Understanding rural‐urban differences in veterans’ internet access, use and patient preferences for telemedicine
BACKGROUND: The expansion of telemedicine (e.g., telephone or video) in the Veterans Health Administration (VA) raises concerns for health care disparities between rural and urban veterans. Factors impeding telemedicine use (e.g., broadband, digital literacy, age) disproportionally affect rural veterans. PURPOSE: To examine veteran-reported broadband access, internet use, familiarity with, and preferences for telemedicine stratified by residentia…
Patient and provider predictors of telemental health use prior to and during the Covid-19 pandemic within the Department of Veterans Affairs
The current study examined patient and provider differences in use of phone, video, and in-person mental health (MH) services. Participants included patients who completed ≥ 1 MH appointment within the Department of Veterans Affairs (VA) from 10/1/17-7/10/20 and providers who completed ≥ 100 VA MH appointments from 10/1/17-7/10/20. Adjusted odds ratios (aORs) are reported of patients and providers: (a) completing ≥1 video MH appointment in the pr…
African American Veterans Storytelling
BACKGROUND: Disparities in hypertension control persist for African American Veterans. OBJECTIVE: To enhance cultural relevance of hypertension control self-management education, in a multisite, stratified randomized trial, we tested an interactive Veteran-to-Veteran storytelling digital video disk (DVD) intervention created with Veteran partners, versus an education-only DVD comparison. METHODS: At 3 VA facilities, African American Veterans with…
Personal Health Record Use and Its Association with Antiretroviral Adherence
Development and Evaluation of an Internet and Personal Health Record Training Program for Low-income Patients With HIV or Hepatitis C
BACKGROUND: Vulnerable populations face difficulties accessing and using the internet and personal health record (PHR) systems for health-related purposes. Populations disconnected from the internet also tend to be disconnected from health care services. OBJECTIVES: To develop and evaluate an intervention to increase skills in health-related internet and PHR use for vulnerable populations with limited computer and internet experience. RESEARCH DE…
Patient-Provider Secure Messaging in VA
BACKGROUND: The Veterans Health Administration has implemented patient to clinical team electronic asynchronous secure messaging (SM). This disruptive technology has the potential to support continuous, coordinated quality care, but limited evidence supports this connection. OBJECTIVES: The objective of this paper is to (1) measure SM implementation and identify facility characteristics associated with higher rates of adoption and (2) understand …
Evaluating Secure Messaging From the Veteran Perspective
Secure messaging (SM) is a secured asynchronous electronic e-mail system within the Veterans Health Administration web-based patient portal, My Health eVet. This electronic tool is part of a national transformation initiative to create new models of care to support patient-provider communication and promote self-care management. SM is designed to empower patients to communicate with their providers, but to date little research has evaluated Veter…
Market and Beneficiary Characteristics Associated With Enrollment in Medicare Managed Care Plans and Fee-for-Service
Enrollment in MMC is related to beneficiary characteristics, including health status and socioeconomic status, and is modified by MMC presence in the local market. Because vulnerable subgroups are more likely to enroll in MMC plans, the Centers for Medicare & Medicaid Services should monitor how changes to Medicare Advantage policies and payment methods may affect beneficiaries in those groups
