Hauora Māori – Māori health
Māori practices are an expression of autonomy in the face of enduring health system failure. Apart from lower immunisation rates, health outcomes were not different from other models of care, despite patients having higher health risk profiles. Across all models, primary care need was unmet for many Māori, despite increased clinical input. Funding must support under-resourced Māori practices and ensure accountability for the health outcomes of Mā…
Nurses’ work in relation to patient health outcomes
Transferring work to nurses in Traditional, Health Care Home, and Corporate practices, would release general practitioner clinical time for other work. Worse patient health outcomes were associated with higher patient need and higher clinical input. It is plausible that there is insufficient clinical input to meet the degree of patient need. More practitioner clinical time is required, especially in practices with high volumes of complex patients
Is there equity of patient health outcomes across models of general practice in Aotearoa New Zealand? A national cross-sectional study
Model-level associations with poor patient outcomes suggest inequity in measures that might be used to target investment in primary care
The long shadow of inequity for Māori with gout
Gout is a health condition that can be managed to prevent morbidity and premature mortality. Mori have a higher prevalence of gout yet are less likely to receive appropriate care than non-Mori. There is scant literature presenting the patient/whnau voice relating to the health system response to gout. The study reported in this article aimed to highlight barriers and enablers in achieving best practice management of gout as defined by patients in…
How does it feel to be a problem? Patients’ experiences of self‐management support in New Zealand and Canada
BACKGROUND: The impact of long-term conditions is the "healthcare equivalent to climate change." People with long-term conditions often feel they are a problem, a burden to themselves, their family and friends. Providers struggle to support patients to self-manage. The Practical Reviews in Self-Management Support (PRISMS) taxonomy lists what provider actions might support patient self-management. OBJECTIVE: To offer providers advice on how to sup…
Understandings of disease among Pacific peoples with diabetes and end‐stage renal disease in New Zealand
BACKGROUND: Compared with New Zealand Europeans, Pacific peoples in New Zealand develop type 2 diabetes at a higher rate and a younger age, and have 3.8 times higher incidence of end-stage renal disease (ESRD). OBJECTIVE: To investigate contextual factors that shape understandings of disease for Pacific peoples with diabetes and ESRD. METHODS: Focussed ethnography. In-depth interviews were conducted with 16 Pacific people on haemodialysis for dia…

A Research Program on Implementing Integrated Care for Older Adults with Complex Health Needs (iCoach)
Health and social care systems across western developed nations are being challenged to meet the needs of an increasing number of people aging with multiple complex health and social needs. Community based primary health care (CBPHC) has been associated with more equitable access to services, better population level outcomes and lower system level costs. Itmay be well suited to the increasingly complex needs of populations; however the implementa…

Effective Organizational Leadership in the Implementation of Integrated Care; Lessons from 9 cases in the iCoach Project
Introduction: Policy makers in many countries are encouraging the development of integrated care strategies and the development of new models of integrated care. These new models require changes at a clinical or service level, organizational level and system level with strong leadership necessary at all three levels. Despite the key role of leadership in these efforts, there has been only limited study of what organizational leadership approach i…
On the Margins and Not the Mainstream
Healthcare system reforms are pushing beyond primary care to more holistic, integrated models of community based primary health care (CBPHC) to better meet the needs of aging populations and their carers. Across the world CBPHC is at varying stages of evolution and no standard model exists. In order to scale up and spread successful models of care it is important to study what works well and why to support broader efforts to implement, scale-up a…

