The policy implementation playbook
BACKGROUND: Tobacco industry interference during policy development is well documented, yet evidence on how the industry responds after policies are adopted and enter into force remains fragmented across policy domains and jurisdictions. This study systematically examines post-adoption tobacco industry conduct across key tobacco control measures and develops a cross-policy taxonomy of post-adoption tactics. We conducted as scoping review and qual…
Reducing the exceptional affordability of hand-rolling tobacco using tax escalators
Sustained duty increases on the least expensive tobacco products could deliver substantial public health gains and reduce health inequalities. Such measures should coincide with strong enforcement against illicit tobacco and comprehensive smoking cessation support
Developing and evaluating an educational intervention on conflicts of interest and corporate influence on science
Financial conflicts of interest resulting from corporate funding of research can bias the evidence base. We designed an educational intervention that sought to enable participants to make informed decisions and mitigate risk when considering corporate funding for research. We used pre/post-test surveys, which comprised a mix of closed and open-ended questions, to evaluate the training and its impact on knowledge (Wilcoxon signed-rank test), attit…
Constructing and contesting industry’s role in multistakeholder governance
BACKGROUND: Multistakeholderism as a norm stating that global public issues should be addressed by all those who affect or are affected by this issue, has become increasingly institutionalised in global governance, including the United Nations (UN) system. Despite an increasing body of evidence showing the risks of corporate capture of multistakeholder governance (MSG) and its related inability to deliver effective public health outcomes, this ap…
The UK Tobacco and Vapes Bill (2023/4)
When faced with tobacco endgame and nicotine control policies in the UK, tobacco and nicotine industries pushed back using framing strategies common to health-harming industries. Despite contradicting their 'transformation' narratives, TTCs still sought to position themselves as public health actors deserving policy access. Advocates, researchers and policymakers can anticipate opposition to endgame policies-such as a generational sales ban-and N…
Industry influence on public health policy formulation in the UK
Unhealthy commodity industries (UCIs) such as tobacco, alcohol, gambling, ultra-processed food and beverage producers are known to influence policy-making to advance their interests, often to the detriment of public health goals. This study mapped the complex system underpinning UCI’s influence on public health policy formulation in the UK and identified potential interventions to shift the system towards being able to better attain public health…
Aligning rhetoric with reality
Global food system governance increasingly relies on multistakeholder initiatives (MSIs) that aim to include those who are affected by and/or affected by an issue. Multistakeholderism’s perceived legitimacy is premised on both its outcomes (output legitimacy) and processes (input legitimacy), the latter in turn based on four key rationales: inclusiveness, procedural fairness, consensual orientation and transparency. To date, evidence on the ineff…
Analysis of Philip Morris International’s ‘aspirational’ target for its 2025 cigarette shipments
PMI's 2025 target was not 'aspirational' but highly conservative-it would have been met based on pre-existing trends in declining cigarette shipments. Yet PMI will nonetheless fail to meet that target providing evidence it is not transforming. Stalling of the decline of PMI and global cigarette sales raises significant concerns about progress in global tobacco control
Commercial determinants of health
Defining and conceptualising the commercial determinants of health
“They try to suppress us, but we should be louder”
BACKGROUND: Tobacco control advocates and researchers face powerful opponents who go to great lengths to protect their interests. While threats and attacks are documented in the grey literature, research into intimidation remains scarce. Building on previous exploratory research, this study seeks to offer in-depth insights into experiences of intimidation in the global tobacco control community. METHODS: Using qualitative description, we conducte…
Advocacy counterstrategies to tobacco industry interference in policymaking
Seeking to be seen as legitimate members of the scientific community? An analysis of British American Tobacco and Philip Morris International’s involvement in scientific events
BAT and PMI representatives attended a large number and wide range of scientific events. Given that scientific events could be a crucial platform for building connections in the scientific sphere and disseminating industry's messages, this work highlights the importance of denormalising the tobacco industry's involvement in scientific events
Tobacco industry interference to undermine the development and implementation of graphic health warnings in Bangladesh
The strategies cigarette companies used closely resemble key techniques from the well-evidenced tobacco industry playbook. The study underlines the importance of continuing monitoring and investigations into industry conduct and suspicious actors. Prioritising the implementation of WHO Framework Convention on Tobacco Control Article 5.3 is crucial for advancing tobacco control, particularly in places like Bangladesh, where close government-indust…
How has the tobacco industry passed tax changes through to consumers in 12 sub-Saharan African countries
This paper provides an overview of TI pricing strategies in response to tax increases in SSA. Governments must monitor how the TI responds to tax changes to ensure that tax increases are effective in impacting price
Document analysis of the Foundation for a Smoke-Free World’s scientific outputs and activities
Our paper identifies FSFW as a new vehicle for agnogenesis, indicating that, over 70 years since the tobacco industry began to manipulate science, efforts to protect science from its interference remain inadequate. This, combined with growing evidence that other industries are engaging in similar practices, illustrates the urgent need to develop more robust systems to protect scientific integrity
Identifying misleading corporate narratives
Narratives are key to the way corporations represent themselves to the outside world, are important to the development of shared understandings and ultimately determine whether and how corporations are able to influence societal norms and participate in policy debates. A leaked corporate affairs strategy document, from the world’s largest transnational tobacco company, Philip Morris International (PMI), suggests a company concerned about credibil…
Corporate communication of the relative health risks of Iqos through a webchat service
A variety of claims and information on the relative safety of IQOS were used in webchat communications. Response variation highlights that clearer regulation is needed to appropriately control corporate communications via live webchat services
Tobacco industry pricing strategies for single cigarettes and multistick packs after excise tax increases in Colombia
The pricing strategies identified suggest excise taxes can be increased further, particularly the specific component, to reduce the price gap between brand segments. Brands should be restricted to a single variant along with prohibitions on launching new brands/brand variants. Lastly, since the pricing of single sticks does not match the pattern of packs, more monitoring of their sales and distribution is required, especially since they promote c…
Quantifying and characterising tobacco content in the most in-demand streamed series in 10 low/middle-income countries in 2019
The continued prevalence of positively characterised tobacco content in youth-focused streamed content that is in high demand in LMICs poses a risk as a driver of smoking uptake in youth populations. There is an urgent need to better enforce tobacco advertising, promotion and sponsorship legislation in LMICs, and to update WHO FCTC guidance in line with rapidly evolving media platforms and content that is available internationally
Understanding the long-term policy influence strategies of the tobacco industry
Objective This paper explores transnational tobacco companies’ (TTCs) long-term policy influence strategies using two case studies, harm reduction and illicit tobacco, to identify lessons for the tobacco control movement and wider efforts to address the commercial determinants of health. Methods Evidence from a broad combination of sources including leaked documents and findings from over two decades of TTC monitoring were reviewed for each case …
To be honest, I’m really scared’
This exploratory study suggests that intimidation is a crucial challenge for the tobacco control community. There are several measures the global tobacco control community could take, including establishing an international support mechanism and building capacity to deal with intimidation and draw attention to it
Commercial use of evidence in public health policy
Background Ultra-processed food industry (UPFI) actors have consistently opposed statutory regulation in health policy debates, including at the WHO. They do so most commonly with claims that regulatory policies do not work, will have negative consequences or that alternatives such as self-regulation work well or better. Underlying this are often assertions that industry is aligned with principles of evidence-based policymaking. In this study, we…
Big food and the World Health Organization
INTRODUCTION: There is an urgent need for effective action to address the over 10 million annual deaths attributable to unhealthy diets. Food industry interference with policies aimed at reducing non-communicable diseases (NCDs) is widely documented at the national level but remains under-researched at the global level. Thus, this study explores how ultra-processed food industry actors have attempted to influence NCD policy at WHO. METHODS: A com…
The case for developing a cohesive systems approach to research across unhealthy commodity industries
OBJECTIVES: Most non-communicable diseases are preventable and largely driven by the consumption of harmful products, such as tobacco, alcohol, gambling and ultra-processed food and drink products, collectively termed unhealthy commodities. This paper explores the links between unhealthy commodity industries (UCIs), analyses the extent of alignment across their corporate political strategies, and proposes a cohesive systems approach to research a…

