What is the youngest possible age for developing posttraumatic stress disorder? A systematic literature review
Due to growing awareness of post-traumatic reactions and development of treatments for young children (0-6 years), there is an increasing need for accurate assessment of posttraumatic stress disorder (PTSD). If the youngest children cannot manifest PTSD due to still-emerging cognitive emotional abilities, then treatments relying on those abilities may be ineffective. The objective of this review is to systematically review the literature of detai…
Exploring Treatment Response Predictors of a Parent-Led Therapist-Assisted Treatment for Childhood Trauma
Purpose: While stepped care trauma-focused cognitive behavioral therapy (SC-TF-CBT) is an effective service delivery model, understanding predictors of Step One, a parent-led therapist-assisted treatment, will help inform how to best match children at baseline to treatments. Method : Potential predictor variables were explored from 63 parent–child Step One participants with 43 responders and 20 non-responders. Baseline tailoring variables explore…
Caregiver and child agreement on traumatic events, PTSD, internalizing, externalizing, and ADHD problems in a child welfare population
Child welfare recipients face unique obstacles for accurate mental health screening and assessment. This study examined the concordance between youth and caregiver respondents on mental health problems. Agreement between youths and caregivers about traumatic event exposure was mostly Moderate or Substantial, but greater discrepancy was associated with greater child psychopathology. Both caregivers and children endorsed PTSD symptoms more often th…
Access to medicaid providers: Availability of mental health services for children and adolescents in child welfare in Louisiana
The purpose of the study was to determine the level of access that youths in child welfare have to mental health providers in a single state. Mystery shoppers called every provider publicly advertised in Medicaid managed care organization networks. Results showed that 25.4% of the advertised network was able to schedule a new appointment for a child in Department of Children and Family Services guardianship. There were 9.7 accessible providers of…
Treating PTSD in preschoolers: A Clinical Guide
Adapting cognitive-behavioral therapy (CBT) to meet the needs of 3- to 6-year-olds with posttraumatic stress disorder (PTSD), this book provides an evidence-based framework for assessment and treatment. Clear, step-by-step instructions are provided for conducting graduated exposure in a safe, developmentally appropriate fashion. Case examples and sample dialogues illustrate how to implement each component of therapy, engage both children and pare…
Stepped and Standard Care for Childhood Trauma: A Pilot Randomized Clinical Trial
Objective: This study explored the feasibility of stepped care trauma-focused cognitive behavioral therapy (SC-TF-CBT) relative to TF-CBT with children (aged 8–12). Method: Children ( N = 33) with post-traumatic stress symptoms (PTSS) were randomly assigned (2:1) to SC-TF-CBT or TF-CBT. SC-TF-CBT consisted of Step 1, parent-led therapist-assisted treatment, and Step 2 (nine TF-CBT sessions). TF-CBT consisted of 12 therapist-directed sessions. Bas…

Exploring psychopathology in early childhood: PTSD and attachment disorders in DC: 0–3 and DSM‐IV
Standard nosologies of psychiatric disorders have made preliminary efforts to accommodate developmental differences in symptom manifestation by young children. Significant gaps remain in the validation of disorders for this age group. The DC: 0–3 represents an alternative nosology for disorders of infancy that attempts to complement existing approaches and assist clinicians and researchers. In this commentary we review research on two disorders o…
A relational perspective on PTSD in early childhood
This paper describes the clinical and research evidence for the importance of the relational context of posttraumatic stress disorder in young children. We review 17 studies that simultaneously assessed parental and child functioning following trauma. In many studies, despite limitations, an association between undesirable parental/family variables and maladaptive child outcomes has been consistently found. We present a model of the parental/fami…
Symptom expression and trauma variables in children under 48 months of age
The present study compared the severity of posttraumatic symptomatology and the differential expression of the PTSD symptom clusters with six child and trauma-related variables. The independent variables were gender, age at time of trauma, acute versus repeated traumas, injured versus not injured, witnessed versus experienced, and threat to caregiver versus no threat to caregiver. The dependent variables were presence of the diagnosis of PTSD, th…
