Use of minimally invasive tissue sampling to determine the contribution of diarrheal diseases to under-five mortality and associated co-morbidities and co-infections in children with fatal diarrheal d…
Achieving the Sustainable Development Goal of reducing child mortality to Shigella /EIEC (14.0%), ST-ETEC (12.4%), rotavirus (26.4%), and adenovirus (non-40/41: 19.0%; 40/41: 5.0%). These pathogens were frequently identified as co-infections. Diarrheal disease accounted for a substantial share of child deaths across CHAMPS sites. Reducing mortality will require preventing diarrhea and addressing key contributors such as malnutrition and HIV
Barriers to contraceptive implant removal among current and prior implant users in Kisumu, Kenya
There has been increased promotion of long-acting reversible contraception (LARC) in the Global South in recent years. Studies have documented that women face barriers to removal when they desire to discontinue their LARC method, though few have quantified how often such experiences occur. We estimated the proportion of current and prior implant users who experienced or anticipated challenges with method removal using secondary data from a popula…
A comparison of all-cause and HIV cause-specific mortality among children under 5 years of age before and during Covid-19 in Kenya, 2018–2022
The impact of the COVID-19 pandemic on pediatric mortality, including measures to ensure continuity of HIV care, is not well described in Kenya. We evaluated causes of death (COD) among decedents under 5 years of age both before and during the COVID-19 pandemic in Kenya. We analyzed Child Health and Mortality Prevention Surveillance (CHAMPS) data collected in February 2018–March 2022. We describe the proportional contribution of specific conditio…
Underlying conditions associated with adverse Covid-19 treatment outcomes in selected Kenyan hospitals, October 2020 to December 2021
Background Data are limited on the impact of chronic underlying conditions on COVID-19 treatment outcomes in sub-Saharan Africa. Objectives Determine the effect of underlying conditions on COVID-19 severity and treatment outcomes in Kenya. Methods We conducted a retrospective cohort study using routine medical records from Kenya’s three large COVID-19 treatment centers. We examined two outcomes: mortality and clinical severity. Patients with lower …
Defining a screening tool for post-traumatic stress disorder in East Africa
Background: Scalable PTSD screening strategies must be brief, accurate and capable of administration by a non-specialized workforce. Methods: We used PTSD as determined by the structured clinical interview as our gold standard and considered predictors sets of (a) Posttraumatic Stress Checklist-5 (PCL-5), (b) Primary Care PTSD Screen for the DSM-5 (PC-PTSD) and, (c) PCL-5 and PC-PTSD questions to identify the optimal items for PTSD screening for …
Contribution of malnutrition to infant and child deaths in Sub-Saharan Africa and South Asia
Introduction Malnutrition contributes to 45% of all childhood deaths globally, but these modelled estimates lack direct measurements in countries with high malnutrition and under-5 mortality rates. We investigated malnutrition’s role in infant and child deaths in the Child Health and Mortality Prevention Surveillance (CHAMPS) network. Methods We analysed CHAMPS data from seven sites (Bangladesh, Ethiopia, Kenya, Mali, Mozambique, Sierra Leone and…
Identifying delays in healthcare seeking and provision
Delays in illness recognition, healthcare seeking, and in the provision of appropriate clinical care are common in resource-limited settings. Our objective was to determine the frequency of delays in the “Three Delays-in-Healthcare”, and factors associated with delays, among deceased infants and children in seven countries with high childhood mortality. We conducted a retrospective, descriptive study using data from verbal autopsies and medical r…
Adverse maternal, fetal, and newborn outcomes among pregnant women with Sars-CoV-2 infection
Causes of death identified in neonates enrolled through Child Health and Mortality Prevention Surveillance (CHAMPS), December 2016 –December 2021
Each year, 2.4 million children die within their first month of life. Child Health and Mortality Prevention Surveillance (CHAMPS) established in 7 countries aims to generate accurate data on why such deaths occur and inform prevention strategies. Neonatal deaths that occurred between December 2016 and December 2021 were investigated with MITS within 24–72 hours of death. Testing included blood, cerebrospinal fluid and lung cultures, multi-pathoge…
Provider verbal disrespect in the provision of family planning in public-sector facilities in Western Kenya
