Managing the move
Men in South Africa are disproportionately underrepresented in HIV care, and men who migrate face challenges using HIV services. Positive coping skills are critical for effective healthcare decision-making, yet little is known how men’s HIV status and mobility-related stressors affect their coping strategies. Johannesburg is home to ∼5 million people who have relocated from elsewhere, mostly men. From February-April 2024, we conducted a cross-sec…
Intersectional forces of urban inequality and the global HIV pandemic
To determine how the intersection of increased urban growth and poverty has impacted HIV incidence and prevalence, given growing HIV inequalities globally. Retrospective analysis using combined data from five publicly available, population-level datasets to determine city- and within-urban countrywide estimates of 95-95-95 treatment targets, prevalence and incidence rates from 2015 to 2019. For city-level estimates, we analysed combined data from…
Redefining global health in the 21st century
Global health is at a critical juncture, with significant achievements in reducing deaths from HIV and under-five mortality since 2000. However, progress in other areas, such as maternal mortality and tuberculosis, remains uneven, and cardiovascular diseases continue to rise. Compounding these challenges is the emerging threat of climate change, which is predicted to cause millions of health-related deaths by the end of the century. This commenta…
HIV Care for Men on the Move
Maladaptive Coping Strategies Mediate the Relationship Between Depression and Anxiety and Moderate-to-Heavy Alcohol Use in Young South Africans with HIV
Impact of Covid-19 Public Health Measures on Antiretroviral Therapy Use Among Ugandans Living with HIV in Sero-Different Couples
Antiretroviral therapy (ART) use and HIV suppression among people living with HIV (PLHIV) are critical for HIV control and prevention. Extreme restrictions on movement early during the COVID-19 pandemic in Uganda may have impeded the ability to initiate and sustain access to and use of ART. From our stepped-wedge cluster-randomized trial of an integrated PrEP and ART intervention for HIV-serodifferent couples at 12 ART clinics in Uganda, we ident…
Study-to-Clinic Transition and Daily Oral PrEP Access Experiences Among Agyw in Eastern Cape, South Africa
The patient-provider relationship mediates the relationship between health literacy and resilience in young South Africans newly diagnosed with HIV
South African adolescents and young adults (AYAs) with HIV experience worse HIV-related clinical and psychological outcomes compared to older South Africans. Psychological resilience is protective against poorer outcomes, but little is known about disparities in psychological resilience among AYAs with HIV nor the mechanisms by which these differences arise. This study analyzed baseline data from a cohort of 100 AYA ages 18-24 years in Cape Town …
Youth-friendly services was the magic
Adherence to daily oral pre-exposure prophylaxis (PrEP) for HIV prevention has been challenging for adolescent girls and young women (AGYW). As part of The Community PrEP Study (CPS), AGYW were randomised to HIV-prevention empowerment counselling (intervention) or basic medication pick-up (control). In this qualitative sub-study, we interviewed AGYW participants ( n = 39) to explore PrEP use and study experiences by study arm, and study staff ( n…
Food insecurity and unemployment as mediators of the relationship between the Covid-19 pandemic and psychological well-being in young South Africans with HIV
Food insecurity may have decreased amongst our sample of young people during the COVID-19 pandemic. Our findings build on our understanding of how the psychological well-being of young people living with HIV was affected by the COVID-19 pandemic and may lend support to interventions targeting food insecurity to improve psychological well-being in this population
Migrant men and HIV care engagement in Johannesburg, South Africa
Our study revealed a high proportion of migrants within our community-based sample of men and demonstrated a need for HIV and other healthcare services that effectively reach migrants in Johannesburg. Future research is warranted to further disaggregate this heterogenous population by different dimensions of mobility and to understand how to design HIV programs in ways that will address migrants' challenges
Family Social Support Mediates the Relationship Between the Covid-19 Pandemic and Psychosocial well-being in a Cohort of Young South Africans Newly Diagnosed with HIV
Behavioral Modeling and its Association with PrEP and ART Use in Ugandan HIV-Serodifferent Couples
Integrating Pre-Exposure Prophylaxis (PrEP) delivery into Antiretroviral Therapy (ART) programs bridges the Human Immunodeficiency Virus (HIV) prevention gap for HIV-serodifferent couples prior to the partner living with HIV achieving viral suppression. Behavioral modeling is one mechanism that could explain health-related behavior among couples, including those using antiretroviral medications, but few tools exist to measure the extent to which …
A Risk Prediction Model to Identify People Living with HIV Who are High-risk for Disengagement from Care after HIV Diagnosis in South Africa
Trajectories of Oral PrEP Adherence Among Young Kenyan Women
Multi-level Factors Driving Pre-exposure Prophylaxis Non-initiation Among Young Women at High Risk for HIV in Kenya
