Experiences of Everyday Discrimination by Sexual Orientation in a Large National Cohort of Female Nurses
While discrimination has been studied among sexual minorities (SM), researchers have called for several necessary innovations, including adopting standard measures of discrimination and identifying attributions of such mistreatment among sexual minorities. We used data from the Nurses' Health Study 2 ( n = 69,483)-a longitudinal cohort of female nurses across the U.S. Experiences of discrimination were measured in 2017 using the Everyday Discrimi…
Barriers, facilitators, and best practices for successful vaping prevention campaigns for sexual and gender minority youth
BACKGROUND: Sexual and gender minority youth (SGMY) vape at higher rates than their heterosexual and/or cisgender peers. SGMY prefer SGM-specific anti-tobacco messaging; however, there have been no vaping prevention campaigns designed for SGMY. PURPOSE: Using the Exploration, Preparation, Implementation, Sustainment (EPIS) framework, we sought to explore specific barriers, facilitators, and best practices for health promotion for SGMY that will i…

Sexual orientation-related disparities in perinatal mental health among a prospective cohort study
Sexually minoritized women (SMW) may be at an increased risk of adverse perinatal mental health, though prior research is limited. We examined sexual orientation-related differences in perinatal mental health (i.e., stress and depression), and antidepressant utilization among those at different severities of clinically significant perinatal depressive symptoms. Nurses' Health Study 3 participants with prospectively assessed pregnancies (N = 6,364…
Trajectories of Mental Distress Among US Women by Sexual Orientation and Racialized Group During the First Year of the Covid-19 Pandemic
Objectives. To describe longitudinal trends in the prevalence of mental distress across the first year of the COVID-19 pandemic (April 2020‒April 2021) among US women at the intersection of sexual orientation and racialized group. Methods. Participants included 49 805 cisgender women and female-identified people from the COVID-19 Sub-Study, a cohort of US adults embedded within the Nurses’ Health Studies 2 and 3 and the Growing Up Today Study. We…
Barriers to Cervical Cancer Screening by Sexual Orientation Among Low-Income Women in North Carolina
We sought to examine cervical cancer screening barriers by sexual orientation among low-income women in North Carolina. The MyBodyMyTest-3 Trial recruited low-income women ( n = 70) to straight/heterosexual women ( n = 683). For both LGBQ and straight respondents, the greatest barriers to screening were lack of health insurance (63% and 66%) and cost (49% and 50%). LGBQ respondents were more likely than straight respondents to report forgetting t…

Associations between sexual orientation, sex education curriculum, and exposure to affirming/disaffirming LGB content in two US-based cohorts of adolescents
Sexual health education experienced by lesbian, gay, and bisexual (LGB) youth varies widely in relevancy and representation. However, associations among sexual orientation, type of sex education, and exposure to affirming or disaffirming content have yet to be examined. Understanding these patterns can help to address gaps in LGB-sensitive sex education. Our goal in this study was to examine the prevalence and associations among abstinence-only u…
Harvard Medical School’s Sexual and Gender Minority Health Equity Initiative
PROBLEM: Sexual and gender minority (SGM) populations face numerous health disparities. Medical school curricula lack adequate educational content preparing students for serving SGM patients, and medical students typically do not experience welcoming, inclusive educational environments conducive to learning about SGM health care. APPROACH: In 2018, Harvard Medical School (HMS) launched the 3-year Sexual and Gender Minority Health Equity Initiativ…

Sexual Minority Men’s Perspectives and Experiences of Adolescent Pregnancy Risk and Pregnancy Prevention Behaviors
Sexual minority men (e.g., gay, bisexual, queer) are more likely than heterosexual men to be involved in an adolescent pregnancy, but little research has been done on the context surrounding this disparity. To address this gap, and as part of the larger Sexual Orientation, Gender Identity, and Pregnancy Experiences (SLOPE) Study, semi-structured interviews and descriptive surveys were conducted with 10 cisgender sexual minority men, ages 29-49, f…
Use of media sources in seeking/receiving sexual health information during adolescence among adults of diverse sexual orientations in a US cohort
The objective in this study was to examine the association between sexual orientation and seeking/receiving sexual and reproductive health (SRH) information from media sources during adolescence. We analysed data from male and female participants (aged 23-35 years) from the U.S.-based Growing Up Today Study (GUTS) in 2016. Sex-stratified, multivariable log-binomial models adjusted for age, cohort and race/ethnicity were used to examine sexual ori…
Sexual Identity and Birth Outcomes
Race-ethnic disparities in birth outcomes are well established, and new research suggests that there may also be important sexual identity disparities in birth weight and preterm birth. This study uses the National Longitudinal Study of Adolescent to Adult Health and is the first to examine disparities in birth outcomes at the intersection of race-ethnicity and sexual identity. We use ordinary least sqaures and logistic regression models with liv…

The Prospective Association of Internalized Sexual Prejudice during Adolescence with Binge Drinking, Smoking, and Disordered Weight Control Behaviors in Adulthood
Prior research shows the importance of internalized sexual prejudice and participation in health indicators for sexual minority people, yet no research has used prospective measures or differentiated sexual minority groups. Data come from the longitudinal cohort study Growing Up Today Study 1 (N = 6,606), limited to participants who provided information on internalized sexual prejudice in adolescence and health indicators in adulthood. Completely…

Addressing the dearth of critical gender analysis in public health and medical pedagogy
Gender-based analysis in public health is a systematic examination of how population health is shaped by systems of gender relations, involving policies and laws, programs and services, research priorities, social norms and practices, and public discourse. To address the paucity of critical gender-based analysis training in most public health, medical, and health policy courses, we designed the capstone course in the Women, Gender, and Health (WG…

Disparities by Sexual Orientation in Frequent Engagement in Cancer-Related Risk Behaviors
Objectives. We examined sexual-orientation disparities in frequent engagement in cancer-related risk indicators of tobacco, alcohol, diet and physical activity, ultraviolet radiation, and sexually transmitted infections (STIs). Methods. We used longitudinal data from the national Growing Up Today Study (1999–2010). Of the analytic sample (n = 9958), 1.8% were lesbian or gay (LG), 1.6% bisexual (BI), 12.1% mostly heterosexual (MH), and 84.5% compl…
Changes in Reported Sexual Orientation Following US States Recognition of Same-Sex Couples
Objectives. To compare changes in self-reported sexual orientation of women living in states with any recognition of same-sex relationships (e.g., hospital visitation, domestic partnerships) with those of women living in states without such recognition. Methods. We calculated the likelihood of women in the Nurses’ Health Study II (n = 69 790) changing their reported sexual orientation between 1995 and 2009. Results. We used data from the Nurses’ …
Influence of Hormonal Contraceptive Use and Health Beliefs on Sexual Orientation Disparities in Papanicolaou Test Use
Objectives. Reproductive health screenings are a necessary part of quality health care. However, sexual minorities underutilize Papanicolaou (Pap) tests more than heterosexuals do, and the reasons are not known. Our objective was to examine if less hormonal contraceptive use or less positive health beliefs about Pap tests explain sexual orientation disparities in Pap test intention and utilization. Methods. We used multivariable regression with p…
