Set Down the Stethoscope
In this moment of pandemic and social unrest, health workers feel overwhelming exhaustion, uncertainty, and powerlessness. At the same time, a deep spirit of resilience and a desire for innovation, discovery, and justice compel health workers to retain their commitment to serving patients and communities. This essay takes this context into consideration and proposes pragmatic steps toward a reimagined and reinvigorated future for our work as heal…
Teaching Constructive Dialogue as a Social Medicine Tool for Peace
The social medicine learning communities of which we are a part comprise individuals from Uganda, Rwanda, Zimbabwe, Haiti, the United States, Canada, and Lebanon among other countries. United in an interest to understand and address the social and structural determination of health, learners have spanned a wide range of religious, ethnic, socioeconomic, and racial backgrounds. These diverse social locations have shaped lives in unique ways and gi…
Northern Uganda and paradigms of HIV prevention
In settings of armed conflict, traditional HIV prevention programmes that promote risk avoidance via abstinence and fidelity and risk reduction via condom use and needle exchange are not viable. In such contexts, HIV risk depends less on personal choice than on exposure to physical, emotional and structural violence. War in northern Uganda has created three realities (internally displaced people's camps, night commuters and child abductions) whic…
Framing HIV Prevention Discourse to Encompass the Complexities of War in Northern Uganda
In northern Uganda, physical and structural violence (political repression, economic inequality, and gender-based discrimination) increase vulnerability to HIV infection. In settings of war, traditional HIV prevention that solely promotes risk avoidance and risk reduction and assumes the existence of personal choice inadequately addresses the realities of HIV transmission. The design of HIV prevention strategies in northern Uganda must recognize …
Access to generic antiretrovirals
The governments of numerous low- and middle-income countries are currently instituting rules that strengthen changes in domestic intellectual property legislation, often made to conform to the mandates of "free" trade agreements signed with the United States. These measures frequently include intellectual property provisions that extend beyond the patent law standards agreed upon in recent World Trade Organization negotiations, which promised to …
Linking anthropological analysis and epidemiological evidence
For twenty years, a region of northern Uganda known as Acholiland has been heavily affected by war, leading to the formation of internally displaced people's camps, rape, transactional sex and child abductions. While it is clear that the war has had onerous consequences for the health of the Acholi people, the specific impact of the war on HIV transmission remains unclear, as the epidemiological evidence presents an ambiguous picture of HIV preva…