Public-sector healthcare providers in low- and middle-income countries are a primary source of family planning but their disrespectful (i.e., demeaning or insulting) treatment of family planning clients may impede free contraceptive choice. The construct of disrespect and abuse has been widely applied to similar phenomena in maternity care and could help to better understand provider mistreatment of family planning clients. With a focus on public…
Sars-CoV-2 seroprevalence in three Kenyan health and demographic surveillance sites, December 2020-May 2021
Background Most of the studies that have informed the public health response to the COVID-19 pandemic in Kenya have relied on samples that are not representative of the general population. We conducted population-based serosurveys at three Health and Demographic Surveillance Systems (HDSSs) to determine the cumulative incidence of infection with SARS-CoV-2. Methods We selected random age-stratified population-based samples at HDSSs in Kisumu, Nai…
Frequency and impact of long wait times for family planning in public-sector healthcare facilities in Western Kenya
BACKGROUND: Long wait times for family planning services are a barrier to high quality care and client satisfaction. Existing literature examining family planning wait times has methodological limitations, as most studies use data collected during exit interviews, which are subject to recall, courtesy, and selection bias. OBJECTIVE: We sought to employ a mixed methods approach to capture the prevalence, length, causes, and impacts of wait times f…
Absenteeism among family planning providers
Public-sector healthcare providers are on the frontline of family planning service delivery in low- and middle-income countries like Kenya, yet research suggests public-sector providers are frequently absent. The current prevalence of absenteeism in Western Kenya, as well as the impact on family planning clients, is unknown. The objective of this paper is to quantify the prevalence of public-sector healthcare provider absenteeism in this region o…
All-cause and cause-specific mortality rates for Kisumu County
Background Understanding the magnitude and causes of mortality at national and sub-national levels for countries is critical in facilitating evidence-based prioritization of public health response. We provide comparable cause of death data from Kisumu County, a high HIV and malaria-endemic county in Kenya, and compared them with Kenya and low-and-middle income countries (LMICs). Methods We analyzed data from a mortuary-based study at two of the l…
Evaluating the feasibility of the Community Score Card and subsequent contraceptive behavior in Kisumu, Kenya
Successful and impactful implementation of the CSC in the Kenyan context requires intensive community and provider sensitization, and pandemic conditions may have muted the impact on contraceptive uptake in this small pilot effort. Further investigation is needed to understand whether the CSC - or other social accountability efforts - can result in improved contraceptive access
Mortuary and hospital-based HIV mortality surveillance among decedents in a low-resource setting
This study shows that in low-resource settings, conducting hospital and mortuary-based surveillance is feasible and can be an alternative source of mortality data when civil registry data are inadequate
Findings from a mixed-methods journey map study of barriers to family planning in western Kenya
Our objective was to map and prioritize barriers to high-quality family planning care in western Kenya. We conducted key informant interviews ( n = 19); focus group discussions with clients ( n = 55); mystery client visits ( n = 180); unannounced visitors ( n = 120); and direct observation of client-provider interactions ( n = 256) at public facilities offering family planning. We synthesized the data into a client and a provider journey map, whi…
Development and Validation of a Sociodemographic and Behavioral Characteristics-Based Risk-Score Algorithm for Targeting HIV Testing Among Adults in Kenya
To inform targeted HIV testing, we developed and externally validated a risk-score algorithm that incorporated behavioral characteristics. Outpatient data from five health facilities in western Kenya, comprising 19,458 adults ≥ 15 years tested for HIV from September 2017 to May 2018, were included in univariable and multivariable analyses used for algorithm development. Data for 11,330 adults attending one high-volume facility were used for valid…
Screening and testing for tuberculosis among the HIV-infected
There were missed opportunities for screening and testing for TB. Screening was reduced by being on ART, having increased patient-encounters, the clinic setup, and by high patient volumes. HIV programmes should focus on quality of TB care in HIV clinics
