Folate and global health
Folate and global health review series, part 4
Background: Autoimmune diseases and bone density loss (osteopenia and osteoporosis) are chronic conditions of complex aetiology that affect diverse populations. Folate may be associated with a higher risk of these disorders due to its essential role in one-carbon metabolism required for nucleotide synthesis, homocysteine metabolism, and methylation processes. However, the evidence on this association is inconclusive. Methods: We searched MEDLINE,…
Folate and global health umbrella review series, part 1
Background: Folate is essential for normal growth and in human health throughout the lifecycle. Clinical deficiency of folate impairs DNA synthesis and results in megaloblastic anaemia, while suboptimal folate status before and in early pregnancy results in an elevated risk of neural tube defects (NTD). The evidence on the association of folate status with other health outcomes is largely fragmented and understudied. We conducted a series of umbr…
Folate and global health review series, part 3
Background: Cardiovascular and metabolic diseases account for an increasing share of morbidity and mortality globally. Folic acid supplementation has been linked to a lowered risk of stroke and some metabolic indicators due to its involvement in homocysteine and one-carbon metabolism and its role in the production of nitric oxide; however, the evidence on these associations is inconclusive. Methods: We searched MEDLINE, Embase, CINAHL, the Cochra…
Folate and global health umbrella review series, part 2
Background: Folate has been examined extensively in relation to carcinogenesis due to its role in one-carbon metabolism impacting the synthesis of DNA and RNA, methylation processes, and genomic integrity. Current evidence on the relationship between folate status and the risk of cancer is equivocal: low or deficient folate status may contribute to an increased risk of cancers, while high-dose folic acid supplementation may have adverse effects o…
Prevalence and socio-demographic predictors of malnutrition in under 5-year-olds in the Western Cape, South Africa
Nutrition policies and regulations promulgated in South Africa to address malnutrition in young children are implemented by the nine provincial governments. To facilitate malnutrition management at provincial level, robust provincial statistics for malnutrition and associated risk factors should be available. We set out to obtain this information for the Western Cape province. We surveyed 1214 under 5-year-olds, measured their weight and height/l…
Has Food Security and Nutritional Status Improved in Children 1–<10 Years in Two Provinces of South Africa between 1999 (National Food Consumption Survey) and 2018 (Provincial Dietary Intake Study (PD…
The 1999 National Food Consumption Survey in South Africa showed that food insecurity (hunger) was prevalent in households with children aged one to <10 years. A repeat of the survey in two provinces: Gauteng (GTG) and the Western Cape (WC) was undertaken in 2018. Results showed that in all domains (living areas) in GTG, food shortage prevalence decreased between 1999 and 2018, from 55.0% to 29.6% in urban informal areas, from 34.1% to 19.4% in u…
Provincial Dietary Intake Study (PDIS)
In 1999, the National Food Consumption Survey found serious risk of dietary deficiency for a number of micronutrients in 1- to 9-year-old children in South Africa. To address these shortfalls, fortification with vitamin A, thiamine, riboflavin, niacin, vitamin B6, folic acid, iron and zinc of maize meal and bread flour was made mandatory in 2003. The aim of this study was to examine micronutrient intakes of 1- to <10-year-old children after nearl…

Provincial Dietary Intake Study (PDIS)
The objective of this study was to determine the prevalence and socio-demographic predictors of malnutrition in two urbanized economically active provinces (Gauteng N = 733, Western Cape N = 593) in South Africa. A multistage stratified cluster random sampling design was applied. Fieldworkers visited homes, measured children aged 1- +2 SD ≤ +3 SD] and 7.0% obese (BAZ > +3 SD). In 5- 1 (OR 2.39), while being in the third wealth quintile was protec…
The HealthKick Study
Caregivers of children in the HK study population presented with a large number of modifiable health risk behaviors. These results highlight the importance of engaging caregivers, as part of a whole school intervention, to promote healthy eating and physical activity
Weight-loss strategies of South African female university students and comparison of weight management-related characteristics between dieters and non-dieters
Attempting weight-loss is common among female students and predicts BMI. Healthy (mainly), unhealthy and extreme weight loss methods are used. Dieters are characterized by a less realistic body image, lower body satisfaction, higher pressure to lose weight, use of informal weight-management information and a healthier life-style
Health status of primary school educators in low socio-economic areas in South Africa
The findings of this study demonstrated a number of characteristics of educators in the two study areas that may influence their risk for developing NCDs and their potential as role models for learners. These included high levels of obesity, high blood pressure, high waist circumference, high cholesterol levels, and high levels of blood glucose. Furthermore, many educators had a wrong perception of their actual body size and a lack of awareness a…
Development of a performance-rating scale for a nutrition knowledge test developed for adolescents
Objective The objectives of the present study were to (i) develop and validate a norm-referenced performance-rating scale to interpret a nutrition knowledge test developed for urban adolescents and (ii) develop a prototype for other researchers to follow when developing nutrition knowledge tests. Design For norm development the nutrition knowledge test (questionnaire) was administered to a sample representative of the questionnaire target group, …
A questionnaire for screening the micronutrient intake of economically active South African adults
Objective The aim of the present study was to develop (phase 1) and validate (phase 2) a screening questionnaire to assess the adequacy of micronutrient intake of economically active South African adults. Design For identification of indicator foods to be included in the screening questionnaire (phase 1), a comprehensive, eighty-six-item, quantified FFQ that reflected the food sources of thirteen selected micronutrients associated with the nutrit…

Objective To investigate the association between the weight status of first–year female students (FYFS) and various weight management–related characteristics to identify possible components of a weight management programme for students. Design Cross‐sectional study. Setting Female residences at a South African university. Subjects A total of 360 FYFS. Results Mean (± standard deviation (SD)) body mass index (BMI) of the FYFS was 21.8 ± 2.6 kg m −…
Obesity in South Africa
Objectives To review data on the prevalence, causes and health consequences of obesity in South Africa and propose interventions to prevent and treat obesity and related outcomes. Methods Data from existing literature were reviewed with an emphasis on changing eating and activity patterns, cultural factors, perceptions and beliefs, urbanisation and globalisation. Results of studies on the health consequences of obesity in South Africans are also …
Nutritional well-being of young children in Duncan Village, East London, South Africa
Objective The aim was to contribute to the nutritional well-being of young children living in Duncan Village by investigating factors that influence clinic attendance of mothers and to formulate recommendations for optimisation of accessibility of primary health care (PHC) clinics in the area. Design PHC clinic accessibility was evaluated by assessing the experiences of mothers who attended clinics in the area as well as the experiences of health…
The accuracy of self-reported weight by overweight and obese women in an outpatient setting
Objective: To investigate the accuracy of self-reported weight of overweight and obese women and characterisation of under-, correct- and over-reporters based on a number of related variables. Design: Weight was self-reported before entering the study. At baseline, actual weight was recorded, and demographic, health, nutritional, psychological and physical activity questionnaires were completed. Setting: A hospital outpatient department. Subjects…
Evaluation of Body Shape, Eating Disorders and Weight Management Related Parameters in Black Female Students of Rural and Urban Origins
This study examined body shape dissatisfaction, eating disorder and weight management-related parameters as well as assimilation of Western cultural norms regarding body shape in black female students of urban and rural origins. Subjects (n=180; 20 ± 4,4 years old) were weighed, their height was measured and they completed the Body Shape Questionnaire, EAT-26, Eating Inventory (restraint scale), Adolescent Self-Concept Scale and a questionnaire c…
