Drivers and barriers to rural and urban healthcare placement in Ghana
Objective: This study explored the level of consensus on the drivers and barriers influencing doctors' decisions to work in rural versus urban areas. The study provides insights into systemic issues affecting healthcare workforce distribution in Ghana. Access to medical care is particularly important given the changing demographics of Ghana, including the growth of the older and chronically ill population and the high proportion of older adults l…
Determinants of Physical Activity and Dietary Habits among Adults in Ghana
A critical understanding of the interrelationship between two behavioral decisions—participating in physical activity, and eating healthily—is lacking in Ghana. This study aimed to determine which factors affect each of the two behavioral decisions, jointly and separately, among adults aged 18 years or older in three metropolises (Kumasi, Accra, and Tamale) of Ghana. The data from the Ghana Obesity Survey 2021 were used. A bivariate probit model …
Determinants of Covid-19-Related Length of Hospital Stays and Long Covid in Ghana
The presence of hypertension and diabetes mellitus determined both length of hospitalisation and long COVID among patients with COVID-19 in Ghana. COVID-19 prevention and management policies should therefore consider these factors
Expect the unexpected? Challenges of prospectively exploring stakeholder engagement in research
Stakeholder engagement in research is increasingly viewed as making a major contribution to assisting impact. This paper draws on a longitudinal, prospective impact study exploring stakeholder engagement in a 3-year tobacco control research project which used stakeholder engagement in the development, testing and dissemination of its return on investment tool. The paper presents the challenges of data collection when undertaking prospective resea…
Smoking and health-related quality of life in English general population
Smoking is significantly and negatively associated with health-related quality of life in English general population and the magnitude of this association is determined by the number of cigarettes smoked. The varying degree of this association, captured through instruments such as EQ-5D, may need to be fed into the design of future economic evaluations where the intervention being evaluated affects (e.g. tobacco control) or is affected (e.g. trea…
The fallacy of the equity-efficiency trade off
In the health systems literature one can see discussions about the trade off between the equity achievable by the system and its efficiency. Essentially it is argued that as greater health equity is achieved, so the level of efficiency will diminish. This argument is borrowed from economics literature on market efficiency. In the application of the economic argument to health, however, serious errors have been made, because it is quite reasonable…
Health systems performance in sub-Saharan Africa
This study suggests that the quality of governance may be an important structural determinant of health systems performance, and could be an indicator to be monitored. The association suggests there might be a causal relationship. However, the cross-sectional design, the level of missing data, and the small sample size, forces tentative conclusions. Further research will be needed to assess the causal relationship, and its generalizability beyond…
Illness reporting and demand for medical care in rural Burkina Faso
The Selection of an Appropriate Count Data Model for Modelling Health Insurance and Health Care Demand
We apply several estimators to Indonesian household data to estimate the relationship between health insurance and the number of outpatient visits to public and private providers. Once endogeneity of insurance is taken into account, there is a 63 percent increase in the average number of public visits by the beneficiaries of mandatory insurance for civil servants. Individuals' decisions to make first contact with private providers is affected by …
Determinants of parental reports of children's illnesses
Household decision-making on child health care in developing countries
Quantitative studies on health care utilization often overlook the importance of capturing the 'pathway' of household decision-making processes. This paper offers a four-step construct which maps out a hierarchical scale of household decision-making regarding child health care. The construct begins with the perception of illness, moves on to choice of care and provider, and finally ends with health care expenditure. The construct is substantiated…
