Structural characteristics of alcohol venues are associated with testing positive for HIV in rural Kwazulu-Natal, South Africa
The HIV epidemic in South Africa is one of the largest globally, with a 20% prevalence among adults. The South African National Strategic Plan recognizes the need to address social and structural barriers to HIV prevention, as well as behavioral drivers, including alcohol abuse. Nested within a rural community-based HIV testing initiative, including at alcohol venues (AVs), we explored whether structural features of AVs were associated with HIV s…
Prevalence and Correlates of Physical Inactivity Among Individuals with HIV During the First Covid-19 Wave
Community-based referral for tuberculosis preventive therapy is effective for treatment completion
Expansion of tuberculous preventive therapy (TPT) is essential to curb TB incidence and mortality among people with HIV (PWH), yet implementation has been slow. Innovative strategies to operationalize TPT are urgently needed. Here we present an evaluation of community-based identification and referral of PWH on completion of a six-month course of isoniazid in a highly prevalent region in rural South Africa. Using a community-based TB/HIV intensiv…

Computer-Based Intervention in HIV Clinical Care Setting Improves Antiretroviral Adherence

Preparing for highly active antiretroviral therapy rollout in rural South Africa
Following a controversial history and before South Africa started the world's largest highly active antiretroviral therapy (HAART) rollout, little was known about community-level information, motivation, and behavioral skills (IMB) regarding HAART in high-HIV-prevalence rural communities. The IMB model has been shown to predict behaviors that are associated with desirable HAART outcomes. We conducted an anonymous, cross-sectional "HAART-Felt Pros…
A randomized comparison of two instruments for measuring self-reported antiretroviral adherence
A randomised trial compared two instruments for assessing self-reported adherence to antiretroviral medications: (1) a day-by-day recall instrument that elicited the number of missed doses in each of the prior three days (3-day instrument; n=64) and (2) a general recall instrument that elicited an estimate of proportion of pills taken during the prior seven days (7-day instrument; n=70). Adherence was measured at study visits over 12 months among…
Attitudes toward needle-sharing and HIV transmission risk behavior among HIV+ injection drug users in clinical care
Risky behavior related to injection drug use accounts for a considerable proportion of incident HIV infection in the United States. Large numbers of injection drug users (IDUs) currently receive antiretroviral therapy in clinical settings and are accessible for risk-reduction interventions to reduce transmission of drug-resistant HIV and spread of HIV to uninfected others. The current study examined attitudes toward needle- or equipment-sharing a…
Quality of life in HIV-infected individuals receiving antiretroviral therapy is related to adherence
This study assesses changes in quality of life (QoL) over time among HIV-infected individuals receiving antiretroviral therapy (ART) and evaluates how this relates to ARTadherence. Prospective, longitudinal data were examined from 1050 participants in two large, randomized, multi-centre antiretroviral clinical trials. QoL was assessed by the SF-12; adherence by the Terry Beirn Community Programs for Clinical Research on AIDS Antiretroviral Medica…
The Consistency of Adherence to Antiretroviral Therapy Predicts Biologic Outcomes for Human Immunodeficiency Virus–Infected Persons in Clinical Trials
We prospectively studied long-term antiretroviral adherence patterns and their impact on biologic outcomes for human immunodeficiency virus (HIV)-infected participants in 2 randomized, multicenter clinical trials. For the period from baseline to month 12 of the study, participants who reported adherence levels of 100%, 80%-99%, and 0%-79% had plasma HIV RNA levels that decreased by 2.77, 2.33, and 0.67 log(10) copies/mL, respectively (P<.001), wh…
Increasing drug users' adherence to HIV treatment
Trust and the Acceptance of and Adherence to Antiretroviral Therapy
BACKGROUND: Antiretroviral therapy (ART) has resulted in reduced AIDS incidence and mortality. Socially marginalized individuals with HIV infection, particularly injection drug users (IDUs), have received less ART and derived less benefit than others. Little is known about the therapeutic process necessary to promote acceptance of and adherence to ART among marginalized HIV-infected populations. We report on the correlates of both acceptance of a…

A prospective study of syphilis and HIV infection among injection drug users receiving methadone in the Bronx, NY
OBJECTIVES. The purpose of this study was to assess the relationship between syphilis and human immunodeficiency virus (HIV) infection in injection drug users. METHODS. A 6-year prospective study of 790 injection drug users receiving methadone maintenance treatment in the Bronx, NY, was conducted. RESULTS. Sixteen percent (4/25) of HIV-seroconverting patients, 4.8% (16/335) of prevalent HIV-seropositive patients, and 3.5% (15/430) of persistently…
Heroin use during methadone maintenance treatment
OBJECTIVES. The purpose of this study was to examine factors associated with heroin use during methadone maintenance treatment. METHODS. Logistic regression statistical models were used to examine data obtained in a cross-sectional sample of 652 methadone patients. RESULTS. Heroin use during the 3 months prior to interview was shown to be greatest among (1) patients maintained on methadone dosages of less than 70 mg/day (adjusted odds ratio [OR] …
Risk Factors for Human Immunodeficiency Virus Infection in Intravenous Drug Users
To identify risk factors for human immunodeficiency virus (HIV) infection in intravenous drug users, we undertook a study of the seroprevalence of HIV antibody in 452 persons enrolled in a methadone-treatment program in the Bronx, New York. The seroprevalence of HIV was 39.4 percent overall, 49.1 percent in blacks, 41.8 percent in Hispanics, and 17.2 percent in non-Hispanic whites (P less than 0.001 for all comparisons). The presence of HIV antib…
Transmission of the Human Immunodeficiency Virus
THE likely routes of transmission of the acquired immunodeficiency syndrome (AIDS) were established before an etiologic agent was identified. The appearance of AIDS in disparate populations, connected only by probable routes of transmission, was among the initial pieces of evidence suggesting an infectious cause. First described among homosexual men in June 1981,1 AIDS was recognized among intravenous drug users and Haitians the following year2 ,…
