Policy innovation in voluntary hospital private health insurance
BACKGROUND: Australia's voluntary hospital Private Health Insurance (PHI) uses community-rated premiums to enhance fairness and accessibility. However, community rating results in predictable profits on young, healthy people and predictable losses on the elderly and chronically ill, creating incentives for insurers to engage in risk selection. To address this, a risk equalisation scheme is applied. OBJECTIVE: This paper evaluates the effectivenes…
Performance Metrics of Substance Use Disorder Care Among Medicaid Enrollees in New York, New York
The results of this cross-sectional study suggest the need for progress in engaging patients in SUD treatment and improvement in the low performance of SUD care and limited variation in MMC plans in New York, New York. Improvement in the overall performance of SUD treatment in Medicaid potentially depends on general program improvements, not moving recipients among plans
Racial/Ethnic Disparities in Substance Use Treatment in Medicaid Managed Care in New York City
OBJECTIVE: The aim was to assess the magnitude of health care disparities in treatment for substance use disorder (SUD) and the role of health plan membership and place of residence in observed disparities in Medicaid Managed Care (MMC) plans in New York City (NYC). DATA SOURCE: Medicaid claims and managed care plan enrollment files for 2015-2017 in NYC. RESEARCH DESIGN: We studied Medicaid enrollees with a SUD diagnosis during their first 6 mont…
Racial and Ethnic Disparities in Detection and Treatment of Depression and Anxiety Among Psychiatric and Primary Health Care Visits, 1995–2005
CONTEXT: Recent evidence questions whether formerly documented disparities in care for common mental disorders among African Americans and Hispanics still remain. Also, whether disparities exist mainly in psychiatric settings or primary health care settings is unknown. OBJECTIVE: To comprehensively examine time trends in outpatient diagnosis and treatment of depression and anxiety among ethnic groups in primary care and psychiatric settings. DESI…
Managed Care and Systems Cost-Effectiveness
OBJECTIVE: The objective of this study was to assess the change in system cost-effectiveness of depression treatment after the introduction of managed care. DATA SOURCES/STUDY SETTING: The study population consisted of adults ages 18 to 69 living in low-income areas of Puerto Rico. STUDY DESIGN: Using a random probability sample of the population, 2 waves (1992-1993, 1993-1994) of data were collected before implementation of managed care and one …
Rethinking a Universal Framework in the Psychiatric Symptom-Disorder Relationship
Research on disparities in health and services should include a critical examination of how social, cultural, and contextual factors affect the connection between symptoms and psychiatric disorders. Systematic differences in this connection by these factors will mean that clinicians using a universalist approach to disorders may make more diagnostic errors for certain patients (e.g., ethnic minorities, the poor). Based on the literature on normat…
The Impact of Managed Care on the Use of Outpatient Mental Health and Substance Abuse Services in Puerto Rico
This paper estimates the impact of managed care on use of mental health services by residents of low-income areas in Puerto Rico. A quasi-experimental design evaluates the impact of a low capitation rate on a minority population using three waves of data from a random community sample. Results indicate that two years after introducing managed care, privatization of mental health services had minimal impact on use. Advocates had hoped health care …
Changes in Access to Mental Health Care Among the Poor and Nonpoor
Objectives. Health care reforms associated with managed care may adversely affect the health care safety net for disadvantaged populations. This study compared changes in health care use among poor and nonpoor individuals enrolled in managed care. Methods. Data from 3 waves of a random community sample were collected on approximately 3000 adults. Changes in use of mental health services were assessed in a pretest–posttest, quasi-experimental desi…
Federal Indian Policy
Unlike many agencies of the United States government, the Bureau of Indian Affairs has not developed a coherent set of standards for evaluating the impacts of specific actions on reservation-based Native Americans. Nor, many would claim, have the judicial, executive, and legislative branches of government offered clear policy directives to the implementing agency. It is argued here, however, that three dominant policy objectives can be abstracted…

The Political Economy of Shrimping in the Gulf of California
Inshore shrimping cooperatives in Mexico's Gulf of California are increasingly marginalized through government regulation, competition with the capital-intensive offshore trawler industry, population growth, and internal political conflict. In its ability to maintain organizational and financial stability and expand its resource base, the Yaqui Indian shrimp cooperative south of Guaymas stands apart from this trend. This success, however, rests o…
