Assessing the approach to perinatal mental health screening and treatment in maternal-child health clinics in Western Kenya
Integrating mental health care into well-attended health services, such as maternal-child health (MCH), offers a promising approach to increasing access to mental health care. To inform integration, we assessed the current approach to screening and treatment of perinatal mood and anxiety disorders (PMAD) in MCH facilities in Western Kenya. We conducted a cross-sectional survey among the facility managers (“in-charges”) at 20 MCH facilities in Wes…
Improving HIV assisted partner services outcomes by eliciting additional partners after the initial encounter
Most assisted partner services (APS) programs elicit partners at the time of HIV diagnosis when index clients may be reluctant to name all partners. Little is known about the benefits of ongoing partner elicitation after the initial visit. We utilized data collected in an APS implementation study across 31 facilities in western Kenya from August 2019 to June 2022. HIV testing service providers offered APS to consenting female index clients and as…
Potential Applications of Realist Methodologies to Implementation Science Research
There is a growing demand for more focus on developing and testing theories in implementation science (IS). This involves creating and testing theories about the different dynamics contributing to implementation strategies’ success or failure in different settings. While the application of realist methodologies is gaining traction in IS, their contributions to the field have not been explicitly articulated. To this end, we highlight six potential…
Exploring implementation strategies in evidence-based open streets programs for promoting physical activity in the Americas
Introduction: Chronic diseases are a leading public health concern in the Americas, and physical inactivity contributes significantly to their burden. Open streets program-community initiatives that temporarily close urban streets to vehicles-promote physical activity and community engagement, demonstrating positive health and social impacts. Effective implementation depends on identifying suitable strategies and frameworks. The Expert Recommenda…
Barriers and facilitators to implementing a single-visit, screen-and-treat approach with thermal ablation for cervical cancer prevention in Kenya
Cervical cancer continues to be a major global threat to women's health, with approximately 660,000 women diagnosed annually, 94% of whom are in low- and middle-income countries (LMICs). The high disease burden in LMICs is partly due to suboptimal adoption and widespread implementation of effective preventive interventions. This study explored drivers of implementation success and failure for a future single-visit, screen, and treat approach with…
Exploring supportive roles for global north investigators in global health research
Many academics are increasingly pushing for solutions to address inequitable partnership dynamics in global health research and practice. Efforts to improve equity in collaborations have prompted academic institutions to grapple with promotion and tenure criteria (usually Global North institutions), as these criteria often require leadership on grants, publications, and conference presentations for advancement. From inequitable funding opportunit…
Can audit and feedback improve health service readiness and delivery outcomes in a low-resource setting? Effectiveness results of the Ideas strategy from central Mozambique
Limited evidence exists on audit and feedback(A&F) in low-resource contexts. We tested the Integrated District Evidence-to-Action (IDEAs), a multicomponent A&F strategy in Mozambique’s maternal and child health (MCH) services. IDEAs include A&F meetings, readiness assessments, and facility support. We report the effectiveness results. IDEAs were implemented in 2016–2020 across two provinces, 12 districts, and 154 primary health facilities in Moza…
Measuring the strength of maternal, newborn and child health care implementation and its association with childhood mortality risk in three rural districts of Tanzania
This observational cohort study explores the association between maternal, newborn and child health care (MNCH) implementation strength and child survival in rural Tanzania from 2011-2015. Measuring this relationship is crucial to improve health outcomes and inform decision-making by identifying which interventions are most effective, where gaps exist, and how to strengthen primary health care systems. We used data from a 2011 service availabilit…
“I feel good because I have saved their lives”
Introduction Assisted partner services (APS), or notification for sexual partners of people diagnosed with HIV, is an efficient, effective, and high yield strategy to identify people living with HIV and is recommended by the World Health Organization (WHO). However, there remains a need to further understand the acceptability of APS qualitatively from a client lens, particularly when APS is integrated into the national health system. We investiga…
Integration of assisted partner services within Kenya’s national HIV testing services program
