A supervised machine learning approach for assessing grant peer review reports
Peer review is essential to the research lifecycle, yet the contents of grant peer review reports remain underexplored. Our study addresses this gap by developing a pipeline to systematically analyze these reports using natural language processing and machine learning. We define 12 categories relevant to funding agencies, create an annotation codebook, fine-tune and validate transformer models, and apply these classifiers to a novel text corpus c…
Airborne transmission risks of tuberculosis and Covid-19 in schools in South Africa, Switzerland, and Tanzania
The COVID-19 pandemic renewed interest in airborne transmission of respiratory infections, particularly in congregate indoor settings, such as schools. We modeled transmission risks of tuberculosis (caused by Mycobacterium tuberculosis , Mtb ) and COVID-19 (caused by SARS-CoV-2) in South African, Swiss and Tanzanian secondary schools. We estimated the risks of infection with the Wells-Riley equation, expressed as the median with 2.5% and 97.5% qu…
Seven years into the North Pacific garbage patch
The North Pacific garbage patch has accumulated floating plastic pollution for several decades. The Ocean Cleanup, a not-for-profit organization that works to retrieve this floating plastic, has conducted systematic surveys in the North Pacific Subtropical Gyre since 2015. The dataset, now spanning seven years, includes the deployment of trawls of varying sizes and the collection of aerial imagery. Here, we report a synthesis of these measurement…
Authorship inequalities in global health research
BACKGROUND: The International epidemiology Databases to Evaluate AIDS conducts research in several regions, including in Southern Africa. We assessed authorship inequalities for the Southern African region, which is led by South African and Swiss investigators. METHODS: We analysed authorships of publications from 2007 to 2020 by gender, country income group, time and citation impact. We used 2020 World Bank categories to define income groups and…
Mental Health, ART Adherence, and Viral Suppression Among Adolescents and Adults Living with HIV in South Africa
We followed adolescents and adults living with HIV aged older than 15 years who enrolled in a South African private-sector HIV programme to examine adherence and viral non-suppression (viral load > 400 copies/mL) of participants with (20,743, 38%) and without (33,635, 62%) mental health diagnoses. Mental health diagnoses were associated with unfavourable adherence patterns. The risk of viral non-suppression was higher among patients with organic …
Cervical Cancer Screening Cascade for women living with HIV
Countries with high HIV prevalence, predominantly in sub-Sahahran Africa, have the highest cervical cancer rates globally. HIV care cascades successfully facilitated the scale-up of antiretroviral therapy. A cascade approach could similarly succeed to scale-up cervical cancer screening, supporting WHO’s goal to eliminate cervical cancer. We defined a Cervical Cancer Screening Cascade for women living with HIV (WLHIV), evaluating the continuum of …
Integrating services for HIV and multidrug-resistant tuberculosis
Tuberculosis (TB) is the leading cause of death among PLHIV and multidrug-resistant-TB (MDR-TB) is associated with high mortality. We examined the management for adult PLHIV coinfected with MDR-TB at ART clinics in lower income countries. Between 2019 and 2020, we conducted a cross-sectional survey at 29 ART clinics in high TB burden countries within the global IeDEA network. We used structured questionnaires to collect clinic-level data on the T…

A spatially variable scarcity of floating microplastics in the eastern North Pacific Ocean
Plastic waste accumulating in the global ocean is an increasingly threatening environmental issue. To date, the floating and thus most visible fraction of ocean plastic pollution has been mapped at global scale. Yet, large knowledge gaps exist in our current understanding of the transport and transformation processes of positively buoyant plastic debris at the sea surface. Observations at sea typically report an apparent scarcity of microplastics…
The physical oceanography of the transport of floating marine debris
Marine plastic debris floating on the ocean surface is a major environmental problem. However, its distribution in the ocean is poorly mapped, and most of the plastic waste estimated to have entered the ocean from land is unaccounted for. Better understanding of how plastic debris is transported from coastal and marine sources is crucial to quantify and close the global inventory of marine plastics, which in turn represents critical information f…

Awareness and management of elevated blood pressure among human immunodeficiency virus–infected adults receiving antiretroviral therapy in urban Zambia
The prevalence of high blood pressure (HBP) and hypertension (HTN), awareness of the diagnoses, and use of anti-hypertensive drugs were examined among human immunodeficiency virus (HIV)-infected individuals on antiretroviral therapy (ART) in Zambia's capital Lusaka. Within a prospective cohort based at two public sector ART clinics, BP was measured at ART initiation and every 6 months thereafter as a routine clinic procedure. Predictors of HBP (s…
Regional Variation of Cost of Care in the Last 12 Months of Life in Switzerland
BACKGROUND: Health care spending increases sharply at the end of life. Little is known about variation of cost of end of life care between regions and the drivers of such variation. We studied small-area patterns of cost of care in the last year of life in Switzerland. METHODS: We used mandatory health insurance claims data of individuals who died between 2008 and 2010 to derive cost of care. We used multilevel regression models to estimate diffe…
CD4 count at antiretroviral therapy initiation and the risk of loss to follow-up
BACKGROUND: Antiretroviral therapy (ART) initiation is now recommended irrespective of CD4 count. However data on the relationship between CD4 count at ART initiation and loss to follow-up (LTFU) are limited and conflicting. METHODS: We conducted a cohort analysis including all adults initiating ART (2008-2012) at three public sector sites in South Africa. LTFU was defined as no visit in the 6 months before database closure. The Kaplan-Meier esti…

