The effect and expense of redemption reintegration services versus usual reintegration care for young African Canadians discharged from incarceration
African Canadians comprise 2.5% of the population, but represent 9.5% of federal inmates - an increase of 80% since 2003-2004. Recidivism among federal inmates is high (about 40%). Innovations in Research with Medically Fragile Populations
A new group of medically fragile young adults are graduating from pediatric palliative care programs with limited expectations to live beyond early adulthood, and no comparable adult services to support their complex needs. Accessing this population is difficult because of the complexity of their conditions, the extensive personal and equipment supports that limit feasibility for travel, and divergent communication abilities. Interprofessional partnerships in chronic illness care
This paper depicts a novel complexity theory-based conceptual model for IHSSP of front-line staff who provide chronic illness care. The conceptualization provides the underpinnings for a comprehensive evaluative framework for partnerships. Two partnership process measurement tools, the PSAT and TCI are valid for IHSSP process measurement with consideration of their strengths and limitations
Case management community care for people living with HIV/Aids (PLHAs)
A case management approach to support services was developed in a Toronto-based AIDS service organization (ASO) in Canada to support people living with HIV/AIDS (PLHAs) whose needs could not be addressed through usual self-directed access to services. Conceptualization and measurement of integrated human service networks for evaluation
This newly developed measure unites multiple perspectives in a comprehensive approach to the measurement of integration of human service networks. Content validity has been established. Future work should focus on further refinement of this instrument through psychometric evaluation (e.g. construct validity) in diverse networks and relating these measures of network integration to client and system outcomes
The cost of poor adjustment to chronic illness
This paper summarizes information about expenditures for people with chronic illness as a function of their psychosocial adjustment to illness. Three studies are discussed: two prospective baseline surveys and one historic cohort analytic study. Each was conducted between 1988 and 1993 in Southern Ontario, Canada in tertiary centres which were treating patients recently referred to a specialty medical clinic for one of 13 types of chronic physica…
Interactional styles of out-patients with poor adjustment to chronic illness receiving problem-solving counselling
A sample of subjects with poor to fair adjustment to chronic illness is described according to three interactional styles distinguished by patterns of beliefs about the self in relation to others along an axis of purpose, dependency and self-concept.
