  • Effects of hospital and obstetric unit closures on postpartum acute care across county racial composition in rural North Carolina

Open Access•Mekhala Dissanayake, John W Jackson et al.•ARTICLE•The Journal of Rural Health•2026

PURPOSE: Hospital and obstetric unit closures are concentrated in the rural Southern United States, often where marginalized racial/ethnic groups reside and access to maternal health care is already strained. Given structural racism's role as a fundamental cause of inequities, we hypothesized that closure effects may vary by race and racial composition of county. METHODS: We used linked North Carolina birth certificates and Medicaid claims from b…

  • Impacts of Initial Prescription Length and Prescribing Limits on Risk of Prolonged Postsurgical Opioid Use

    Jessica C Young, Nabarun Dasgupta et al.•ARTICLE•Medical Care•2022•Referencias: 26

BACKGROUND: In response to concerns about opioid addiction following surgery, many states have implemented laws capping the days supplied for initial postoperative prescriptions. However, few studies have examined changes in the risk of prolonged opioid use associated with the initial amount prescribed. OBJECTIVE: The objective of this study was to estimate the risk of prolonged opioid use associated with the length of initial opioid prescribed a…

  • Single-arm Trials With External Comparators and Confounder Misclassification

    Michael Webster-Clark, Michael Webster‐Clark et al.•ARTICLE•Medical Care•2020•Referencias: 28

BACKGROUND: "Single-arm trials" with external comparators that contrast outcomes in those on experimental therapy to real-world patients have been used to evaluate efficacy and safety of experimental drugs in rare and severe diseases. Regulatory agencies are considering expanding the role these studies can play; guidance thus far has explicitly considered outcome misclassification with little discussion of misclassification of confounding variabl…

  • Comparison of Medicare Claims-based Proxy Measures of Poor Function and Associations With Treatment Receipt and Mortality in Older Colon Cancer Patients

    Sophie E Mayer, H J Tan et al.•ARTICLE•Medical Care•2019•Referencias: 18

BACKGROUND: Multiple claims-based proxy measures of poor function have been developed to address confounding in observational studies of drug effects in older adults. We evaluated agreement between these measures and their associations with treatment receipt and mortality in a cohort of older colon cancer patients. METHODS: Medicare beneficiaries age 66+ diagnosed with stage II-III colon cancer were identified in the Surveillance, Epidemiology, a…

  • Doubly Robust Estimation of Causal Effects

Michele Jonsson Funk, Daniel Westreich et al.•ARTICLE•American Journal of Epidemiology•2011

Doubly robust estimation combines a form of outcome regression with a model for the exposure (i.e., the propensity score) to estimate the causal effect of an exposure on an outcome. When used individually to estimate a causal effect, both outcome regression and propensity score methods are unbiased only if the statistical model is correctly specified. The doubly robust estimator combines these 2 approaches such that only 1 of the 2 models need be…