Puffing Away? Explaining the Politics of Tobacco Control in Germany
Germany is noted within Europe for its weak tobacco control policies and its opposition to European Union tobacco control legislation. In this article, we aim to explain Germany's stance on tobacco control. We review two explanations commonly proposed, namely tobacco industry donations to political parties and the legacy of the Nazis' opposition to smoking, and examine the politics of tobacco control in detail. We suggest that the interplay of nu…
Tobacco Industry Influence on Science and Scientists in Germany
Using tobacco industry documents, we examined how and why the tobacco industry sought to influence science and scientists in Germany as a possible factor in explaining the German opposition to stricter tobacco regulation. Smoking and health research programs were organized both separately by individual tobacco companies and jointly through their German trade organization. An extensive network of scientists and scientific institutions with tobacco…

Is the increasing policy use of Impact Assessment in Europe likely to undermine efforts to achieve healthy public policy
European policymakers have recently become increasingly committed to using Impact Assessment (IA) to inform policy decisions. Welcoming this development, the public health community has not yet paid sufficient attention to conceptual concerns about IA or to corporate efforts to shape the way in which IA is used. This essay is a thematic analysis of literature concerning IA and associated tools and a related assessment of the European Union's (EU)…
Changes in Smoking Prevalence in 8 Countries of the Former Soviet Union Between 2001 and 2010
Objectives. We sought to present new data on smoking prevalence in 8 countries, analyze prevalence changes between 2001 and 2010, and examine trend variance by age, location, education level, and household economic status. Methods. We conducted cross-sectional household surveys in 2010 in Armenia, Belarus, Georgia, Kazakhstan, Kyrgyzstan, Moldova, Russia, and Ukraine. We compared smoking prevalence with a related 2001 study for the different coun…
International cooperation and health. Part I
The world is increasingly shaped by powerful global forces, many of which have consequences for human health and the social, economic, and environmental factors that influence health are increasingly determined at a supranational level. As a result, local or national level efforts to influence health determinants can have only a limited impact and it is all too easy for the individual public health practitioner to feel powerless. Yet while public…

