Mentoring Early-Career Investigators of HIV/STI Health Disparities Research
BackgroundTo build research capacity for early-career faculty conducting HIV/STI research with minoritized communities and to enhance diversity in the scientific workforce, the University of California, San Francisco (UCSF) Center for AIDS Prevention (CAPS) conducts a training program for visiting professors (VPs), begun in 1996. VPs are in residence at CAPS for three summers, complete a pilot research project, and prepare National Institutes of …
Integrating Phylogenetic Biomarker Data and Qualitative Approaches
Mixed methods studies of human disease that combine surveillance, biomarker, and qualitative data can help elucidate what drives epidemiological trends. Viral genetic data are rarely coupled with other types of data due to legal and ethical concerns about patient privacy. We developed a novel approach to integrate phylogenetic and qualitative methods in order to better target HIV prevention efforts. The overall aim of our mixed methods study was …
An Approach to Reducing Problematic Data in an Acasi Sexual Behavior Assessment
Minimizing error in self-reported sexual behavior could reduce investigators' likelihood of rejecting truly successful interventions to decrease HIV and STI transmission risk. Sexual behavior assessments can elicit problematic data. This may manifest in the form of elevated levels of non-response, inaccurate point estimates, or misclassification errors resulting in inappropriately answering or, perhaps more importantly, skipping questions. We pro…
Bathhouse distribution of HIV self-testing kits reaches diverse, high-risk population
We distributed free OraQuick In-home HIV Test® kits to men at a gay bathhouse. Men were systematically selected to receive a coupon, which could be redeemed that night for an HIV self-testing kit. Those offered the coupon were asked to take an 11-item survey. About 181 men received coupons, of whom 92 (51%) accepted the coupon, and 61 (66%) men redeemed the coupon. Those who redeemed test kits and completed a survey (n = 53) were more ethnically …
A community-engaged randomized controlled trial of an integrative intervention with HIV-positive, methamphetamine-using men who have sex with men
Contingency management (CM) is an evidence-based intervention providing tangible rewards as positive reinforcement for abstinence from stimulants such as methamphetamine. Integrative approaches targeting affect regulation could boost the effectiveness of CM in community-based settings and optimize HIV/AIDS prevention efforts. This randomized controlled trial with HIV-positive, methamphetamine-using men who have sex with men (MSM) is examining the…
Knowing Your Status and Knowing Your Partner’s Status Is Really Where It Starts”
Gay and bisexual men are at disproportionate risk for human immunodeficiency virus (HIV) infection. While prevention efforts often emphasize consistent condom use, there is growing evidence that men are using seroadaptive safer-sex strategies, such as serosorting and seropositioning. This qualitative analysis of 204 HIV-negative and HIV-positive gay and bisexual men explored the ways that a sexual partner’s HIV status can influence safer-sex stra…

Community-Based Harm Reduction Substance Abuse Treatment with Methamphetamine-Using Men Who Have Sex with Men
Harm reduction approaches endeavor to assist individuals with avoiding the most detrimental consequences of risk taking behaviors, but limited research has documented the outcomes of harm reduction substance abuse treatment. In total, 211 methamphetamine-using men who have sex with men (MSM) enrolled in two outcome studies of substance abuse treatment programs that were implementing an evidence-based, cognitive-behavioral intervention (i.e., the …