Economic vulnerability and non-initiation of antiretroviral therapy in India
In India, many people living with HIV (PLHIV) do not successfully initiate antiretroviral therapy (ART) after diagnosis. We conducted a clinic-based qualitative study at the Y.R. Gaitonde Centre for AIDS Research in Chennai, Tamil Nadu to explore factors that influence ART non-initiation. We interviewed 22 men and 15 women; median age was 42 (IQR, 36-48) and median CD4+ was 395 (IQR, 227-601). Participants were distrustful of HIV care freely avai…
“You are not alone”
In South Africa, despite universal antiretroviral therapy (ART) availability, 60% of persons living with HIV (PLWH) ages 15-24 are not on treatment. This qualitative study aimed to identify barriers to ART initiation and the implications for a proposed community-based Youth Treatment Club to improve ART initiation for young PLWH in limited-resource, high HIV-prevalence communities in Cape Town, South Africa. Recruiting participants at community t…
Understanding Repeat Positive HIV Testing in South Africa Under Changing Treatment Guidelines
Educate, Amplify, and Focus to Address Covid-19 Misinformation
Exploring attitudes of adolescents and caregivers towards community-based delivery of the HPV vaccine
The study highlights an opportunity for increasing HPV vaccination among some racial and ethnic minority populations by leveraging trusted community organizations to provide information and seed conversations with a potentially broad group of trusted providers. A task-shifting approach, or reliance on staff with fewer formal credentials, may offer opportunities to support vaccination in resource-constrained settings
The Treatment Ambassador Program
Trajectories of HIV-related internalized stigma and disclosure concerns among art initiators and noninitiators in South Africa
Among ART-eligible South African PLHIV entering into HIV care, internalized stigma modestly declined over time but disclosure concerns persisted. PLHIV who initiated ART were more likely to have persistent disclosure concerns over time as compared with those who did not start ART
Medical mistrust in the context of Ebola
This investigation explores Ebola conspiracy beliefs, a form of medical mistrust, and their potential impact on health behavior. Results of an online survey in the United States in December 2014 demonstrated that 16 percent of 202 participants held conspiracy beliefs. Participants who were less knowledgeable about Ebola, more mistrustful of medical organizations, and more xenophobic more strongly endorsed conspiracy beliefs. Participants who more…

Psychosocial correlates of alcohol and other substance use among low-income adolescents in peri-urban Johannesburg, South Africa
Alcohol and other drug use is prevalent among peri-urban, South African adolescents. We identified correlates of alcohol and other drug use by gender among adolescents (age = 16-18 years; N = 822) in peri-urban Johannesburg. Interviewer-administered surveys assessed sexual activity, alcohol and other drug use, and relevant psychosocial factors. In separate logistic regression models of alcohol and other drug use stratified by gender, violence exp…
Internalized Stigma, Social Distance, and Disclosure of HIV Seropositivity in Rural Uganda
BACKGROUND: HIV is highly stigmatized, compromising both treatment and prevention in resource-limited settings. PURPOSE: We sought to study the relationship between internalized HIV-related stigma and serostatus disclosure and to determine the extent to which this association varies with the degree of social distance. METHODS: We fit multivariable Poisson regression models, with cluster-correlated robust estimates of variance, to data from 259 pe…
Impact of HIV‐related stigma on treatment adherence
Socio-cultural and economic antecedents of adolescent sexual decision-making and HIV-risk in rural Uganda
With more than half of new infections occurring among youth, HIV/AIDS remains a major contributor to morbidity and mortality in Uganda. Semi-structured interviews were performed with 48 adolescents and 15 adult key informants in a rural Ugandan community to identify influences on adolescent sexual decision-making. Inductive data analytic methods revealed five thematic influences: (1) social pressure, (2) decline of the Senga (a familial figure wh…

Understanding HIV-infected patients’ experiences with Pepfar-associated transitions at a Centre of Excellence in KwaZulu Natal, South Africa
South Africa was the largest recipient of funding from the President's Emergency Plan for AIDS Relief (PEPFAR) for antiretroviral therapy (ART) programs from 2004 to 2012. Funding decreases have led to transfers from hospital and non-governmental organization-based care to government-funded, community-based clinics. We conducted semi-structured interviews with 36 participants to assess patient experiences related to transfer of care from a PEPFAR…
Barriers to HPV immunization among blacks and latinos
Optimizing HPV immunization rates may benefit from a multi-pronged approach to holistically address provider, structural, and individual barriers to care. Further research should examine strategies for providing multiple modalities of support for providers, including a routinized system of vaccine promotion and delivery, and for addressing families' concerns about vaccine safety and efficacy
