Prevalence and Correlates of Social Risks and Receipt of Resources Among Rural and Urban Veterans
An Innovative Nurse Navigator Approach to Screen For and Address Social Needs in a Veterans Health Administration Women’s Health Clinic
BACKGROUND: Women Veterans are the fastest-growing population in the Veterans Health Administration (VHA), but little is known about how to identify and address their social needs. This program evaluation examined the implementation of a social screening and referral initiative, Assessing Circumstances and Offering Resources for Needs (ACORN), using nurse navigators in a VHA women's health clinic. OBJECTIVES: (1) Describe the implementation proce…
Racial, Ethnic, and Sex Differences in Need and Receipt of Support for Social Needs Among Veterans
This study found that there was wide variation in the health-related social need domains in which VHA race, ethnicity, and sex subpopulations reported needing support. Applying an intersectional lens when evaluating social needs lays the groundwork for equity-guided social care interventions in the VHA

Eliminating Algorithmic Racial Bias in Clinical Decision Support Algorithms
The Veterans Health Administration uses equity- and evidence-based principles to examine, correct, and eliminate use of potentially biased clinical equations and predictive models. We discuss the processes, successes, challenges, and next steps in four examples. We detail elimination of the race modifier for estimated kidney function and discuss steps to achieve more equitable pulmonary function testing measurement. We detail the use of equity le…
VA Research and Operations Uniting to Combat Covid-19 Inequities
As novel coronavirus 2019 disease (COVID-19) began to spread across the United States in early 2020, health care systems faced extreme demands on resources. As the country's largest single-payer health care system, the U.S. Department of Veterans Affairs (VA) was uniquely positioned to study how the virus impacted different communities and work to improve care provided to all. Early on, a literature review of prior epidemics revealed that occupat…
Racial Disparities in Uterine Fibroid Treatment Among Veterans Using VA Health Care
The Effects of Residential Segregation on Black and White Mortality in the United States
Higher rates of black mortality compared to whites in the United States are longstanding and well documented. Wide variation across racial and socioeconomic groups suggests that many deaths may be preventable. We hypothesize that higher mortality for African Americans is due to the fundamental causes of structural racism and poverty. We developed a new index, the Racial Isolation of Poverty (RIP), to examine how the race/class nexus of disadvanta…
Veterans Health Administration Is Working Urgently to Reduce High Rates of Suicide Among American Indian and Alaska Native Veteran
Time Trends in Racial/Ethnic Differences in Covid-19 Infection and Mortality
Studies documenting coronavirus disease 2019 (COVID-19) racial/ethnic disparities in the United States were limited to data from the initial few months of the pandemic, did not account for changes over time, and focused primarily on Black and Hispanic minority groups. To fill these gaps, we examined time trends in racial/ethnic disparities in COVID-19 infection and mortality. We used the Veteran Health Administration's (VHA) national database of …

Racial Disparities in the Frequency of Patient Safety Events
BACKGROUND: Although there is extensive evidence of racial disparities in processes and outcomes of medical care, there has been limited investigation of disparities in patient safety. OBJECTIVE: To determine whether there are racial disparities in the frequency of adverse events studied in the Medicare Patient Safety Monitoring System. DESIGN AND SUBJECTS: Abstraction of 102,623 randomly selected charts from hospital discharges of non-Hispanic w…

Gaps in Data for American Indians and Alaska Natives in the National Healthcare Disparities Report
The aim of this study was to identify and quantify gaps in health care data for American Indians and Alaska Natives. Findings indicate that only 42% of measures of health care quality and access tracked in the National Healthcare Disparities Report could be used to assess disparities among American Indians and Alaska Natives. Patient safety data was especially limited. Data from American Indians and Alaska Natives need to be improved to allow bet…
Children??s Health Care in the First National Healthcare Quality Report and National Healthcare Disparities Report
BACKGROUND: The first National Healthcare Quality Report (NHQR) and National Healthcare Disparities Report (NHDR) are landmark events for children's health care quality and are expected to stimulate local measurement, benchmarking, and quality improvement efforts. METHOD: The authors select findings from the NHQR and NHDR, focusing on topics reflecting a range of health care and health care settings affecting children. They highlight disparities …
Preparing the National Healthcare Disparities Report
Background: Efforts to quantify, monitor, understand, and reduce disparities in health care are critically dependent on the collection of high-quality data that support such analyses. In producing the first National Healthcare Disparities Report (NHDR), a number of gaps in data were encountered that limited the ability to assess racial, ethnic, and socioeconomic disparities in health care. Objectives: The objectives of this study were to identify…
The National Healthcare Quality and Disparities Reports
BACKGROUND: Congress directed the Agency for Healthcare Research and Quality (AHRQ) to lead an effort for the US Department of Health and Human Services (DHHS) to develop 2 annual reports: a National Healthcare Quality Report (NHQR) and a National Healthcare Disparities Report (NHDR). OBJECTIVES: This article lays out key concepts, definitions, statistical methods, and findings from these first ever national reports on quality and disparities. We…
Measurement Challenges in Developing the National Healthcare Quality Report and the National Healthcare Disparities Report
OBJECTIVES: The objective of this study was to describe 2 measurement challenges faced in the development of the National Healthcare Quality Report (NHQR) and the National Healthcare Disparities Report (NHDR): the use of federal data on race and ethnicity and the selection of measures of socioeconomic status (SES). METHODS: Over 30 federal and nonfederal data systems were examined to identify measures of race, ethnicity, and SES and to evaluate t…
Racial, Ethnic, and Socioeconomic Disparities in Estimates of AHRQ Patient Safety Indicators
BACKGROUND: Patient safety events that result from the happenstance of mistakes and errors should not occur systematically across racial, ethnic, or socioeconomic subgroups. OBJECTIVE: To determine whether racial and ethnic differences in patient safety events disappear when income (a proxy for socioeconomic status) is taken into account. RESEARCH DESIGN: This study analyzes administrative data from community hospitals in 16 states with reliable …
Variation in Quality of Men??s Health Care by Race/Ethnicity and Social Class
Minority men are at a markedly elevated risk for the receipt of poor health care quality. However, generalizations about "minority" men are likely to be misleading and incomplete. There is a considerable variation in the magnitude, direction, and significance of these risks