Substance Use and HIV Risk Behavior among Men Who Have Sex with Men
HIV Risk Associated With Gay Bathhouses and Sex Clubs
Objectives. We studied the HIV risk behaviors of patrons of the 3 commercial sex venues for men in Seattle, Washington. Methods. We conducted cross-sectional, observational surveys in 2004 and 2006 by use of time–venue cluster sampling with probability proportional to size. Surveys were anonymous and self-reported. We analyzed the 2004 data to identify patron characteristics and predictors of risk behaviors and compared the 2 survey populations. …
HIV Transmission Risk at a Gay Bathhouse
Previous research found up to 14% of men who go to bathhouses engage in unprotected anal intercourse (UAI) and tend to have multiple sexual partners during their bathhouse visit, thus appearing to support concerns that such venues could foster acute outbreaks of new HIV infections. We conducted a two-stage probability sample of men exiting a gay bathhouse, and focused our analysis on whether the partnering patterns of the men who engaged in UAI p…
Rapid vs. Standard HIV Testing in Bathhouses
Building stakeholder partnerships for an on‐site HIV testing programme
Because of the large number of individuals at risk for HIV infection who visit gay saunas and sex clubs, these venues are useful settings in which to offer HIV outreach programmes for voluntary counselling and testing (VCT). Nevertheless, establishing a successful VCT programme in such a setting can be a daunting challenge, in large part because there are many barriers to managing the various components likely to be involved. Using qualitative da…
Experiencing release
There are nonsexual reasons that may motivate people to seek out sexual activity with others. Some men who have sex with men may seek out sex environments to engage in sexual behavior. Among the nonsexual reasons that exist for men who have sex with men is a desire to escape from distressing thoughts and feelings. The amplified sexuality and other unique characteristics of sex environments allow men to have more intense emotional experiences arou…
Perspectives on the Drug Court Model Across Systems
Drug courts have been in existence since 1989, yet few process evaluations have appeared in the literature to help inform the discussion about their effectiveness. This article reports findings from a process evaluation of a drug court program in San Mateo, California. The evaluation was designed to document the history of the program, to examine program strengths and areas of improvement, to assess the roles and relationships among the various a…
Availability of combination therapy for HIV
The objective of this study was to assess the impact of successes in the medical treatment of HIV disease on the perceptions of high-risk gay and bisexual men attending an anonymous testing site towards becoming infected with HIV and on their decisions to have unsafe sex. A cross-sectional survey with a face-to-face interview and self-recorded information was used. A convenience sample of high-risk men seeking anonymous HIV antibody counselling w…
Public Health Policy and Gay Bathhouses
Public health policy on bathhouses has been limited and poorly documented. This volume is intended to expand policy-makers' and prevention-professionals' knowledge and awareness about gay bathhouses. The present paper provides a context and an overview for the volume by describing the bathhouse environment and how it differs from other public sex environments, and by describing public policies that have been implemented
A Theoretical Approach to Bathhouse Environments
HIV prevention guidelines have aimed primarily at the individual level, although recently the field of public health has begun to focus more on structural level interventions. This paper explores an application of Rudolf Moos' person-environment theory as one model that helps to provide an understanding of the dynamic relationship between bathhouse patrons and the environment within which they engage in sexual activities. Understanding how differ…
Number and Distribution of Gay Bathhouses in the United States and Canada
Although gay bathhouses have been the subject of debate and some public health policy for decades, the relative number and geographic distribution of these establishments has not been described. As a result, it is easy to miss or ignore them in making public policy in response to discase prevention. No straightforward methodology for such a description is available, so we used a series of gay travel books, first published in 1965 by the Damron Co…
The San Francisco Centralized Intake Unit
Using client data from the publicly-funded drug abuse treatment system in San Francisco, California, this study compared demographic characteristics of clients of a central intake unit (CIU) to those of clients who did not access the CIU, and examined characteristics of CIU episodes. The San Francisco CIU was intended to make appropriate referrals of CIU clients to treatment programs, and 47.9% of these episodes were followed by a subsequent trea…
Substance use and sexual transmission risk behavior of HIV-positive men who have sex with men
Drug Court Effectiveness
Over the past two decades, drug courts have emerged as a viable alternative for addressing drug cases within the criminal justice system. In California, the Drug Court Partnership Program (DCPP) was created in 1998 and has supported and funded the development of drug courts throughout the State. This article reports on a review of California drug court evaluations through January 2000 conducted as part of an evaluation of the California DCPP. A t…
Differential HIV Risk in Bathhouses and Public Cruising Areas
Objectives. This report investigates differences in risk behaviors among men who have sex with men (MSM) who went to gay bathhouses, public cruising areas, or both. Methods. We used a probability sample of MSM residing in 4 US cities (n = 2881). Results. Men who used party drugs and had unprotected anal intercourse with nonprimary partners were more likely to go to sex venues than men who did not. Among attendees, MSM who went to public cruising …

Facilities and HIV prevention in bathhouse and sex club environments
This project evaluated the extent to which businesses with a primary purpose of providing opportunities for sexual encounters between men (e.g., bathhouses and sex clubs) have implemented strategies that target their customers with important HIV and STD prevention messages. Between October 1996 and February 1997, we conducted structured telephone interviews with 63 businesses throughout the United States in order to describe their facilities and …
Affirmative Psychotherapy for Gay Men
This article explores unique issues that confront gay male clients These issues include identity development and management, interpersonal issues, and special issues, such as the impact of aging, antigay violence, and acquired immune deficiency syndrome (AIDS). Diversity within the male gay community is also addressed throughout the article, and case examples are used to illustrate issues more fully. The article concludes with recommendations for…
Changes in needle sharing behavior among intravenous drug users
We analyzed data for San Francisco intravenous drug users entering treatment, April 1986-September 1988 (N = 7,660). The proportion of cases reporting any needle sharing in the month preceding treatment decreased from 50 percent in 1986 to 28 percent in 1988. Similar decreases were reported by two longitudinal cohorts (needle sharing by the same individuals) admitted in 1986 and 1987 (n = 303), and in 1986 and 1988 (n = 205). In a multiple logist…
