Building the Infrastructure to Integrate Social Care in a Safety Net Health System
A recent National Academies report recommended that health systems invest in new infrastructure to integrate social and medical care. Although many health systems routinely screen patients for social concerns, few health systems achieve the recommended model of integration. In this critical case study in an urban safety net health system, we describe the human capital, operational redesign, and financial investment needed to implement the Nationa…
The Association of Early Life Factors and Declining Incidence Rates of Dementia in an Elderly Population of African Americans
These results emphasize the importance of early life factors including rural residence and education for the risk for dementia later in life
Sex and Race Differences in the Relationship between Obesity and C-Reactive Protein
C-reactive protein (CRP) is a risk factor for cardiovascular disease and mortality; it is known to be positively associated with obesity but there is some evidence that this association differs by race or sex. We used nationally representative data of adults aged >50 years to investigate sex and race modifiers of the associations between obesity and CRP in non-Hispanic White males (n=3,517) and females (n=4,658), and non-Hispanic Black males (n=4…
Age Differences in the Association Between Body Mass Index Class and Annualized Medicare Expenditures
Excess Medicare expenditures related to obesity may be concentrated among severely obese young-old adults. Preventing severe obesity among middle and older aged adults may have large cost implications for society
Weight Loss Attitudes and Social Forces in Urban Poor Black and White Women
Social networks and emotions may be critical factors in weight management and lifestyle program participation
The Influence of Cognitive Impairment, Special Care Unit Placement, and Nursing Facility Characteristics on Resident Quality of Life
Our results highlight the need to ensure adequate levels of paraprofessional direct care staff and the availability of dementia-focused (SCU)s despite current constraints on long-term care funding
A Comparison and Cross-validation of Models to Predict Basic Activity of Daily Living Dependency in Older Adults
BACKGROUND: A simple method of identifying elders at high risk for activity of daily living (ADL) dependence could facilitate essential research and implementation of cost-effective clinical care programs. OBJECTIVE: We used a nationally representative sample of 9446 older adults free from ADL dependence in 2006 to develop simple models for predicting ADL dependence at 2008 follow-up and to compare the models to the most predictive published mode…
The Meaning and Significance of Self-Management Among Socioeconomically Vulnerable Older Adults
Improved understanding of the role of social context in expectations regarding aging, and awareness of and priorities for self-management, could lead to improvements in self-management support and thus chronic care outcomes
Educational Disparities in the Prevalence and Consequence of Physical Vulnerability
OBJECTIVES: The purpose of this study was to estimate educational differences in the prevalence and mortality consequence of physical vulnerability among older adults in the United States. METHODS: Data came from the 1998 and 2000 waves of the Health and Retirement Study, a nationally representative cross-sectional and prospective cohort study of community-based adults aged 65 and older. We created a physical vulnerability score from age, gender,…
Outcomes of an Exercise Program for Older Women Recruited through Primary Care
These data suggest that even suboptimal adherence to moderate-intensity exercise can yield health benefits
Body-Mass Index and 4-Year Change in Health-Related Quality of Life
The findings suggest a significant impact of BMI on changes in HRQL that is independent of disease severity and baseline HRQL

Physical Activity and Its Correlates Among Urban Primary Care Patients Aged 55 Years or Older
Given the prevalence of inactivity, knowledge deficits, and perceived barriers to physical activity, population-specific interventions may be required to improve rates of physical activity among socially disadvantaged and medically vulnerable adults
Predictors of Mobility and Basic ADL Difficulty among Adults Aged 70 Years and Older
We use data from the 1993-1995 Assets and Health Dynamics of the Oldest-Old survey to estimate predictors of onset of mobility difficulty, onset of basic activity of daily living (ADL) difficulty, and improvement to no mobility difficulty. Onset of mobility difficulty occurred among 20% of those with no difficulty at baseline (n = 2,857) and was most likely among those 85 years of age or over, women, those with a body-mass index of 30 or over, an…
A Race- and Gender-Specific Replication of Five Dimensions of Functional Limitation and Disability
Activity of daily living (ADL) and instrumental ADL (IADL) data from the 1989 National Long-Term Care Survey (NLTCS) are used to replicate two domains of functional status with a total of five dimensions among them. LISREL analysis of the 14,415 Black and White older adults obtained from the 1989 NLTCS screen replicates three ADL disability dimensions (basic, household, and advanced) with few modifications, except among Black males. Similarly, an…
The Structure of Health Status Among Hispanic, African American, and White Older Adults
Activities of daily living (ADLs), instrumental ADLs, and disability markers have traditionally been the most common indicators of functional status. The study on Asset and Health Dynamics Among the Oldest Old (AHEAD) is used to replicate a five-dimensional measurement model composed of these observable indicators among the older adult self-respondents. The items available to measure upper body disability were found wanting, but the lower body di…
US trends in disability and institutionalization among older Blacks and Whites
OBJECTIVES: This study estimated and compared the prevalence of disability and institutionalization in 1982, 1984, and 1989 among the older Black and White populations of the United States. METHODS: Data on over 1100 Blacks and 14,000 Whites in each of a series of three National Long Term Care Surveys were used. RESULTS: Diverging trends for Blacks and Whites led to statistically significant increases in the age- and sex-adjusted odds of disabili…
The effect of walking on lower body disability among older blacks and whites
OBJECTIVES. This study investigated the association between regular physical activity and risk of or increase in lower body disability in older, community dwelling Blacks and Whites. METHODS. The present study used the 1984 to 1990 Longitudinal Study on Aging, which included 413 Black and 3428 White self-respondents 70 years of age or older. Discrete-time hazard models provided estimates of the effects of self-reported walking frequency, and regu…
A Chronic Disease Score with Empirically Derived Weights
Different types of medication prescribed during a 6-month period for the treatment and management of chronic conditions were utilized in the refinement and validation of a chronic disease score. Prescription data, in addition to age and sex, were utilized to develop a chronic disease score based on empirically derived weights for each of three outcomes: total cost, outpatient cost, and primary care visits. The ability of the revised chronic disea…
Dynamics of functional impairment in late adulthood
Race, Aging, and Functional Health
This article presents evidence of a Black/White crossover in functional health. Its existence supports the hypothesis, based on selective survival, that older members of socially disadvantaged populations are relatively more physiologically robust and thus exhibit relatively favorable functional capacities. Longitudinal data on 5,150 persons 70 years of age and older shows that within young-old age groups, Blacks are more likely to experience fun…
Trajectories of Functional Impairment in Later Life
Functional impairment trajectories in late adulthood over a decade are characterized using the Longitudinal Retirement History Study (LRHS) data set. Non-linear patterns of age-related increase in impairment are documented with longitudinal data. Subsets of panelists from the initial large, nationally representative probability sample of employed males and unmarried employed females (N = 11,000) also exhibit differential non-linear trajectories o…
