Self-reported and Point-of-Care Biomarkers of Substance Use Measures Among Older Adults with HIV from Low- and Middle-Income Countries
Measuring alcohol and other drug (AOD) use remains challenging in HIV care in low- and middle-income countries, where self-report and point-of-care (POC) urine biomarkers capture different time windows. Among 2,059 people with HIV aged 40 years or older across eight IeDEA clinics (2020 to 2022), we examined what self-reported AOD use (AUDIT, ASSIST) and POC urine tests (uEtG for alcohol; 5-panel drug screen) each capture. Self-report identified m…
Syndemic vulnerability for suicidal ideations among youth with and without HIV in India
Suicide is a vital public health challenge among adolescents in low-middle-income countries. HIV might be clustering health conditions among adolescents in India, contributing to suicidal ideation (SI). We used data from a cross-sectional and conveniently sampled study among adolescents (15-19 years) at a tertiary care government hospital conducted between August 2018 and June 2020. Latent class analysis was used to identify syndemic vulnerabilit…
Does knowledge impact adherence
WHO’s 2013 PMTCT guidelines recommended lifelong antiretroviral therapy (ART) for HIV-infected pregnant and breastfeeding women, exclusive breastfeeding (EBF), nevirapine prophylaxis (NVP) and early infant diagnosis (EID) for HIV-exposed-breastfed infants. We examined the association between knowledge and adherence to these guidelines among 550 HIV-infected pregnant women in Maharashtra, India. Knowledge of PMTCT guidelines was assessed using a s…
High prevalence of late presentation of ART-naïve perinatally infected children for care in Pune, India
Delayed presentation to care of perinatally infected children in India continues to be a hindrance to achieving the “end pediatric HIV by 2020” goal. In this study, we characterize this issue by describing the prevalence, risk factors and temporal trends of delayed presentation to care of perinatally infected, antiretroviral therapy (ART) – naïve children using programmatic data from a tertiary care center in western India. Delayed presentation w…
The burden of anxiety among people living with HIV during the Covid-19 pandemic in Pune, India
The burden of anxiety was high during COVID lockdown in our population of socioeconomically disadvantaged PLHIV in Pune and appeared to be influenced by concerns about ART availability. The burden of anxiety among PLHIV will likely increase with the worsening pandemic in India, as sources of anxiety are expected to persist. We recommend the regular use of short screening tools for anxiety to monitor and triage patients as an extension of current …
Acceptability and feasibility of a behavioral and mobile health intervention (Combind) shown to increase uptake of prevention of mother to child transmission (PMTCT) care in India
This study found that COMBIND intervention is scalable, feasible, beneficial and very well accepted by ORWs and patients, however structural challenges in goods and services remain
Elevated highly sensitive C-reactive protein and d -dimer levels are associated with food insecurity among people living with HIV in Pune, India
Objective To assess the prevalence and determinants of food insecurity among people living with HIV (PLWH) in Pune, India and its association with biomarkers known to confer increased risks of morbidity and mortality in this population. Design Cross-sectional analysis assessing food insecurity using the standardized Household Food Insecurity Access Scale. Participants were dichotomized into two groups: food insecure and food secure. Logistic regr…