During aging, changes in gene expression are associated with decline in physical and cognitive abilities. Here, we investigated the connection between changes of mRNA and protein expression in the brain by comparing the transcriptome and proteome of the mouse cortex during aging. Our transcriptomic analysis revealed that aging mainly triggers gene activation in the cortex. We showed that increase of mRNA expression correlates with protein express…

Background/Aims Vision impairment (VI) increases the risk of various comorbidities, including depression. However, the relationship between depression and VI is unclear, and existing findings are inconsistent. We therefore investigated the risk of depression in adults before and after their registration as visually impaired. Methods In this longitudinal, nationwide, retrospective cohort study, 131 434 patients (aged ≥18 years) with newly register…

Determinants of and inequalities in self-perceived health in Ukraine
The Living Tomorrow Project
Philip Morris is using a tourist attraction to promote its views on control of second hand smoke (SHS) and accommodation of smokers and non-smokers in the indoor environment. However, ventilation does not deal with the health effects of SHS. Policymakers must be cognisant of the devious tactics the industry employs to promote its own agenda, especially in relation to indoor air quality and smoking in public places. Tobacco control legislation sho…

Exploring the impact of foreign direct investment on tobacco consumption in the former Soviet Union
The findings suggest that liberalisation of inward investment has a significant and positive impact on cigarette consumption and that without appropriate safeguards, market liberalisation may have long term negative impacts on health. Specific trade rules are needed to govern trade and investment in this uniquely harmful product. Implementation of effective tobacco control policies should precede tobacco industry privatisation. International fina…

Changes in smoking prevalence in Russia, 1996–2004
Overall smoking prevalence in both men and women in Russia remained stable between 1996 and 2004 but, given the sample size, a moderate increase in female smoking cannot be ruled out. In men, smoking increased among the least educated and declined in the most educated. In women the opposite appeared to occur, in addition to an increase in smoking in rural areas. More long term monitoring of smoking patterns in Russia, especially among women, usin…

The invisible hand
BAT's corporate misbehaviour included a wide number of anticompetitive practices, contravened Organisation of Economic Cooperation and Development's and BAT's own business standards on competition and restricted revenue arising from privatisation. This suggests that TNCs have contributed to the failure of privatisation in the FSU. Conducting open tenders and using enforceable codes to regulate corporate conduct would help deal with some of the pr…
Trends in the prevalence of smoking in Russia during the transition to a market economy
For the first time, it has been shown unequivocally that smoking among women increased markedly during the transition to a market economy in Russia. The already high prevalence of smoking among men has continued to rise. These changes are likely to reflect the activity of the tobacco industry and provide further evidence of the harms of privatisation. Effective tobacco control policies are urgently needed
The law was actually drafted by us but the Government is to be congratulated on its wise actions”
The extent of BAT's influence over public policy is consistent with the observations that, despite ratifying the Framework Convention on Tobacco Control, progress in implementing tobacco control measures in Kenya has been limited. The benefits of tobacco farming seem to be deliberately exaggerated, and an analysis of its true cost benefits is urgently needed. Tobacco farmers must be protected against BAT's predatory practices and fully informed a…

Gaining access to Vietnam's cigarette market
British American Tobacco (BAT) has made concerted efforts since the late 1980s to establish a major presence in Vietnam, among the world's 10 fastest growing tobacco markets. Until 2000, Vietnam's tight regulation of the industry has been largely driven by trade and investment policy, resulting in a stronger domestic industry but increased production and consumption of tobacco products. BAT gained market access, and achieved a dominant market sha…
Public attitudes towards smoking and tobacco control policy in Russia

Smoking uptake and prevalence in Ghana
Despite rapid economic growth and a sustained tobacco industry presence, smoking prevalence in Ghana is low, particularly among younger people. This suggests that progression of an epidemic increase in smoking has to date been avoided
Smoke-free homes in England
There has been a marked secular trend towards smoke-free homes, even when parents themselves are smokers. Living in a smoke-free home offers children a considerable, but not complete, degree of protection against exposure to parental smoking
Status of implementation of Framework Convention on Tobacco Control (FCTC) in Ghana
Whilst Ghana has ratified the FCTC, there is an urgent need for action to pass a national tobacco control bill into law to enable it to implement the treaty, sustain tobacco control efforts and prevent Ghana's further involvement in the global tobacco epidemic
