Rural-Urban Differences in Coverage Gains Under Continuous Postpartum Medicaid Eligibility
Black/White disparities in low birth weight across maternal trajectories of social mobility in South Carolina
Continuous Medicaid Eligibility During the Covid-19 Pandemic and Postpartum Coverage, Health Care, and Outcomes
In this cohort study, continuous Medicaid eligibility during the COVID-19 PHE was associated with significantly reduced postpartum uninsurance for people with Medicaid-paid births, but was not associated with postpartum visit attendance, contraception use, breastfeeding, or depressive symptoms at approximately 4 months postpartum. These findings, though limited to the context of the COVID-19 PHE, may offer preliminary insight regarding the potent…
Consistency and Adequacy of Public and Commercial Health Insurance for US Children, 2016 to 2021
The findings of this cross-sectional study indicate that insurance gaps are a particular problem for publicly insured children, whereas insurance inadequacy and particularly, out-of-pocket costs are a challenge for commercially insured children. Both challenges improved during the COVID-19 PHE. Improving children's health coverage after the PHE will require policy solutions that target the unique needs of commercially and publicly insured childre…

Screening for and Experiences of Intimate Partner Violence in the United States Before, During, and After Pregnancy, 2016–2019
Objectives. To measure rates of intimate partner violence (IPV) screening during the perinatal period among people experiencing physical violence in the United States. Methods. We used 2016–2019 Pregnancy Risk Assessment Monitoring System data (n = 158 338) to describe the incidence of physical IPV before or during pregnancy. We then assessed the prevalence of IPV screening before, during, or after pregnancy and predictors of receiving screening …
Using Modernized Medicaid Data to Advance Evidence-Based Improvements in Maternal Health
Medicaid is the primary payor for nearly half of all births in the United States and plays a disproportionate role in covering maternity care for low-income people, rural people, and minoritized racial groups. Newly available, modernized Medicaid claims data—the Transformed Medicaid Statistical Information System Analytic Files (TAF)—offer a significant opportunity to conduct novel research that can drive the development of evidence-based program…
Association of Health Insurance, Geography, and Race and Ethnicity With Disparities in Receipt of Recommended Postpartum Care in the US
The findings of this cross-sectional survey of postpartum individuals in the US suggest that inequities in postpartum care content were extensive and compounded for patients with multiple disadvantaged identities. Examining only 1 dimension of identity may understate the extent of disparities. Future studies should consider the content of postpartum care visits
Rural Hospital Administrators’ Beliefs About Safety, Financial Viability, and Community Need for Offering Obstetric Care
In this survey of US rural hospitals that offer obstetric services, many administrators indicated prioritizing local community needs for obstetric care over concerns about financial viability and staffing
Severe Maternal Morbidity and Mortality Risk at the Intersection of Rurality, Race and Ethnicity, and Medicaid
After Childbirth
Factors Associated With Postpartum Uninsurance Among Medicaid-Paid Births
In this cross-sectional survey study of 43 states, a high proportion of people with Medicaid-paid births were uninsured in the postpartum period, particularly those living in Medicaid nonexpansion states. The study findings suggest that state extensions of pregnancy-related Medicaid eligibility through the first year postpartum could disproportionately benefit Hispanic and Indigenous people, unmarried people, those with lower education levels, an…
Characteristics of US Rural Hospitals by Obstetric Service Availability, 2017
Objectives. To describe characteristics of rural hospitals in the United States by whether they provide labor and delivery (obstetric) care for pregnant patients. Methods. We used the 2017 American Hospital Association Annual Survey to identify rural hospitals and describe their characteristics based on the lack or provision of obstetric services. Results. Among the 2019 rural hospitals in the United States, 51% (n = 1032) of rural hospitals did …
Amphetamine- and Opioid-Affected Births
Objectives. To estimate trends in incidence, outcomes, and costs among hospital deliveries related to amphetamines and opioids. Methods. We analyzed 2004-to-2015 data from the National Inpatient Sample, a nationally representative sample of hospital discharges in the United States compiled by the Healthcare Cost and Utilization Project, by using a repeated cross-sectional design. We estimated the incidence of hospital deliveries related to matern…
