“The satisfaction we get by doing this, you do not get it with any money”
While community volunteers are increasingly recognized for addressing supply-side gaps in healthcare delivery (e.g., provider shortages), their role in mitigating demand-side barriers (e.g., stigma) remains underexplored. This qualitative study investigated the experiences, knowledge, and roles of community volunteers in Goa, India. We examined barriers and facilitators to community-based work, and strategies for integrating community participati…
The impact of enhancing nutrition and antenatal infection treatment on birth outcomes in Amhara, Ethiopia
INTRODUCTION: We aimed to determine the impact of antenatal interventions to optimise maternal nutrition and infection management on birth outcomes in Ethiopia. METHODS: We conducted a pragmatic, open-label, 2×2 factorial randomised clinical effectiveness study among pregnant women enrolled <24 weeks gestation in 12 rural health centres in Amhara, Ethiopia. Eligible health centres were randomised to deliver an enhanced nutrition package (ENP) (ir…
Socioeconomic inequalities in access to online learning and its association with early childhood development during the Covid-19 pandemic in Yunnan Province, China
While online learning was widely adopted by schools during the COVID-19 pandemic globally, little is known about its effects on preschool children's development in China. Using a unique dataset collected in Yunnan Province, China in 2020, with a sample of 11,990 children aged 3–5 years from 285 kindergartens, we provide the first comprehensive assessment of socioeconomic inequalities in access to online learning and its association with early chi…
The use of geographic information systems (GIS) in studying mental health service delivery
Geographic information systems (GIS) are computer-based spatial mapping tools widely used in public health to examine service availability and access disparities and healthcare utilization. While GIS has supported evidence-based health planning in various domains, its application in mental healthcare service delivery remains underexplored. Our scoping review aimed to address this gap by exploring the scope and type of GIS usage in studying three …
Global development assistance for early childhood care and education in 134 low- and middle-income countries, 2007–2021
INTRODUCTION: Low- and middle-income countries (LMICs) often dedicate limited domestic funds to expand quality early childhood care and education (ECCE), making complementary international donor support potentially important. However, research on the allocation of international development assistance for ECCE has been limited. METHODS: We analysed data from the Creditor Reporting System on aid projects to assess global development assistance for …
Recognizing Early Childhood Education as a Human Right in International Law
There is incontrovertible evidence that early learning opportunities shape long-term development and health. Nevertheless, early childhood care and education (ECCE) is not expressly mentioned as part of the right to education in the Convention on the Rights of the Child, the International Covenant on Economic, Social and Cultural Rights, and the Convention on the Rights of Persons with Disabilities. This paper argues that the right to education c…
Assessing the long-term effects of Basic Medical Insurance on catastrophic health spending in China
Many developing countries have implemented social health insurance programmes to protect their citizens against the financial risks of seeking healthcare. While many studies have explored how individual insurance enrolments affect catastrophic health spending (CHS) in the short term, there is a lack of evidence on the long-term macro-level effects of social health insurance on CHS in low- and middle-income countries. This study examines the long-…
Global estimates of the implications of Covid-19-related preprimary school closures for children’s instructional access, development, learning, and economic wellbeing
Observational data collected prior to the pandemic (between 2004 and 2019) were used to simulate the potential consequences of early childhood care and education (ECCE) service closures on the estimated 167 million preprimary-age children in 196 countries who lost ECCE access between March 2020 and February 2021. COVID-19-related ECCE disruptions were estimated to result in 19.01 billion person-days of ECCE instruction lost, 10.75 million additio…
Prevalence and number of children living in institutional care
Inequalities in early childhood care and development in low/middle-income countries
Background: Inequalities in early childhood development (ECD) tend to persist into adulthood and amplify across the life course. To date, little research on inequalities in early childhood care and development in low/middle-income countries has been available to guide governments, donors and civil society in identifying which young children and families should be targeted by policies and programmes to improve nurturing care that could prevent the…
Maternal healthcare coverage for first pregnancies in adolescent girls
BACKGROUND: Effective maternal service delivery for adolescent mothers is essential in achieving the targets for maternal mortality under the Sustainable Development Goals. Yet little is known about levels of maternal service coverage in adolescents compared with adult mothers. METHOD: We used data from 283 Demographic and Health Surveys or Multiple Cluster Indicator Surveys for 105 countries between 2000 and 2019 to estimate the levels and trend…
Assessing national and subnational inequalities in medical care utilization and financial risk protection in Rwanda
Poverty and poor education were significant contributors to inequalities in medical care utilization and HCHS in Rwanda. Policies or interventions targeting poor households or households headed by persons receiving no education are needed in order to effectively reduce inequalities in medical care utilization and HCHS
Inequalities in socio-emotional development and positive parenting during childhood
Socio-emotional development (SED) is a critical dimension of early childhood development (ECD). However, little research has been conducted thus far regarding inequalities across family income status in children's SED and positive parenting scores in China, which has the second largest population of children in the world. Using nationally representative data from the China Family Panel Survey (CFPS), we addressed this knowledge gap by assessing t…
Decomposing health inequality with population-based surveys
Expanding insurance coverage and reducing travel time to health facilities for those living in poverty could be used as policy instruments to mitigate inequalities in medical care utilization and HCHS between the poverty and non-poverty groups
Investing in the foundation of sustainable development
Early childhood development coming of age
Assessing levels and trends of child health inequality in 88 developing countries
BACKGROUND: Reducing child mortality was one of the Millennium Development Goals. In the current Sustainable Development Goals era, achieving equity is prioritized as a major aim. OBJECTIVE: This study aims to provide a comprehensive and updated picture of inequalities in child health intervention coverage and child health outcomes by wealth status, as well as their trends between 2000 and 2014. METHODS: Using data from Demographic Health Surveys…
Is the medical financial assistance program an effective supplement to social health insurance for low-income households in China? A cross-sectional study
The MFA program is currently not an effective supplement to SHI programs in China in terms of promoting SHI enrollment and providing financial risk protection. The Chinese government needs to invest more funds to expand further low-income household enrollment in SHI programs and to widen the benefit package of MFA cash aid
Sensitivity of measuring the progress in financial risk protection to survey design and its socioeconomic and demographic determinants
Risk of poor development in young children in low-income and middle-income countries
BACKGROUND: A 2007 study published in The Lancet estimated that approximately 219 million children aged younger than 5 years were exposed to stunting or extreme poverty in 2004. We updated the 2004 estimates with the use of improved data and methods and generated estimates for 2010. METHODS: We used country-level prevalence of stunting in children younger than 5 years based on the 2006 Growth Standards proposed by WHO and poverty ratios from the …
Community-Based Health Financing and Child Stunting in Rural Rwanda
Objectives. We analyzed the likelihood of rural children (aged 6–24 months) being stunted according to whether they were enrolled in Mutuelles, a community-based health-financing program providing health insurance to rural populations and granting them access to health care, including nutrition services. Methods. We retrieved health facility data from the District Health System Strengthening Tool and calculated the percentage of rural health cent…
