Socioeconomic inequity in extreme outcomes within very pre-term and/or very low birthweight infants
Background Pre-term birth (< 37 weeks' gestation) is a major cause of neonatal mortality, with very pre-term (< 32 weeks' gestation) and extremely pre-term (< 28 weeks' gestation) infants facing the highest risks. While socioeconomic disparities in pre-term birth are well-documented, relatively little is known about inequities among the highest risk subgroups. Methods Using data from the RECAP Pre-term Project across six high-income countries, we…
A Longitudinal Study of Preterm Infants at 12 and 30 Months
Development takes place when change in one domain cascades into change in another domain. Preterm infants exhibit disruptions to their object play and the maintenance of a joint focus of attention with another person. Likewise, they tend to experience cognitive delays throughout childhood. By the developmental cascades model, early features of object play and joint engagement in preterm infants predict cognitive development. We examined longitudi…
A heterogeneity analysis of health-related quality of life in early adults born very preterm or very low birthweight across the sociodemographic spectrum
Preterm birth and very low birthweight (VP/VLBW) are associated with poorer health-related quality of life (HRQoL) outcomes extending into adulthood, yet it remains unclear how these effects differ across sociodemographic subgroups. This study aimed to identify heterogeneity in the association of VP/VLBW on HRQoL in early adulthood, specifically examining maternal age, education, and ethnicity. Individual-level data from three longitudinal cohort…
End-of-Life Decision Making Between Doctors and Parents in Nicu
We report the development and assessment of a novel coding framework in the context of research into neonatal end-of-life decision making conversations. Data comprised 27 formal conversations between doctors and parents of critically ill babies, recorded in two neonatal intensive care units. The coding framework was developed from a qualitative analysis of the recordings using the method of conversation analysis (CA). Codes underpinned by our qua…
Variations in coping stages for individuals with chronic kidney disease
Using a cross-sectional design, we examined coping stages (Kübler-Ross) among patients with end-stage renal disease at nephrology practices incorporating professional social workers as patient navigators, providing person-centered education and support ( N = 420). We evaluated associations with behavioral counseling constructs ( assess–advise–agree–assist–arrange). Coping stages comprised denial = 35.24 percent, acceptance = 24.05 percent, depres…
Parental involvement in neonatal critical care decision-making
The article analyses the decision-making process between doctors and parents of babies in neonatal intensive care. In particular, it focuses on cases in which the decision concerns the redirection of care from full intensive care to palliative care at the end of life. Thirty one families were recruited from a neonatal intensive care unit in England and their formal interactions with the doctor recorded. The conversations were transcribed and anal…
Health insurance status is associated with periodontal disease progression among Gullah African-Americans with type 2 diabetes mellitus
Increased access to health care, including dental services, may achieve reduction in chronic periodontal disease progression (as determined by CAL) for this study population. These results are very timely given the March 2010 passing of the US healthcare reform bills
Inattentive behaviour is associated with poor working memory and slow processing speed in very pre-term children in middle childhood
BACKGROUND. Problem behaviour is common following pre-term birth, but the underlying nature of these difficulties is not well known. AIMS. We sought to establish the mechanisms underpinning behavioural difficulties in very pre-term (VPT) children in middle childhood by comparing their performance to that of term born peers on tasks of working memory, inhibition, and processing speed, and relating these to parent and teacher assessments of their b…
