Reparations and distributive justice in global health
box Reparations are a global health imperative
Structural adjustment
Beginning in the 1980s and 1990s, the International Monetary Fund (IMF) and the World Bank implemented neoliberal structural adjustment programmes (SAPs) across most countries in Asia, Africa and Latin America. SAPs imposed austerity, privatisation and economic deregulation and have been associated with severe negative impacts on human welfare, including (a) declining real wages and working-class consumption in some regions, (b) increased rates o…
Sustaining Hope Within Entangled Accompaniments
The series of papers in this special issue, "Ethnography of and in Clinical Formation: Poetics and Politics of Dual Subjectivity," touch on several themes that are at the core of social medicine: the web of social structures and power relations that organize the risk and prematurity of disease and death, who gets care when and where, and what that care looks like and does within situated social worlds. As Levenson and Samra (this issue) describe …
University-age vaccine mandates
For the first comparison, we weighed predicted hospitalisations prevented by one booster dose of BNT162b2 with vaccine-associated SAEs from the manufacturer's randomised trial (3/5055).3 We found that the rate of expected SAEs would outweigh the benefits of the booster against hospitalisation by at least 18-fold. Lam and Nichols suggest that this was an inappropriate comparison, as not all SAEs result in hospitalisation. However, the definition o…
Covid-19 vaccine boosters for young adults
In 2022, students at North American universities with third-dose COVID-19 vaccine mandates risk disenrolment if unvaccinated. To assess the appropriateness of booster mandates in this age group, we combine empirical risk-benefit assessment and ethical analysis. To prevent one COVID-19 hospitalisation over a 6-month period, we estimate that 31 207–42 836 young adults aged 18–29 years must receive a third mRNA vaccine. Booster mandates in young adu…
Programmatic implementation of depression screening and remote mental health support sessions for persons recently diagnosed with TB in Lima, Peru during the Covid-19 pandemic
Depressive symptoms were common among persons recently diagnosed with TB. We observed improvements in depressive symptoms 6 months later for most participants who received remote sessions of PFA
The unintended consequences of Covid-19 vaccine policy
Vaccination policies have shifted dramatically during COVID-19 with the rapid emergence of population-wide vaccine mandates, domestic vaccine passports and differential restrictions based on vaccination status. While these policies have prompted ethical, scientific, practical, legal and political debate, there has been limited evaluation of their potential unintended consequences. Here, we outline a comprehensive set of hypotheses for why these p…
Re-evaluating the merits of decentralization as a core strategy for effective delivery of drug-resistant tuberculosis care in Pakistan
Decentralized, person-centred models of care delivery for drug-resistant tuberculosis (DR-TB) continue to be under-resourced in high-burden TB countries. The implementation of such models—made increasingly urgent by the COVID-19 pandemic—are key to addressing gaps in DR-TB care. We abstracted data of rifampicin-resistant (RR)/multidrug-resistant tuberculosis (MDR-TB) patients initiated on treatment at 11 facilities between 2010 and 2017 in Sindh …

PROBLEM: American Indians and Alaska Natives hold a state-conferred right to health, yet significant health and health care disparities persist. Academic medical centers are resource-rich institutions committed to public service, yet few are engaged in responsive, equitable, and lasting tribal health partnerships to address these challenges. APPROACH: Maniilaq Association, a rural and remote tribal health organization in Northwest Alaska, partner…
Toward patient-centered tuberculosis preventive treatment
There is demand for shorter regimens and child-friendly formulations for tuberculosis preventive treatment in high-burden settings. Individual preferences differ, suggesting that patient-centered care would best be supported by having multiple shorter regimens available
Trends, patterns and health consequences of multimorbidity among South Korea adults
BACKGROUND: Multimorbidity is a global challenge. It is more common in the elderly and deprived populations. Health systems are not providing appropriate care for people with multimorbidity as they are focused on managing single diseases and are not oriented to effectively manage complexity of care-coordination for multimorbidity. This study aims to examine trends, disparities and consequences of multimorbidity over a 10-year period. It also aims…
Covid-19 reveals weak health systems by design
The COVID-19 pandemic demonstrates the critical need to reimagine and repair the broken systems of global health. Specifically, the pandemic demonstrates the hollowness of the global health rhetoric of equity, the weaknesses of a health security-driven global health agenda, and the negative health impacts of power differentials not only globally, but also regionally and locally. This article analyses the effects of these inequities and calls on g…
Health systems performance in managing tuberculosis
BACKGROUND: Tuberculosis (TB) is a major global health burden, which has been inadequately addressed. This study aims to analyze different patterns and gaps of care along the care cascade across countries and to develop a model to examine the relationship between performance of tuberculosis programmes in high and low burden countries along the tuberculosis care cascade and tuberculosis disease burden. METHODS: We used the World Health Organizatio…

Structural Violence and Clinical Medicine
Structural violence refers to the social structures that put people in harm's way. Farmer and colleagues describe the impact of social violence upon people living with HIV in the US and Rwanda.
Bleeding Babies in Badakhshan
The bleeding of infants via the skin (pile) and the roof of the mouth (qüm) is practiced in Badakhshan, the easternmost province of Tajikistan. Like folk practices elsewhere, pilé and qüm exist at the interstices of modern society and reflect a complex religious, historical, and social response to poverty, marginality, and the global processes associated with the collapse of the Soviet Union. In this article, I attempt to move beyond an ethnomedi…
