Reporting of equity in observational epidemiology
Background: Observational studies can inform how we understand and address persisting health inequities through the collection, reporting and analysis of health equity factors. However, the extent to which the analysis and reporting of equity-relevant aspects in observational research are generally unknown. Thus, we aimed to systematically evaluate how equity-relevant observational studies reported equity considerations in the study design and an…
Improving social justice in observational studies
Achieving global imperatives such as the Sustainable Development Goals (e.g., SDG 10 Reduced inequalities, SDG 3 Good health and wellbeing) requires advancing health equity in research. The implementation of the STROBE-Equity guidelines will enable a better awareness and understanding of health inequities through better reporting. We will broadly disseminate the reporting guideline with tools to enable adoption and use by journal editors, authors…
Prioritising Cochrane reviews to be updated with health equity focus
This project developed and implemented a methodology to set priorities for updating systematic reviews spanning multiple health topics with a health equity focus. It prioritised reviews that reduce overall mortality, are relevant to disadvantaged populations, and focus on conditions with a high global burden of disease. This approach to the prioritisation of systematic reviews of interventions that reduce mortality provides a template that can be…
Associations between dimensions of empowerment and nutritional status among married adolescent girls in East Africa
Our findings suggest that investment in girls' access to information through education and mass/social media and their economic empowerment may be important contributors to their overall empowerment and nutritional status. However, caution is needed as greater autonomy may contribute to increased consumption of unhealthy foods that can contribute to overweight and obesity. Strategies to empower married adolescent girls should be tailored to their…
Considerations and guidance in designing equity-relevant clinical trials
Health research has documented disparities in health and health outcomes within and between populations. When these disparities are unfair and avoidable they may be referred to as health inequities. Few trials attend to factors related to health inequities, and there is limited understanding about how to build consideration of health inequities into trials. Due consideration of health inequities is important to inform the design, conduct and repo…
Robins-I
Non-randomised studies of the effects of interventions are critical to many areas of healthcare evaluation, but their results may be biased. It is therefore important to understand and appraise their strengths and weaknesses. We developed ROBINS-I (“Risk Of Bias In Non-randomised Studies - of Interventions”), a new tool for evaluating risk of bias in estimates of the comparative effectiveness (harm or benefit) of interventions from studies that d…
Effectiveness of a hospital-initiated smoking cessation programme
Considering the relatively low cost, greater adoption of hospital-initiated tobacco cessation interventions should be considered to improve patient outcomes and decrease subsequent healthcare usage
A tutorial on sensitivity analyses in clinical trials
BACKGROUND: Sensitivity analyses play a crucial role in assessing the robustness of the findings or conclusions based on primary analyses of data in clinical trials. They are a critical way to assess the impact, effect or influence of key assumptions or variations--such as different methods of analysis, definitions of outcomes, protocol deviations, missing data, and outliers--on the overall conclusions of a study. The current paper is the second i…
The Effect of Telephone Support Interventions on Coronary Artery Disease (Cad) Patient Outcomes During Cardiac Rehabilitation
Background Cardiac rehabilitation is offered to individuals after cardiac events to aid recovery and reduce the likelihood of further cardiac illness. However, patient participation remains suboptimal and the provision of high quality care to an expanding population of patients with chronic heart conditions is becoming increasingly difficult. As a result, the feasibility and effectiveness of using telehealth interventions to deliver care have rec…

Evidence-based discussion increases childhood vaccination uptake
BACKGROUND: Childhood vaccination rates are low in Lasbela, one of the poorest districts in Pakistan's Balochistan province. This randomised cluster controlled trial tested the effect on uptake of informed discussion of vaccination costs and benefits, without relying on improved health services. METHODS: Following a baseline survey of randomly selected representative census enumeration areas, a computer generated random number sequence assigned 1…

Consumer‐driven health care
Over the past four decades, there has been a widespread movement to increase the involvement of patients and the public in health care. Strategies to effectively foster consumer participation are occurring within all research activities from research priority setting to utilization. One of the ten principles of the Cochrane Collaboration is to ‘enable wide participation’, and this includes consumers. The Cochrane Musculoskeletal Group (CMSG) is a…
A survey of the decision‐making needs of Canadians faced with complex health decisions
Objective To describe the decision‐making needs of Canadians when faced with ‘complex’ health decisions characterized by balancing advantages against disadvantages. Although a national report emphasized that public confidence in the health‐care system depends on support for personal knowledge and decision‐making, there has been no systematic investigation of the Canadian population's decision‐making needs. Design Cross‐sectional telephone survey …
What factors are associated with a woman's decision to take hormone replacement therapy? Evaluated in the context of a decision aid
Objectives To understand the factors associated with a post‐menopausal woman deciding to take hormone replacement therapy (HRT) after reviewing a decision aid (DA) and having a counselling visit with her physician as well as the factors associated with the act of taking HRT 2 months after the counselling interview. Design A secondary analysis of data collected for a randomized controlled trial evaluating two DAs. Main outcome results Although 28%…
The effects of an `explicit' values clarification exercise in a woman's decision aid regarding postmenopausal hormone therapy
Objective To evaluate the incremental effect of a graphic weigh‐scale values clarification exercise to explicitly consider the personal importance of the benefits versus the risks in a woman's decision aid regarding postmenopausal hormone therapy. Design Randomized controlled trial. Intervention Decision aid including information on options, benefits and risks, and their probabilities either followed by: (1) a graphic weigh‐scale values clarifica…

Goethe and the Intermaxillary Bone
Johann Wolfgang Goethe* (1749–1832) believed that in 1784 he demonstrated the presence of the intermaxillary (premaxillary) bone in man, and that after a certain amount of opposition professional anatomists accepted his findings. This paper tries to show what the anatomical facts are, what it was that Goethe discovered, how his beliefs about his contribution and influence arose, and how his discovery is related to his general scientific aims and …
