Availability of Hib, Pneumococcal, Rotavirus, and HPV vaccines in China
Vaccines are cornerstones of public health, yet equitable access remains a major global challenge. While China has achieved sustained high coverage of National Immunization Program (NIP) vaccines, some WHO-recommended cost-effective non-NIP vaccines, such as Hib, pneumococcal conjugate (PCV), rotavirus, and human papillomavirus vaccines (HPV) are not universally accessible and often require out-of-pocket payment. This study examines geographic an…

Objectives This study aimed to investigate the prevalence of psychoactive substance use (PSU), identify related factors, and examine the associations among perceived social support, depressive symptoms, and PSU among men who have sex with men (MSM) in China. Methods This study recruited MSM from March 17 to May 28, 2023, via Blued, a popular geosocial networking application among MSM in China. Data on sociodemographic characteristics, sexual behav…
How do tensions between medical professionalism and financial incentives play out under case-based payment reform in China
To address rapidly increasing healthcare expenditures of social health insurance, the Chinese government has recently introduced a provider payment reform. While some studies have examined the payment reform effects, findings are mixed, and the deeper mechanisms of how these changes influence physicians' behaviours remain unclear. Drawing on semi-structured interviews conducted in 2024 with 21 hospital-based physicians from two hospitals with dif…
Enhancing human and animal health data integration and informed actions for pandemic preparedness at the primary healthcare level
A key priority for strengthening global health capacity for pandemic response is rapid risk assessment for timely, context-specific decision-making. However, integrating human and animal health data for preparedness remains a challenge, especially at the primary healthcare (PHC) level. Here we review Vietnam's pandemic response and propose a conceptual framework for improving data integration across sectors in low- and middle-income countries. De…
Pandemic preparedness in Vietnam
In this viewpoint, we explore Vietnam's health system vulnerabilities and its national response to the COVID-19 pandemic, as well as critical areas of health system resilience, including health financing, workforce distribution, information systems, and governance. While Vietnam achieved early success through strong governance and mass vaccination campaigns, the pandemic revealed weaknesses in resource procurement, workforce imbalance, and limita…
A Digital Health Intervention on Healthy Behaviors Among Men Who Have Sex with Men in China Based on Information-Motivation-Behavioral Skills Model
Background Men who have sex with men (MSM) remain significantly affected by HIV/AIDS due to high-risk sexual behaviors. The adoption rate of preventive measures like pre-exposure prophylaxis (PrEP) use and HIV testing among MSM is insufficient in China, which requires more effective prevention strategies. Objective This study aims to evaluate the feasibility and effectiveness of an Information-Motivation-Behavioral Skills (IMB) model-based digital…
A Chinese conundrum
This paper evaluates the relationship between the degree of cost-sharing and the utilization of outpatient and inpatient health services in China. Using data from the 2015 China Health and Retirement Longitudinal Study (CHARLS), we estimated the association between outpatient and inpatient service utilization and cost-sharing levels associated with outpatient and inpatient services, as well as a comparative metric that quantifies the relative cos…
Independent and joint associations of sex and birth order with non‐national immunization program vaccine coverage among Chinese children
Birth order of children was independently associated with non-NIP vaccines coverage, whereas sex showed no significant. However, sex and birth order were jointly associated with receipt of non-NIP vaccines, with later born children and girls having significantly lower coverage than first-born boys. In the context of the three-child policy in China, interventions to promote non-NIP vaccine equity should consider children's sex and birth order
Differential effect of China’s Zero Markup Drug Policy on provider-induced demand in secondary and tertiary hospitals
Following the marketization of China’s health system in the 1980’s, the government allowed public hospitals to markup the price of certain medications by 15% to compensate for reduced revenue from government subsidies. This incentivized clinicians to induce patient demand for drugs which resulted in higher patient out-of-pocket payments, higher overall medical expenditure, and poor health outcomes. In 2009, China introduced the Zero Markup Drug P…
Between now and later
In urban areas, Chinese caregivers' intent to delay vaccination in favour of 9vHPV vaccine over receiving the more accessible options was influenced by a mix of individual and contextual factors. Focussed health communication strategies are needed to accelerate HPV vaccination among adolescents
The effectiveness of pay-it-forward in addressing HPV vaccine delay and increasing uptake among 15–18-year-old adolescent girls compared to user-paid vaccination

Socio-Economic and Rural-Urban Differences in Healthcare and Catastrophic Health Expenditure Among Cancer Patients in China
Objective: In China, cancer accounts for one-fifth of all deaths, and exerts a heavy toll on patients, families, healthcare systems, and society as a whole. This study aims to examine the temporal trends in socio-economic and rural-urban differences in treatment, healthcare service utilization and catastrophic health expenditure (CHE) among adult cancer patients in China. We also investigate the relationship between different types of treatment a…
The co-management of tuberculosis-diabetes co-morbidities in Indonesia under the National Tuberculosis Control Program
TB-DM case detection significantly improved in 2019 after introducing TB-DM co-management activities in Jakarta, while gaps in TB-DM co-management existed between bacteriologically positive and clinically diagnosed TB patients, and across different types of health facilities. Collaboration between TB and DM departments should be strengthened, and more resources need to be mobilized to further improve the co-management of TB-DM in Indonesia
Ten-year impacts of China’s rural health scheme
China has made profound progress in advancing universal health coverage (UHC) over the past two decades. New Cooperative Medical Scheme (NCMS) was initiated in 2003 to provide health insurance coverage to rural population. Its benefit packages and cost-sharing mechanism have changed significantly over time. This study aims to assess the impact of changing NCMS policies on NCMS enrollees’ service utilisation, medical financial burden and equity be…
Elements and Performance Indicators of Integrated Healthcare Programmes on Chronic Diseases in Six Countries in the Asia-Pacific Region
BACKGROUND AND AIMS: Globally, hospital-based healthcare models targeting acute care, are not effective in addressing chronic conditions. Integrated care programmes for chronic diseases have been widely developed and implemented in Europe and North America and to a much lesser extent in the Asia-Pacific region to meet such challenges. We completed a scoping review aiming to examine the elements of programmes identified in the literature from sele…

