Cost and cost-effectiveness of alternative screening and diagnostic pathways for achieving hepatitis C elimination in the country of Georgia
We evaluated the cost and cost-effectiveness of alternative screening pathways during 2018–2022 within Georgia’s hepatitis C elimination program, which started in 2015. We calculated patient-level costs (2022 USD$) of hepatitis C treatment with centralized and decentralized diagnostic testing in hospitals, primary healthcare (PHC), harm reduction providers (HRP), or specialized providers (SP) from reimbursement records including the value of dona…

Can real-world scaling up HCV treatment in people who inject drugs be cost-saving
INTRODUCTION: UK is one of a few countries that are likely to meet WHO HCV elimination targets of reducing incidence to <2 per 100 person years in People Who Inject Drugs (PWID), but the economic case may be important for investment elsewhere and in future in UK. We illustrate the impact of real-world scaled-up HCV treatment to assess under what drug treatment costs could the investment be cost-saving. METHODS: We utilised a mathematical and econ…
Modelling the impact of cuts in US Pepfar funding for opioid agonist therapy and needle and syringe programmes on drug-related deaths and HIV and hepatitis C transmission among people who inject drugs
BACKGROUND: The US government paused funding for the US President's Emergency Plan for AIDS Relief (PEPFAR) in 2025, including disruptions in funding for opioid agonist therapy (OAT) and needle and syringe programmes (NSP). We evaluated the impact of these disruptions on HIV/HCV transmission and drug-related deaths (DRDs) among people who inject drugs (PWID) for 9 countries where PEPFAR funds OAT. METHODS: We developed a static model of HIV/HCV t…
Time for tailored harm reduction focus on new initiates to injecting drug use
The risk of drug overdose and disease acquisition is highly elevated for those recently initiated into injecting drug use. Despite this, there are few harm reduction interventions developed specifically for new initiates. Further, international technical documents have historically provided little guidance on how to appropriately meet the needs of what is an inexperienced and difficult to engage sub-population. However, the recently updated opera…

Gender differences in hepatitis C virus incidence in and out of opioid agonist treatment among people who inject drugs in Montréal, Canada, March 2011–March 2024
We found similar HCV incidence among women and men who inject drugs in Montréal; however, among OAT-eligible men, OAT engagement was strongly associated with a lower risk of HCV acquisition. These findings highlight that gender disparities in HCV, as observed in some settings, are not consistent. Further research is needed to determine factors underlying gender differences
Gender differences in social networks of women and men who inject drugs in Kenya
Optimal provision of opioid agonist therapy (OAT) and needle and syringe programmes (NSP)
These findings show how interacting relational and structural mechanisms generate optimal outcomes, guiding policy and service design. Implementing a multi-agency approach, integrating different mechanisms across the identified contexts, may be necessary to achieve optimal service delivery

Hepatitis C virus cascade of care among adults in Sindh province, Pakistan
Pakistan has the largest national burden of hepatitis C virus (HCV) infections (9.8 million). High levels of testing and treatment are needed to achieve HCV elimination, but little data exists on this in Pakistan. A household sero-survey from Sindh province (2019–2020) collected self-reported data from adults on previous HCV testing and treatment, and undertook HCV-antibody (HCV-Ab) testing of participants (2988 children ( 25 years), and adults w…
Characterising benzodiazepine use and the association with non-fatal overdose among people who inject opioids in England, Wales and Northern Ireland
Benzodiazepine use is common among PWIO in England, Wales, and Northern Ireland, and is associated with higher prevalence of non-fatal overdose. These findings underscore the need to strengthen overdose prevention and harm reduction efforts addressing benzodiazepine use across the UK
Impact of scaling up harm reduction interventions on injecting risk behaviours, ART outcomes and HIV incidence among people who inject drugs in Kenya
This study provides strong evidence for the benefits of NSP and OAT on varied HIV outcomes among PWID in Africa
Has the HCV cascade of care changed among people who inject drugs in England since the introduction of direct-acting antivirals
COC stages from seeing a specialist onwards improved after DAAs became widely available. Further improvements in HCV testing are needed to eliminate HCV in England