Using Information Communication Technology in Models of Integrated Community-Based Primary Health Care
Introduction: Information and communication technology (ICT) is a promising enabler to support delivery of integrated care by inter-disciplinary teams by supporting information sharing across professional and organizational boundaries; arguably a crucial aspect of successful models of integrated care. The literature suggests that there are core components of ICT functionality such as interoperability between systems, supported chronic disease man…
Extending “Continuity of Care” to include the Contribution of Family Carers
The Maori Provider Organisation supported family carer-provider partnership enabled by shared Maori cultural values and social mandate of building family-centred wellbeing. Relational continuity was the most important level of continuity of care; it sets precedence for family carers and providers to establish the other levels - informational and management - continuity of care for their family member cared for. Family carers need to be considered…
How do Policy and Institutional Settings Shape Opportunities for Community-Based Primary Health Care? A Comparison of Ontario, Québec and New Zealand
Community-based primary health care describes a model of service provision that is oriented to the population health needs and wants of service users and communities, and has particular relevance to supporting the growing proportion of the population with multiple chronic conditions. Internationally, aspirations for community-based primary health care have stimulated local initiatives and influenced the design of policy solutions. However, the wa…
Are Patient and Carer Experiences Mirrored in the Practice Reviews of Self-management Support (Prisms) Provider Taxonomy
Patient and carer data mapped to provider activities. The taxonomy was not explicit on provider relationships and engagement with, or separate support needs of, patients and carers
Managerial strategies for integrated care
Introduction: The implementation of integrated care involves efforts to engage health care providers in a vision of health care delivery that includes coordination and collaboration for the care of individual clients. The processes by which health care providers engage with an organizational vision for integrated care, and the challenges associated with encouraging health care providers to adopt integrated practices remain under-explored in the r…
Are patient and carer experiences mirrored in the Practice Reviews of Self-Management Support (PRISMS) provider taxonomy
Introduction: Patient self-management support is a central component of care for long term conditions and for integrated care. Patients and their carers are the final arbiter of whether support for self-management has been effective. We sought to validate a new 14-item taxonomy (Practical Reviews in Self-Management Support - PRISMS) of provider activities that support patient self-management.Methods: We interviewed 74 patients and family carers i…
Mapping for conceptual clarity
Background: Studying implementation of integrated models of community-based primary health care takes a “whole systems” multidisciplinary approach to capture micro (patient/carer and provider), meso (organization) and macro (system/policy) factors. In new research studying the implementation of these integrated models intended to improve care for older adults with complex care needs, our project team adopted four theoretical perspectives to captu…
Implementing Community Based Primary Healthcare for Older Adults with Complex Needs in Quebec, Ontario and New-Zealand
The aim of this paper is to set the foundation for subsequent empirical studies of the "Implementing models of primary care for older adults with complex needs" project, by introducing and presenting a brief descriptive comparison of the nine case studies in Quebec, Ontario and New Zealand. Each case is described based on key dimensions of Rainbow model of Valentijn and al (2013) with a focus on "meso level" integration. Meso level integration is…
Population health in New Zealand 2000-2013
Introduction: Health system restructuring has emphasized a shift from medical care to population health and primary health care in an attempt to reduce social exclusion and disparities in health. Broad notions of integrated care emphasize connections between health and social services. A population health approach demands operational commitment to reducing inequalities. The health system is recognized as a determinant of health interacting with o…
Consumers with high complex needs co-create a relationship-based approach to care
Introduction: People in all countries experience different social circumstances that result in avoidable differences in health. The health care system has an important role to play in creating opportunities for all populations to be as healthy as possible, especially those with high complex health and social needs. In New Zealand, Māori, Pacific peoples, and those with lower socioeconomic status experience higher levels of chronic illness, which …

Patient resistance as a resource
This study uses conversation analysis to explore 'candidate obstacles', a practice observed in sequences of patient resistance to lifestyle advice within health professional consultations. This article presents illustrative analyses of selected data excerpts drawn from audio-visual recordings of 116 tracked consultations between health professionals and 34 patients newly diagnosed with type 2 diabetes mellitus in New Zealand. The analysis shows t…

When Equity is Central to Research
This paper is a response to our recognition that approaches to equity and consumer involvement in research differed in emphasis between our researchers and jurisdictions. Whilst we shared common aspirations we varied in our priorities between equity groups and methods to represent consumer interests. Assessment without action; a randomised evaluation of the inter RAI home care compared to a national assessment tool on identification of needs and service provision for older people in New Zealand
Comprehensive geriatric assessment (CGA) is considered the cornerstone of good practice, as it identifies need across multiple domains such as social, physical and psychological. The interRAI home care (interRAI-HC), probably the most well-researched and supported community-based CGA has been implemented globally, often at considerable expense. Policy-makers, managers and clinicians anticipate significant gains in health outcomes following such i…
Approaches to reducing the most important patient errors in primary health-care
We have previously reported a preliminary taxonomy of patient error. However, approaches to managing patients' contribution to error have received little attention in the literature. This paper aims to assess how patients and primary care professionals perceive the relative importance of different patient errors as a threat to patient safety. It also attempts to suggest what these groups believe may be done to reduce the errors, and how. It addre…
Health equity in the New Zealand health care system
There is a struggle to put equity principles into practice, indicating will without enactment. Equity is not addressed systematically below strategic levels and equity does not shape funding decisions, program development, implementation and monitoring. Equity is not incentivized although examples of exceptional practice, driven by individuals, are evident across New Zealand

Patients’ engagement in primary care
BACKGROUND: Primary health care does not adequately respond to populations known to have high needs such as those with compounding jeopardy from chronic conditions, poverty, minority status and age; as such populations report powerlessness. OBJECTIVE: To explore what poor older adults with chronic conditions who mostly belong to ethnic minority groups say they want from clinicians. SETTING AND PARTICIPANTS: Participants were older adults whose ch…
