Racial/ethnic/gender-Based Differences in Health Trajectories Among American Older Adults
Health disparity by race/ethnicity or gender has been well-documented. However, few researchers have examined health outcomes based on the intersection of individuals’ race, ethnicity, and gender or investigated various health dimensions. Guided by an intersectionality framework, this study explores racial/ethnic/gender-based differences in trajectories of multiple health outcomes over a ten-year period among American older adults. Longitudinal d…
Intersectional Discrimination Attributions and Health Outcomes Among American Older Adults
Guided by an intersectionality framework, this study examined intersectional discrimination attributions and their associations with health outcomes. Older respondents (aged ≥50) from the Health and Retirement Study in 2014-2015 were included ( N = 6286). Their reasons for discrimination (age, gender, sexual orientation, race, national origin, religion, financial status, weight, physical appearance, disability, and others) were examined. Latent c…
The intersection of sociopolitical trends and Aging Network training
The Aging Network provides services to over 11 million older adults and their caregivers annually. Three national conferences (AIRS, n4a, and NASUAD) support the diffusion of critical information to members of the Aging Network. This study employed a content analysis of these national conferences held between 2009 and 2018. An a priori coding scheme including major codes and subcodes utilized in an earlier study was modified for use in the curren…
Social Work and Aging in Place
Aging in place (AIP) has become a major focus of aging policies, programs, and scholarship in recent years. This article presents a scoping review of the social work literature on AIP. Based on predefined eligibility criteria, the authors identified 22 empirical articles published between 1980 and 2014. Many of these studies focused on settings that are particularly relevant to gerontological social work practice, including congregate housing alt…
Challenges to Engaging Diverse Participants in Community-Based Aging in Place Initiatives
Naturally Occurring Retirement Community Social Service Programs (NORC programs) and Villages focus on strengthening community-based systems to support aging in place. This study examined data from a national survey of NORC program and Village leaders on participant diversity. Villages and NORC Programs are serving a majority of white participants on average. Initiatives implemented in urban locations, and with more full-time staff, reported grea…

Changes in the Medicare Home Health Care Market
BACKGROUND: The Balanced Budget Act of 1997 introduced 2 new reimbursement structures, the Interim Payment System (IPS, 1997-2000) and the Prospective Payment System (PPS, begun October 2000) for Medicare home health agencies (HHAs) under the fee-for-service program. OBJECTIVE: This article describes and compares the impact of these changes on the Medicare home health market from a period before the BBA through the IPS and PPS in relation to agen…
Tracing the History of Medicare Home Health Care
We trace key policy changes that affected use of the Medicare home health benefit from the 1980s through the prospective payment system implemented in 2000, analyzing the impact on three measures of home care use: expenditures, users and visits. We demonstrate the impact of policies generated in the legislative, the judicial, and the executive branches of government and the gaming behavior of home health agencies in response to policy changes. Ou…
Supporting Patient Autonomy
This study examines the policies and procedures that home health care agencies have developed to handle the incapacitated patient and life-sustaining treatment decisions. Data collected from a survey of 154 home health care agency directors and interviews with 92 local agency staff (including nurses and social workers) and 67 patients confirmed that directors, staff, and patients agree that patients are informed about their legal rights. When ask…
