Propensity of US Military Personnel to Seek Mental Health Care When Community Psychiatric Capacity Changes
This cohort study of the US military population suggests that active duty military personnel rely largely on military treatment facilities for their mental health care and that there are meaningful responses to military treatment facility psychiatrist capacity changes even in communities with high civilian psychiatric capacity. Realigning military treatment facility psychiatrists across communities with shortages and high-capacity military treatm…
Comparison of Long-run Trends in 30-day Readmission by Degrees of Medicare Payment Cuts
BACKGROUND AND OBJECTIVE: The Affordable Care Act enacted significant Medicare payment reductions to providers, yet long-term effects of such major reductions on patient outcomes remain uncertain. Using the 1997 Balanced Budget Act (BBA) as an experiment, we compare long-run trends in 30-day readmission across hospitals with different amount of payment cuts. RESEARCH DESIGN, SUBJECTS, AND MEASURES: Using 100% Medicare claims between 1995 and 2011…
Reductions in Medicare Payments and Patient Outcomes
BACKGROUND: The Affordable Care Act enacted significant Medicare payment reductions to providers, yet the effects of such major reductions on patients remain unclear. We used the Balanced Budget Act (BBA) of 1997 as a natural experiment to study the long-term consequence of major payment reductions on patient outcomes. OBJECTIVES: To analyze whether mortality trends diverge over the years between hospitals facing different levels of payment cuts …
Effects of Iraq/Afghanistan Deployments on Major Depression and Substance Use Disorder
Objectives. Our objective was to analyze the association between deployment characteristics and diagnostic rates for major depression and substance use disorder among active duty personnel. Methods. Using active duty personnel serving between 2001 and 2006 (n = 678 382) and deployment information from the Contingent Tracking System, we identified individuals diagnosed with substance use disorders and major depression from TRICARE health records. …
Changes in Emergency Department Access Between 2001 and 2005 Among General and Vulnerable Populations
Objectives. We analyzed how ease of geographic access to emergency departments (EDs), defined as driving time to the closest ED, changed between 2001 and 2005, and whether access deterioration was more likely to occur in vulnerable communities. Methods. We classified communities on the basis of American Hospital Association and Census data into 3 categories according to driving time to the nearest ED: no increase, less than a 10-minute increase, …
Understanding the Risk Factors of Trauma Center Closures
OBJECTIVES: We analyze whether hazard rates of shutting down trauma centers are higher due to financial pressures or in areas with vulnerable populations (such as minorities or the poor). MATERIALS AND METHODS: This is a retrospective study of all hospitals with trauma center services in urban areas in the continental US between 1990 and 2005, identified from the American Hospital Association Annual Surveys. These data were linked with Medicare c…
Out-of-Pocket Health Spending Between Low- and Higher-Income Populations
OBJECTIVE: We studied the effects of health insurance, health care needs, and demographic and area characteristics on out-of-pocket health care spending for low and higher income insured populations. MATERIALS AND METHODS: We used the 2002 National Survey of America's Families to analyze out-of-pocket health spending. People were classified into 3 levels of expenses based on their out-of-pocket health care spending and 3 levels of financial burde…
The Effects of HMO Ownership on Hospital Costs and Revenues
Characteristics of Occasional and Frequent Emergency Department Users
OBJECTIVE: The objective of this study was to explore how insurance coverage, access to care, and other individual characteristics are related to the large differences in emergency department (ED) use among the general population. MATERIALS AND METHODS: We used the 1997 and 1999 National Survey of America's Families, a nationally representative sample. People were classified into 3 ED use levels based on the number of visits over the 12 months be…
Changes in Hospital Performance after Ownership Conversions
This paper examines the effects of ownership conversions on hospital performance between 1987 and 1998 in areas of financial performance, staffing, capacity, and unprofitable care. Conversions to government and for-profit ownership both increased the profit margin: the former due to rising revenue, and the latter due to reduced operating costs and rising revenue. Hospitals that converted to for-profit ownership had the greatest reduction in staff…
Gender, High- and Low-Schedule-Control Housework Tasks, and Psychological Distress
In this analysis, with data collected from a random sample of 265 employed dual-earner couples, we estimated the association between time spent on housework tasks on one hand and psychological distress on the other. Based on the literature relating job control to mental health outcomes, we hypothesized that hours spent in low-schedule-control tasks, but not total hours spent in housework tasks or hours spent in high-schedule-control tasks, would …
