Establishing cost-effectiveness threshold in China
INTRODUCTION: The willingness to pay per quality-adjusted life year gained (WTP/Q) is commonly used to determine whether an intervention is cost-effective in health technology assessment. This study aimed to evaluate the WTP/Q for different disease scenarios in a Chinese population. METHODS: The study employed a quadruple-bounded dichotomous choice contingent valuation method to estimate the WTP/Q in the general public. The estimation was conduct…
Cardiovascular health and economic outcomes under improved air quality in China
INTRODUCTION: China faces the dual challenge of high air pollution and an increasing burden of cardiovascular disease (CVD). We aimed to estimate the healthcare costs associated with CVD and the quality-adjusted life years (QALYs) under scenarios of improved air quality in China. METHODS: A health prediction model was developed to estimate 10-year CVD-related costs and QALY associated with PM2.5 levels in 2015, as well as two hypothetical improve…
Patient preferences for anti-hyperglycaemic medication for type 2 diabetes mellitus in China
OBJECTIVE: This study aimed to investigate the preferences regarding risks, benefits and other treatment attributes of patients with type 2 diabetes mellitus (T2DM) in China when selecting a second-line anti-hyperglycaemic medicine. METHODS: A discrete choice experiment with hypothetical anti-hyperglycaemic medication profiles was performed using a face-to-face survey administered to patients with T2DM. The medication profile was described using …

Factors associated with health care utilization and catastrophic health expenditure among cancer patients in China
Background: Cancer, the leading cause of mortality in China, is a significant burden on patients, their families, the medical system, and society at large. However, there is minimal data on health service utilization and catastrophic health expenditure (CHE) among cancer patients in China. The objective of this study was to identify factors associated with health care utilization and CHE in Chinese cancer patients. Methods: The 2018 wave of a nat…
Socioeconomic inequality in health care use among cancer patients in China
Background: Cancer is a major public health problem worldwide and the leading cause of death in China, with increasing incidence and mortality rates. This study sought to assess socioeconomic-related inequalities in health care use among cancer patients in China and to analyze factors associated with this disparity. Methods: This study used data collected for the China Health and Retirement Longitudinal Study in 2018. Patients who reported having…

Incorporating productivity loss in health economic evaluations
INTRODUCTION: Productivity loss may contribute to a large proportion of costs of health conditions in an economic evaluation from a societal perspective, but there is currently a lack of methodological consensus on how productivity loss should be measured and valued. Despite the research progress surrounding this issue in other countries, it has been rarely discussed in China. METHODS: We reviewed the official guidelines on economic evaluations i…
Multimorbidity and catastrophic health expenditure among patients with diabetes in China
INTRODUCTION: Multimorbidity is common among patients with diabetes and can lead to catastrophic health expenditure (CHE) for their families. This study aims to investigate the prevalence of multimorbidity and CHE among people with diabetes in China, and the association between multimorbidity and CHE and whether this is influenced by socioeconomic status and health insurance type. METHODS: A national survey was conducted in China in 2013 that inc…
What can we learn from China’s health insurance reform to improve the horizontal equity of healthcare financing
The horizontal inequity of healthcare finance in China over the period 2002-2007 was reduced by general taxation and social insurance, but strongly affected by out-of-pocket payments. Increasing the benefits from social health insurance would help to reduce horizontal inequity
Measuring Hypertension Progression With Transition Probabilities
This paper assessed the transition probabilities between the stages of hypertension severity and the length of time an individual might spend at a particular disease state using the new American College of Cardiology/American Heart Association hypertension blood pressure guidelines. Data for this study were drawn from the Ghana WHO SAGE longitudinal study, with an analytical sample of 1884 across two waves. Using a multistate Markov model, we est…
Ten years of progress towards universal health coverage
INTRODUCTION: This study aims to systematically evaluate vertical and horizontal equity in the Chinese healthcare financing system over the period 2008-2018 during the progress towards Universal Health Coverage (UHC), and to examine how both types of equity have changed during this period. METHODS: Household information on healthcare payments was collected from 2398 households involving 7021 individuals in 2008, 3600 households involving 10 466 i…
Impact of overweight and obesity on life expectancy, quality-adjusted life years and lifetime costs in the adult population of Ghana
INTRODUCTION: Prior studies have revealed the increasing prevalence of obesity and its associated health effects among ageing adults in resource poor countries. However, no study has examined the long-term and economic impact of overweight and obesity in sub-Saharan Africa. Therefore, we quantified the long-term impact of overweight and obesity on life expectancy (LE), quality-adjusted life years (QALYs) and total direct healthcare costs. METHODS…
Using strategic price negotiations to contain costs and expand access to medicines in China
box As China moves toward achievement of universal health coverage, it is critical that measures are in place to contain expenditures on medicines. Between 2001 and 2016, such expenditure rose by 664%.1 Historically, perverse provider incentives in which doctors were remunerated through commissions on markups on sales of medicines, as well as a fragmented system of regulation, have been major contributors to these cost

Assessing equity in benefit distribution of government health subsidy in 2012 across East China
China's primary level health care facilities were found to have a more equitable benefit distribution of government health subsidies than the secondary- and tertiary- level facilities. Increased government budget allocations and insurance imbursement rates, and the provision of technical support and qualified medical staff to lower-level hospitals were key factors. However, the provision of equal subsidies to all socioeconomic levels was found to…
