Improving Care of People With Serious Medical Illness—An Economic Research Agenda for Palliative Care
In 2019, the US spent nearly $4 trillion on health care, almost 20% of the overall economy and far more than other high-income nations. The top 10% of spenders, many with serious chronic illness, account for two-thirds of this cost. Despite such high spending, many patients with serious illnesses (and their families) experience unmanaged pain and other symptoms, unmet needs, and treatment that is inconsistent with their preferences. Nearly 30% of…
Methods for Constructing and Assessing Propensity Scores
OBJECTIVES: To model the steps involved in preparing for and carrying out propensity score analyses by providing step-by-step guidance and Stata code applied to an empirical dataset. STUDY DESIGN: Guidance, Stata code, and empirical examples are given to illustrate (1) the process of choosing variables to include in the propensity score; (2) balance of propensity score across treatment and comparison groups; (3) balance of covariates across treat…
Quality of Palliative Care at US Hospices
BACKGROUND: The National Quality Forum (NQF) identified hospice services as a national priority area for health care quality improvement and endorsed a set of preferred practices for quality palliative and hospice care. This study reports the first national data regarding hospices' self-reported implementation of the NQF preferred practices and identifies hospice characteristics associated with more comprehensive implementation. METHODS: We condu…
Elevated Rates of Prolonged Grief Disorder in African Americans
The prevalence of Prolonged Grief Disorder (PGD) in non-Whites is currently unknown. This study was performed to explore the prevalence of PGD in African Americans (AAs). Multivariable analysis of two studies of recently bereaved individuals found AAs to have significantly higher rates of PGD than Whites (21% [14 of 66] vs. 12% [55 of 471], respectively; p = 0.03). Experiencing a loved one's death as sudden or unexpected was also significantly as…
Effect of Inpatient Quality of Care on Functional Outcomes in Patients With Hip Fracture
OBJECTIVES: We sought to examine the relationship between functional outcome and process of care for patients with hip fracture. RESEARCH DESIGN AND PARTICIPANTS: We undertook a prospective cohort study in 4 hospitals of 554 patients treated with surgery for hip fracture. MEASUREMENTS: Information on patient characteristics and processes of hospital care collected from the medical record, interviews, and bedside observations. Follow-up informatio…
