Advancing Data Capacity for Economic Outcomes in Patient-Centered Outcomes Research
The economic impacts of health care treatments and services on individuals and their families are central to many decisions people make about the use of health care. However, without the high-quality data needed to generate evidence on the clinical effectiveness and economic impacts of an intervention, decision-makers are generally limited in their ability to make informed health care decisions that reflect patient values and preferences. The imp…
Developing an Integrated Longitudinal Dataset for Patient-Centered Outcome Measures in Cost-Related Medication Nonadherence
BACKGROUND: Cost-related medication nonadherence (CRN) is an important patient-centered outcome measure. Longitudinal follow-up of CRN is rare. OBJECTIVE: We propose to develop a novel integrated dataset to study CRN longitudinally. RESEARCH DESIGN: A dataset of 2000 Medicare beneficiaries at high risk of hospitalization surveyed quarterly on CRN and followed up individually for 8 quarters between 2013 and 2018 was linked to Medicare files. A met…
Variability in comorbidites and health services use across homeless typologies
Differences in behavioral-health conditions and health-system use across homeless typologies highlight the particularly high burden among homeless who are disconnected from homeless services. Fragmented and high use of emergency departments for care should motivate health systems and payers to promote housing solutions, especially those that incorporate substance use and mental health treatment
Advancing oral health policy through persuasive messaging and effective research measures
Oral health is often excluded from broad health policy discussions, addressed only in a policy silo. There is a paucity of research to guide policymaking as it relates to oral health. In response, AcademyHealth's Oral Health Interest Group organized a meeting during the 2019 AcademyHealth Annual Research Meeting to promote transdisciplinary dialog on the current state of oral health policy and the steps necessary to improve the oral health of Ame…
The Relative Ability of Comorbidity Ascertainment Methodologies to Predict In-Hospital Mortality Among Hospitalized Community-acquired Pneumonia Patients
BACKGROUND: Despite widespread use of comorbidities for population health descriptions and risk adjustment, the ideal method for ascertaining comorbidities is not known. We sought to compare the relative value of several methodologies by which comorbidities may be ascertained. METHODS: This is an observational study of 1596 patients admitted to the University of Chicago for community-acquired pneumonia from 1998 to 2012. We collected data via cha…
Recommendations for Conduct, Methodological Practices, and Reporting of Cost-effectiveness Analyses
IMPORTANCE: Since publication of the report by the Panel on Cost-Effectiveness in Health and Medicine in 1996, researchers have advanced the methods of cost-effectiveness analysis, and policy makers have experimented with its application. The need to deliver health care efficiently and the importance of using analytic techniques to understand the clinical and economic consequences of strategies to improve health have increased in recent years. OB…
Changes in health status and frequency of attending religious services among medical inpatients with repeat admissions
Although frequent attendance at religious services is associated with healthier behaviors and improved health outcomes, this relationship is confounded to the extent that attending religious services requires and displays a certain degree of health. This study surveyed patients over multiple hospitalizations at a large urban academic medical center to test the hypothesis that changes in health status would be accompanied by parallel changes in re…
Real-World Data
As real-world data (RWD) in health care begin to cross over to the Big Data realms, a panel of health economists was gathered to establish how well the current US policy environment further the goals of RWD and, if not, what can be done to improve matters. This report summarizes these discussions spanning the current US landscape of RWD availability and usefulness, private versus public development of RWD assets, the current inherent bias in term…

Are Evidence-based Practices Associated With Effective Prevention of Hospital-acquired Pressure Ulcers in US Academic Medical Centers
BACKGROUND: In 2008, the Centers for Medicare and Medicaid Services (CMS) established nonpayment policies resulting from costliness of hospital-acquired pressure ulcers (HAPUs) to hospitals. This prompted hospitals to adopt quality improvement (QI) interventions that increase use of evidence-based practices (EBPs) for HAPU prevention. OBJECTIVE: To evaluate the longitudinal impact of CMS policy and QI adoption on HAPU rates. MATERIALS AND METHODS…

Design of an Intervention to Promote Entry of Minority Youth into Clinical Research Careers by Aligning Ambition
The theory of aligned ambition posits that adolescents’ career aspirations are shaped by both contextual and attitudinal factors. Minority students are less likely to exhibit career-specific knowledge, realistic attitudes, and successful behaviors, which could exacerbate racial disparities in the health care workforce. This article describes the theoretical grounding and preliminary implementation of an intervention designed to promote aligned am…
Health Utilities for Children and Adults With Type 1 Diabetes
OBJECTIVE: We studied health utilities in patients with type 1 diabetes to understand potential differences in health utilities as function of age, type of respondent (self report vs. proxy report), and method of assessment (direct vs. indirect). RESEARCH DESIGN AND METHODS: We elicited self-reported health utilities for adults (n=213) and children (n=238) with type 1 diabetes, and by parent proxy report (n=223) for overall quality of life [Healt…
The Prevalence, Correlates, and Impact of Logically Inconsistent Preferences in Utility Assessments for Joint Health States in Prostate Cancer
BACKGROUND: Variations in health state utilities can impact cost-effectiveness analyses. One potential source of error is when joint health state (JS) utilities are rated higher than the embedded single state (SS) utilities. Knowing when and in whom this occurs can improve cost-effectiveness analyses. METHODS: Men (n = 323) were surveyed at the time of prostate biopsy. Time tradeoff SS and JS utilities for prevalent prostate cancer (PCa) health s…
Exploring the use of social network methods in designing healthcare quality improvement teams
The Cultivation of Esteem and Retrieval of Scientific Knowledge in Physician Networks
This article evaluates how physicians draw upon scientific and other forms of knowledge in different professional communities. We argue that because physicians will draw upon clinical research findings to improve their reputation with colleagues, and because the terms for accruing esteem in an academic hospital may differ depending on the dominant task structure of the organization, the form of knowledge that is valued by a physician will vary wi…
Relationship Between Quality of Care and Functional Decline in Hospitalized Vulnerable Elders
BACKGROUND: Although process of care is a valuable dimension of quality, process-of-care-based quality indicators (POC-QIs) are ideally associated with meaningful patient outcomes. The relationship between POC-QIs for hospitalized older patients and functional decline, a relevant outcome for older patients, is unknown. OBJECTIVE: To assess the relationship between POC-QIs for hospitalized elders and functional decline. RESEARCH DESIGN: Observatio…
Variation in Treatment Preferences and Care Goals Among Older Patients With Diabetes and Their Physicians
BACKGROUND: Older persons with diabetes are heterogeneous with respect to life expectancy and frailty, and new guidelines recommend individualizing care. OBJECTIVES: (1) To describe variation in the preferences of older patients with diabetes regarding aggressiveness of glycemic control and avoiding diabetic complications. (2) To determine correlates of patient preferences and physician treatment goals. (3) To assess whether physicians' goals wer…
