With 5-Year Changes in Hypertension and Diabetes Outcomes at Federally Qualified Health Centers
In this nationally representative cohort study, Medicaid expansion was associated with sustained increases in insurance coverage and improvements in chronic disease outcome measures at FQHCs after 5 years overall and among Black and Hispanic populations. States considering Medicaid expansion may benefit from improved longer-run health measures for underserved patients with chronic conditions
What the New Biden Administration May Mean for Medicaid
The Medicaid program provides health insurance coverage to more than 68 million low-income Americans. Over the past 4 years, the Trump administration aimed to reduce spending and enrollment in Medicaid through work requirements, monthly premiums, proposed block granting, and repealing the Affordable Care Act (ACA), upon which the authority and funding for Medicaid expansion rests. Although the upcoming Supreme Court ruling on the constitutionalit…
The Impact of Rhode Island’s Multipayer Patient-centered Medical Home Program on Utilization and Cost of Care
OBJECTIVE: To estimate the effect of patient-centered medical home (PCMH) participation on cost and utilization of care for patients in Rhode Island's statewide, multipayer PCMH program, which serves over one-third of the state population. DATA SOURCES/STUDY SETTING: 2009-2014 claims data from all payers in Rhode Island, representing >743,000 unique patients. STUDY DESIGN: A propensity score-matched difference-in-differences framework was used to…
State Variation in Quality Outcomes and Disparities in Outcomes in Community Health Centers
BACKGROUND: Prior studies have reported that community health centers perform as well as other sites of care, despite serving more vulnerable patient populations. However, there is little prior study of geographic variation in quality outcomes and disparities in outcomes in this setting. Quantifying geographic variation is important so as to target quality improvement efforts and funding and to learn from states where total quality is highest and…
