What works in real-world implementation? Practice-based insights from the Enjoy IMP-ACT project promoting physical activity among older adults
Physical inactivity among older adults remains a significant public health issue. Age-friendly active spaces with the Seniors Exercise Park equipment in local parks offer an accessible, low-cost opportunity to promote health through physical activity. The ENJOY IMP-ACT project was a multi-site implementation initiative aimed at scaling up the use of the Seniors Exercise Park across six sites in Victoria, Australia. This paper examines factors inf…
Reverse translating parkrun’s scale-up success
Introduction: The scale-up evaluation field has been limited by a conceptualisation of success dominated by quantitative spread and restriction to purposively and centrally disseminated initiatives. Empirical research shows, however, that working definitions of scale-up success are far more varied, nuanced, and context-dependent. Moreover, excluding examination of organically scaling initiatives has narrowed what may be learnt about sustainable d…
Implementation factors influencing utilisation of Seniors Exercise Parks
Identified 13 barriers or facilitators influencing uptake of Seniors Exercise Parks by older adults • Used socio-ecological model to analyse stakeholder interviews across five LGAs • Found social connection and peer support key to sustained park engagement • Barriers included poor health, fear of injury, low awareness, and poor visibility • Volunteer-led programs and co-location with amenities boosted participation To examine implementation-relat…
Scalability and scaling-up strategy of a physical activity policy intervention in Australian childcare centres
There is an urgent need for scalable interventions to promote physical activity in early childhood. An early childhood education and care (ECEC) physical activity policy intervention with implementation support strategies (Play Active) has been proposed for scale-up in Australia. This study sought to assess the scalability of Play Active and describe the Play Active scaling-up strategy. The Intervention Scalability Assessment Tool was used to ass…
Taking stock
The complexity of research translation is well recognized. Efforts to accelerate the process have emerged through implementation science, scale-up, scalability, and sustainability. This area of research is now inundated with a plethora of new concepts, creating overlap and confusion for those working in public health, implementation science, and scale-up as well as those seeking to understand it. This perspective examines the commonly used proces…
Physical activity policies in the six Gulf Cooperation Council countries

Device-captured incidental physical activity, sedentary behaviour and all-cause mortality risk
BACKGROUND: Incidental physical activity from normal daily living may be a more feasible approach to improve health status in adults who are unwilling or unable to complete traditional structured exercise. We aimed to determine if incidental physical activity can attenuate or eliminate the all-cause mortality risk associated with high-sedentary behaviour. METHODS: We examined the tertile-based joint associations of accelerometry-derived sedentary…
National, regional, and global trends in insufficient physical activity among adults from 2000 to 2022
BACKGROUND: Insufficient physical activity increases the risk of non-communicable diseases, poor physical and cognitive function, weight gain, and mental ill-health. Global prevalence of adult insufficient physical activity was last published for 2016, with limited trend data. We aimed to estimate the prevalence of insufficient physical activity for 197 countries and territories, from 2000 to 2022. METHODS: We collated physical activity reported …
Coming back for more
Most physical activity (PA) maintenance research has concerned adherence to small-scale interventions or infrequent observation in cohort studies. We analysed individual attendance trajectories and their drivers in a large-scale ‘real-world’ community-based weekly PA event (parkrun) cohort in Australia. Data were weekly attendance (walking/running) of 223 224 unique parkrun participants over their first 3 years of participation. An unweighted mov…
A cross-sectional study of Australian teachers’ health
Teacher’s lifestyle behaviours are important because they lead to positive health outcomes for teachers themselves and because teachers model behaviour to their students. This cross-sectional study examined the lifestyle behaviours of a large sample of teachers in New South Wales (NSW), Australia and assessed the association between work-related factors and lifestyle behaviours. From February to October 2021, data were collected on the lifestyle …
Real-world public health interventions demonstrate how research evidence informs program scale-up
Evidence-based population interventions rely on intervention testing (efficacy and effectiveness trials) to determine what works to improve public health. We investigated the characteristics of real-world public health interventions to address obesity and explored the extent to which research testing was undertaken prior to scale-up. We identified 90 population health interventions targeting physical activity, nutrition or obesity-related health …
Sustaining health obesity prevention programs
Current understanding of what sustains public health programs is derived primarily from evaluations of programs delivered for only a few years. To improve program planning and knowledge on program sustainment, we examined real-world population programs that have been delivered for >2 years. Our focus was physical activity and nutrition programs for obesity prevention. We identified programs through published literature and searched through public…

Impact of the first year of the “This Girl Can” physical activity and sport mass media campaign in Australia
The initial wave of the TGC-Victoria mass media campaign showed reasonably high levels of community awareness and encouraging decreases in women feeling judged whilst being active, but these did not yet translate into overall physical activity gains. Further waves of the TGC-V campaign are in progress to reinforce these changes and further influence the perception of being judged among low-active Victorian women
Mapping and analysis of laws influencing built environments for walking and cycling in Australia
Increasing physical activity is a systems issue, requiring actions across multiple sectors. An in-depth examination of the legal environment is an important step towards understanding and influencing the existing physical activity system, why it may not be generating desired outcomes, and potential opportunities for improvement. Our findings reveal opportunities where laws could be strengthened to promote more active environments. Updating this d…
Device-based physical activity measures for population surveillance—issues of selection bias and reactivity
Recruiting a population representative sample for device-based measurement of physical activity is challenging, and there is a substantial risk of sample selection bias and measurement reactivity. Further research in this area is needed if device-based measures should be considered for population physical activity surveillance

A Hierarchy of Unhealthy Food Promotion Effects
We assessed the evidence for a conceptual “hierarchy of effects” of marketing, to guide understanding of the relationship between children’s exposure to unhealthy food marketing and poor diets and overweight, and drive the research agenda. We reviewed studies assessing the impact of food promotions on children from MEDLINE, Web of Science, ABI Inform, World Health Organization library database, and The Gray Literature Report. We included articles…
Health behaviour and the school environment in New South Wales, Australia
Evidence, Ethics, and Values
We propose a new approach to guide health promotion practice. Health promotion should draw on 2 related systems of reasoning: an evidential system and an ethical system. Further, there are concepts, values, and procedures inherent in both health promotion evidence and ethics, and these should be made explicit. We illustrate our approach with the exemplar of intervention in weight, and use a specific mass-media campaign to show the real-world dang…

Impact of an outdoor gym on park users’ physical activity
Using natural experimental studies to guide public health action
Despite smaller effect sizes, interventions delivered at population level to prevent non-communicable diseases generally have greater reach, impact and equity than those delivered to high-risk groups. Nevertheless, how to shift population behaviour patterns in this way remains one of the greatest uncertainties for research and policy. Evidence about behaviour change interventions that are easier to evaluate tends to overshadow that for population…
Bicycle Use for Transport in an Australian and a Belgian City
The walkability attributes of neighborhood environments (residential density, land use mixture, and connectedness of streets) have been found to be associated with higher rates of walking. However, relatively less is known about the associations of walkability attributes with bicycle use for transport. We examined the relationships between adults' bicycle use for transport and measures of neighborhood walkability in two settings: an Australian ci…
Effects of Human–Dog Interactions on Salivary Oxytocin Concentrations and Heart Rate Variability
Dog ownership is often advocated for its potential benefits to human health, with changes to oxytocin and autonomic nervous system (ANS) activity proposed as possible underlying mechanisms. The aims of the current study were to a) examine the effect of two common types of human–dog interactions (dog-walking and affiliative interactions) on salivary oxytocin concentrations and heart rate variability (HRV, an indicator of ANS activity), and b) inve…