Investigating the Associations between Drought, Poverty, High-Risk Sexual Behaviours, and HIV Incidence in Sub-Saharan Africa
Climate change is increasing the likelihood of drought in sub-Saharan Africa, where HIV prevalence is high. Drought could increase HIV transmission through various mediating mechanisms; we investigated these associations. We used data on people aged 15-59 from Population-Based HIV Impact Assessment surveys from 2016 in Eswatini, Lesotho, Tanzania, Uganda, and Zambia. Survey data were geospatially linked to precipitation data for 2014-2016, with l…

Modeling the impact of the Covid-19 pandemic on achieving HCV elimination amongst young and unstably housed people who inject drugs in San Francisco
The COVID-19 pandemic impeded progress toward achieving HCV elimination. Our findings indicate that existing partial rebounds in HCV testing and treatment may achieve the elimination goal by 2030, with an additional scale-up of interventions aimed at YPWID or unstably housed PWID ensuring San Francisco is likely to achieve elimination by 2030
The cost of safe sex
There is some evidence that female sex workers (FSWs) receive greater earnings for providing unprotected sex. In 2003, the landscape of the fight against HIV/AIDS dramatically changed in India with the introduction of Avahan, the largest HIV prevention programme implemented globally. Using a unique, cross-sectional bio-behavioural dataset from 3591 FSWs located in the four Indian states where Avahan was implemented, we estimate the economic loss …
The Effect of Systemic Racism and Homophobia on Police Enforcement and Sexual and Emotional Violence among Sex Workers in East London
There is extensive qualitative evidence of violence and enforcement impacting sex workers who are ethnically or racially minoritized, and gender or sexual minority sex workers, but there is little quantitative evidence. Baseline and follow-up data were collected among 288 sex workers of diverse genders (cis/transgender women and men and non-binary people) in London (2018-2019). Interviewer-administered and self-completed questionnaires included r…
HIV infection risk and condom use among sex workers in Senegal
Social desirability bias, which is the tendency to under-report socially undesirable health behaviours, significantly distorts information on sensitive behaviours that is gained from self-reports. As a result, self-reported condom use among high-risk populations is thought to be systematically over-reported, and it is impossible to identify the determinants of condom use. The main objective of the article is to elicit unbiased information on cond…
Community Mobilization and Empowerment of Female Sex Workers in Karnataka State, South India
Objectives. We examined the impact of community mobilization (CM) on the empowerment, risk behaviors, and prevalence of HIV and sexually transmitted infection in female sex workers (FSWs) in Karnataka, India. Methods. We conducted behavioral–biological surveys in 2008 and 2011 in 4 districts of Karnataka, India. We defined exposure to CM as low, medium (attended nongovernmental organization meeting or drop-in centre), or high (member of collectiv…

Prevention of HIV infection for people who inject drugs
A dose-response relationship between exposure to a large-scale HIV preventive intervention and consistent condom use with different sexual partners of female sex workers in southern India
Study findings suggest that exposure to a large-scale HIV intervention for FSWs was associated with increased CCU with commercial clients. Moreover, there were dose-response relationships between CCU with clients and increased duration since first contacted by staff, times contacted by staff and number of condom demonstrations. Additional program effort is required to increase condom use with non-commercial partners
The costs of HIV prevention for different target populations in Mumbai, Thane and Banglalore
Avahan, the India AIDS Initiative, delivers HIV prevention services to high-risk populations at scale. Although the broad costs of such HIV interventions are known, to-date there has been little data available on the comparative costs of reaching different target groups, including female sex workers (FSWs), replace with ‘high risk men who have sex with men (HR-MSM) and trans-genders. Costs are estimated for the first three years of Avahan scale u…
Differences Between Seven Measures of Self-Reported Numbers of Clients of Female Sex Workers in Southern India
Hepatitis C virus treatment for prevention among people who inject drugs
UNLABELLED: Substantial reductions in hepatitis C virus (HCV) prevalence among people who inject drugs (PWID) cannot be achieved by harm reduction interventions such as needle exchange and opiate substitution therapy (OST) alone. Current HCV treatment is arduous and uptake is low, but new highly effective and tolerable interferon-free direct-acting antiviral (DAA) treatments could facilitate increased uptake. We projected the potential impact of …
Could low dead-space syringes really reduce HIV transmission to low levels
Relationship between exposure to the Avahan intervention and levels of reported condom use among men who have sex with men in southern India
Direct contact with Avahan program staff is associated with increased reported condom use among men who have sex with men in Bangalore. Reported consistent condom use and condom use at last sex act are associated with contacts involving demonstrations of correct condom use, and with receiving condoms, respectively