Other Fronts, Other Wars?
Other Fronts, Other Wars? goes beyond the Western Front geographically and delves behind the trenches focusing on the social and cultural history of the First World War: it covers front experiences in the Ottoman and Russian Armies, captivity in Japan and Turkey, occupation at the Eastern war theatre, medical history (epidemics in Serbia, medical treatment in Germany) and war relief (disabled soldiers in Austria). It studies the home front from t…
The Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) Statement
Much biomedical research is observational. The reporting of such research is often inadequate, which hampers the assessment of its strengths and weaknesses and of a study's generalisability. The Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) Initiative developed recommendations on what should be included in an accurate and complete report of an observational study. We defined the scope of the recommendations to cove…

Individual and spousal education, mortality and life expectancy in Switzerland
BACKGROUND: Household measures of socioeconomic position may better account for the shared nature of material resources, lifestyle, and social position of cohabiting persons, but household measures of education are rarely used. We aimed to evaluate the association of combined educational attainment of married couples on mortality and life expectancy in Switzerland. METHODS: The study included 3,496,163 ever-married persons aged ≥30 years. The 200…
What does your neighbourhood say about you? A study of life expectancy in 1.3 million Swiss neighbourhoods
BACKGROUND: Switzerland had the highest life expectancy at 82.8 years among the Organisation for Economic Co-operation and Development (OECD) countries in 2011. Geographical variation of life expectancy and its relation to the socioeconomic position of neighbourhoods are, however, not well understood. METHODS: We analysed the Swiss National Cohort, which linked the 2000 census with mortality records 2000-2008 to estimate life expectancy across ne…

Euthanasia and Assisted Suicide in Selected European Countries and US States
BACKGROUND: Legal in some European countries and US states, physician-assisted suicide and voluntary active euthanasia remain under debate in these and other countries. OBJECTIVES: The aim of the study was to examine numbers, characteristics, and trends over time for assisted dying in regions where these practices are legal: Belgium, Luxembourg, the Netherlands, Switzerland, Oregon, Washington, and Montana. DESIGN: This was a systematic review of…
A Swiss neighbourhood index of socioeconomic position
BACKGROUND: Area-based measures of socioeconomic position (SEP) suitable for epidemiological research are lacking in Switzerland. The authors developed the Swiss neighbourhood index of SEP (Swiss-SEP). METHODS: Neighbourhoods of 50 households with overlapping boundaries were defined using Census 2000 and road network data. Median rent per square metre, proportion households headed by a person with primary education or less, proportion headed by a…
Smoking and health promotion in Nazi Germany
In his recent paper in this journal "A birth cohort analysis of the smoking epidemic in West Germany", Brenner' presents data showing that smoking rates increased dramatically from the late 1920s onwards and that the reductions now being seen are limited to men with higher education. His conclusions are that intensified education programmes are required among teenagers, together with res- trictive smoking policies at the workplace and on transpor…

Bias in meta-analysis detected by a simple, graphical test
Objective: Funnel plots (plots of effect estimates against sample size) may be useful to detect bias in meta-analyses that were later contradicted by large trials. We examined whether a simple test of asymmetry of funnel plots predicts discordance of results when meta-analyses are compared to large trials, and we assessed the prevalence of bias in published meta-analyses. Design: Medline search to identify pairs consisting of a meta-analysis and …
Meta-Analysis Bias in Location and Selection of Studies
Meta-analysis has received a mixed reception since the outset. Some people have rejected what they see as exercises in “mega-silliness,”1 while the purveyors of a highly distinguished series of meta-analyses of perinatal medical care2 have been dismissed as “an obstetrical Baader-Meinhof gang.”3 To some clinicians objecting to the findings of meta-analyses, “a tool has become a weapon.”4 At the other end of the spectrum, the application of a tech…

The Revised Consort Statement for Reporting Randomized Trials
Overwhelming evidence now indicates that the quality of reporting of randomized, controlled trials (RCTs) is less than optimal. Recent methodologic analyses indicate that inadequate reporting and design are associated with biased estimates of treatment effects. Such systematic error is seriously damaging to RCTs, which boast the elimination of systematic error as their primary hallmark. Systematic error in RCTs reflects poor science, and poor sci…

Source of funding and results of studies of health effects of mobile phone use
There is concern regarding the possible health effects of cellular telephone use. We conducted a systematic review of studies of controlled exposure to radiofrequency radiation with health-related outcomes (electroencephalogram, cognitive or cardiovascular function, hormone levels, symptoms, and subjective well-being). We searched Embase, Medline, and a specialist database in February 2005 and scrutinized reference lists from relevant publication…

Consort 2010 Explanation and Elaboration
Overwhelming evidence shows the quality of reporting of randomised controlled trials (RCTs) is not optimal. Without transparent reporting, readers cannot judge the reliability and validity of trial findings nor extract information for systematic reviews. Recent methodological analyses indicate that inadequate reporting and design are associated with biased estimates of treatment effects. Such systematic error is seriously damaging to RCTs, which …
