A global scoping review of task shifting and sharing interventions to improve the mental health of people living with HIV/Aids
People living with HIV/AIDS (PLWH) often experience co-morbid/co-occurring mental health conditions, e.g., depression, anxiety, and post-traumatic stress disorder (PTSD). In resource-limited settings, where provider shortages are common, task shifting and task sharing (i.e., service delivery by non-professionals) are recommended strategies to promote access to and utilization of mental health and psychosocial support (MHPSS) services among PLWH. …
High-Acuity Alcohol-Related Complications During the Covid-19 Pandemic
In this cohort study of a national, commercially insured population, high-acuity alcohol-related complication episodes increased beyond what was expected in 4 of 18 COVID-19 pandemic months. Women aged 40 to 64 years experienced 33.3% to 56.0% increases in complication episodes in 10 of 18 pandemic months, a pattern associated with large and sustained increases in high-acuity alcohol-related liver disease complications. Findings underscore the ne…
Addressing the Covid-19 Pandemic in Populations With Serious Mental Illness
With Serious Mental IllnessThe coronavirus disease 2019 (COVID-19) pandemic will present an unprecedented stressor to patients and health care systems across the globe.Because there is currently no vaccine or treatment for the underlying infection, current health efforts are focused on providing prevention and screening, maintaining continuity of treatment for other chronic conditions, and ensuring access to appropriately intensive services for t…
Addressing Loneliness in the Era of Covid-19
Prolonged loneliness is a major yet underappreciated determinant of health, 1 placing individuals at greater risk of premature death than obesity, inadequate physical activity, or air pollution. 2 With more adults than ever before living alone 3 and a large proportion of older adults without access to online connectivity tools, the US has a dangerously fertile environment for an epidemic of loneliness caused by coronavirus disease 2019 (COVID-19)…
Effect of Medicaid Disenrollment on Health Care Utilization Among Adults With Mental Health Disorders
BACKGROUND: Medicaid is an important source of insurance coverage for those with mental health (MH) disorders in the United States. Although disruptions in Medicaid coverage are common, little is known about the dynamic relationship between Medicaid disenrollment and MH care utilization. OBJECTIVE: We estimated changes in all-cause and MH-related health care use post Medicaid disenrollment among a nationwide cohort of adults with MH disorders. SU…

Excess mortality in persons with severe mental disorders
Excess mortality in persons with severe mental disorders (SMD) is a major public health challenge that warrants action. The number and scope of truly tested interventions in this area remain limited, and strategies for implementation and scaling up of programmes with a strong evidence base are scarce. Furthermore, the majority of available interventions focus on a single or an otherwise limited number of risk factors. Here we present a multilevel…
Discontinuity of Medicaid Coverage
BACKGROUND: Gaps in Medicaid coverage may disrupt access to and continuity of care. This can be detrimental for beneficiaries with chronic conditions, such as major depression, for whom disruptions in access to outpatient care may lead to increased use of acute care. However, little is known about how Medicaid coverage discontinuities impact acute care utilization among adults with depression. OBJECTIVE: Examine the relationship between Medicaid …
Impact of Medicaid Expansion on Medicaid-covered Utilization of Buprenorphine for Opioid Use Disorder Treatment
BACKGROUND: Buprenorphine has been proven effective in treating opioid use disorder. However, the high cost of buprenorphine and the limited prescribing capacity may restrict access to this effective medication-assisted treatment for opioid use disorder. OBJECTIVE: To examine whether Medicaid expansion and physician prescribing capacity may have impacted buprenorphine utilization covered by Medicaid. RESEARCH DESIGN: We used a quasi experimental …

Insurance Status, Use of Mental Health Services, and Unmet Need for Mental Health Care in the United States
OBJECTIVE: The purpose of this study was to provide updated national estimates and correlates of service use, unmet need, and barriers to mental health treatment among adults with mental disorders. METHODS: The sample included 36,647 adults ages 18-64 (9,723 with any mental illness and 2,608 with serious mental illness) from the 2011 National Survey on Drug Use and Health. Logistic regression models were used to examine predictors of mental healt…
Involvement in the criminal justice system among attendees of an urban mental health center
Incarceration rates for people with serious mental illnesses are higher than the general population. However, research has been limited in regards to patterns of incarcerations for patients treated in public mental health settings. This study examines differences in lifetime imprisonment rates among patients of a U.S. urban Community Mental Health Center (CMHC) and national samples, within gender, race and education subgroups. Participants were i…
The Impact of Mental Illness Stigma on Seeking and Participating in Mental Health Care
Treatments have been developed and tested to successfully reduce the symptoms and disabilities of many mental illnesses. Unfortunately, people distressed by these illnesses often do not seek out services or choose to fully engage in them. One factor that impedes care seeking and undermines the service system is mental illness stigma. In this article, we review the complex elements of stigma in order to understand its impact on participating in ca…

Change in Health Insurance Coverage in Massachusetts and Other New England States by Perceived Health Status
Objectives. We examined the impact of Massachusetts health reform and its public health component (enacted in 2006) on change in health insurance coverage by perceived health. Methods. We used 2003–2009 Behavioral Risk Factor Surveillance System data. We used a difference-in-differences framework to examine the experience in Massachusetts to predict the outcomes of national health care reform. Results. The proportion of adults aged 18 to 64 years…
Addressing Public Stigma and Disparities Among Persons With Mental Illness
Stigma against mental illness is a complex construct with affective, cognitive, and behavioral components. Beyond its symbolic value, federal law can only directly address one component of stigma: discrimination. This article reviews three landmark antidiscrimination laws that expanded protections over time for individuals with mental illness. Despite these legislative advances, protections are still not uniform for all subpopulations with mental…
Do Race, Ethnicity, and Psychiatric Diagnoses Matter in the Prevalence of Multiple Chronic Medical Conditions
BACKGROUND: The proportion of people in the United States with multiple chronic medical conditions (MCMC) is increasing. Yet, little is known about the relationship that race, ethnicity, and psychiatric disorders have on the prevalence of MCMCs in the general population. METHODS: This study used data from wave 2 of the National Epidemiologic Survey on Alcohol and Related Conditions (N=33,107). Multinomial logistic regression models adjusting for …

Mental Comorbidity and Quality of Diabetes Care Under Medicaid
BACKGROUND: Patients with comorbid medical and mental conditions are at risk for poor quality of care. With the anticipated expansion of Medicaid under health reform, it is particularly important to develop national estimates of the magnitude and correlates of quality deficits related to mental comorbidity among Medicaid enrollees. METHODS: For all 657,628 fee-for-service Medicaid enrollees with diabetes during 2003 to 2004, the study compared He…