Does decentralisation improve human resource management in the health sector? A case study from China
Persistent problems of access to appropriate, affordable TB services in rural China
Access to appropriate, affordable TB services is still problematic in some rural areas of China, and receipt of care and affordability declines with declining socio-economic position. These findings highlight the current shortcomings of the national TB control programme in China and the formidable challenge it faces if it is to reach all sections of the population, including the poor with the highest burden of disease
Tackling the challenges to health equity in China
Reform of how health care is paid for in China
Evaluating the financial protection of patients with chronic disease by health insurance in rural China
A significant proportion of patients with chronic diseases face catastrophic healthcare costs and these are especially heavy for the poor. The NCMS offers only a limited degree of financial protection. The heavy financial burden of healthcare for chronic disease poses an urgent challenge to the NCMS. There is an urgent need for a clear policy on how to offer financial protection to those with chronic disease
The impact of smoking and quitting on household expenditure patterns and medical care costs in China
If a household member smokes, there is less money available for commodities such as education and medical care. Medical care expenditure is substantially higher among households with quitters, as ill-health is the main reason for quitting smoking in China. Smoking impoverishes a substantial number of poorer rural households
Changes and Equity in Use of Maternal Health Care in China
Can rural health insurance improve equity in health care utilization? a comparison between China and Vietnam
China has achieved high population coverage rate over a short time period, starting with a limited benefit package. However, poor people have less benefit from NCMS in terms of health service utilization. Compared to China, Vietnam health insurance system is doing better in equity in health service utilization within the health insurance members. However with low population coverage, a large proportion of population cannot enjoy the health insura…
On residents’ satisfaction with community health services after health care system reform in Shanghai, China, 2011
The respondents showed more satisfaction with the clinical services (average score=3.79) and public health services/interventions (average score=3.79); and less satisfaction with the health insurance system (average score=3.23) and the essential drug system (average score=3.20). Disadvantaged groups showed lower satisfaction levels overall relative to non-disadvantaged groups
Controlling cost escalation of healthcare
An increasingly number of low- and middle-income countries have developed and implemented a national policy towards universal coverage of healthcare for their citizens over the past decade. Among them is China which has expanded its population coverage by health insurance from around 29.7% in 2003 to over 90% at the end of 2010. While both central and local governments in China have significantly increased financial inputs into the two newly esta…
Impact of Financial and Educational Interventions on Maternity Care
The Incoherence of China’s National Health Development Strategies
China ambitiously promised to provide safe, effective, and affordable health care services to all citizens. However, the national strategies for enhancing health remain patchy, and the policy frameworks to empower and inspire individuals and communities to pursue a healthy lifestyle are largely fragmented. The incoherency is well epitomized by China’s failure to implement key parts of the Framework Convention on Tobacco Control treaty. We seek to…
Changes in health expenditures in China in 2000s
The rapid increase of public funding to subsidize health insurance in China, as part of the reform strategy, did not mitigate the out-of-pocket payment for healthcare over the past decade. Financial burden of healthcare on the rural population increased. Affordability among the rural households with sick members, particularly in the less developed region, is getting worse. It needs effective measures on cost control including healthcare provider …
A community-based approach to non-communicable chronic disease management within a context of advancing universal health coverage in China
Paralleled with the rapid socio-economic development and demographic transition, an epidemic of non-communicable chronic diseases (NCDs) has emerged in China over the past three decades, resulting in increased disease and economic burdens. Over the past decade, with a political commitment of implementing universal health coverage, China has strengthened its primary healthcare system and increased investment in public health interventions. A commu…
Analysis of the equity of emergency medical services
In Chongqing city, equity of needs, utilization, and resources allocation of EMS is low, and the provision of such services has not met the needs of patients. To narrow the gap of equity, improvement in the capability of EMS to decrease cerebrovascular, cardiovascular, injury and poisoning cases, should be regarded as a top priority. In poor areas, allocation of facilities and human resources needs to be improved, and the economy should also be e…
The effect of NCMS on catastrophic health expenditure and impoverishment from tuberculosis care in China
The NCMS has partial effect on protecting households from CHE and impoverishment from TB care. The limited protection could be enhanced by redesigning benefit coverage to improve the "height" of the NCMS and representing fee-for-service with alternative payment mechanisms
Financial burden of healthcare for cancer patients with social medical insurance
Social economic development was not necessarily associated with total medical expense but determined the level of financial protection. The economic burden of insured cancer patients was reduced by insurance but it is still necessary to provide further financial protections and improve affordability of healthcare for cancer patients in China
