Developing a regional mental health plan for Dar es Salaam, Tanzania
Mental health services in sub-Saharan Africa, including Tanzania, are little studied and hence challenges and opportunities are not well known, leading to difficulties in improving access to, and quality of, services to those in need. 1. To conduct a situational analysis of mental health services in Dar es Salaam region, 2. To consult with key stakeholders on mental health services delivery and planning, and 3. To develop a regional mental health…
A model-based approach to estimating the prevalence of disease combinations in South Africa
BACKGROUND: The development of strategies to better detect and manage patients with multiple long-term conditions requires estimates of the most prevalent condition combinations. However, standard meta-analysis tools are not well suited to synthesising heterogeneous multimorbidity data. METHODS: We developed a statistical model to synthesise data on associations between diseases and nationally representative prevalence estimates and applied the m…
Effects of Pack training on the management of asthma and chronic obstructive pulmonary disease by primary care clinicians during 2 years of implementation in Florianópolis, Brazil
BACKGROUND: Training primary care doctors and nurses to use Practical Approach to Care Kit (PACK) improved management of asthma and chronic obstructive pulmonary disease (COPD) in a previous randomised trial. The present study examined the training effects including a second year of follow-up with expanded coverage of repeated training sessions. METHODS: Using a stepped-wedge cluster randomised trial design, 48 clinics were randomly allocated eit…

Strengthening integrated depression services within routine primary health care using the RE-AIM framework in South Africa
Integration of mental health into routine primary health care (PHC) services in low-and middle-income countries is globally accepted to improve health outcomes of other conditions and narrow the mental health treatment gap. Yet implementation remains a challenge. The aim of this study was to identify implementation strategies that improve implementation outcomes of an evidence-based depression care collaborative implementation model integrated wi…
Multimorbidity pattern and risk of dementia in later life
Background Several long-term chronic illnesses are known to be associated with an increased risk of dementia independently, but little is known how combinations or clusters of potentially interacting chronic conditions may influence the risk of developing dementia. Methods 447 888 dementia-free participants of the UK Biobank cohort at baseline (2006–2010) were followed-up until 31 May 2020 with a median follow-up duration of 11.3 years to identif…
Management of Depression in Chronic Care Patients Using a Task-Sharing Approach in a Real-World Primary Health Care Setting in South Africa
Depressive symptoms are common in South African primary care patients with chronic medical conditions, but are usually unrecognised and untreated. This study evaluated an integrated, task-sharing collaborative approach to management of depression comorbid with chronic diseases in primary health care (PHC) patients in a real-world setting. Existing HIV clinic counsellors provided a manualised depression counselling intervention with stepped-up ref…
Applying learning health systems thinking in codeveloping integrated tuberculosis interventions in the contexts of Covid-19
The COVID-19 pandemic reversed much of global progress made in combatting tuberculosis, with South Africa experiencing one of the largest impacts on tuberculosis detection. The aim of this paper is to share our experiences in applying learning health systems (LHS) thinking to the codevelopment of an intervention improving an integrated response to COVID-19 and tuberculosis in a South African district. A sequential partially mixed-methods study wa…

Integrating Care for Diabetes and Hypertension with HIV Care in Sub-Saharan Africa
INTRODUCTION: Although HIV continues to have a high prevalence among adults in sub-Saharan Africa (SSA), the burden of noncommunicable diseases (NCD) such as diabetes and hypertension is increasing rapidly. There is an urgent need to expand the capacity of healthcare systems in SSA to provide NCD services and scale up existing chronic care management pathways. A scoping review mapped extant policy and evidence based literature on the feasibility …
Ethical issues in intervention studies on the prevention and management of diabetes and hypertension in sub-Saharan Africa
Conducting intervention studies in Africa, where medicines supply for chronic conditions is inequitable and patchy, raises major ethical issues. \n \nHere we discuss what should the ethical approach be for a research programme in terms of provision of a steady and sustainable supply of medicines for patients with diabetes and hypertension
Patient-level predictors of detection of depressive symptoms, referral, and uptake of depression counseling among chronic care patients in KwaZulu-Natal, South Africa
Nurses detected patients with depressive symptoms at rates comparable to primary care providers in high-resource settings, though gaps in referral and uptake persist. Nurses were more likely to detect symptoms among patients in more severe mental distress. Implementation strategies for integrated mental health care in low-resource settings should target improved rates of detection, referral, and uptake

Comparison of the effectiveness of a milk‐free soy‐maize‐sorghum‐based ready‐to‐use therapeutic food to standard ready‐to‐use therapeutic food with 25% milk in nutrition management of severely acutely…
Community-based Management of Acute Malnutrition using ready-to-use therapeutic food (RUTF) has revolutionised the treatment of severe acute malnutrition (SAM). However, 25% milk content in standard peanut-based RUTF (P-RUTF) makes it too expensive. The effectiveness of milk-free RUTF has not been reported hitherto. This non-blinded, parallel group, cluster randomised, controlled, equivalence trial that compares the effectiveness of a milk-free s…
