Decomposing gender gaps in HIV service outcomes
This study uses Population-based HIV Impact Assessments survey data to examine factors associated with gender disparities in HIV outcomes. The analysis examined the share of adult males and females living with HIV who are aware of their status, are on treatment and have achieved viral load suppression across 13 African countries. The study then used the Blinder-Oaxaca statistical method to decompose these gaps into three core elements: (1) the pa…
Supporting local ownership of transition processes
Globally, an increasing number of countries have made progress towards HIV epidemic control, yet decreasing donor funding and insufficient domestic investments threaten these strides. Within this context, the global community is interested in how to successfully transition responsibility for HIV services from donor-funded programmes to local ownership within the broader healthcare system. This analysis explores how to centre local leadership in t…
Mapping care pathways
In Uganda, where HIV prevalence remains high, innovative strategies are sought to achieve 95-95-95 targets in combating the epidemic. In 2020, Uganda joined the Activity-Based Costing and Management (ABC/M) Initiative, a multi-country effort to monitor resource allocation and funding for HIV services. As part of this Initiative, data collectors generated process maps, specific to each facility, from the data collected by observing patients as the…

Assessing the impact of the president’s emergency plan for Aids relief on all-cause mortality
This study estimated the impacts of PEPFAR on all-cause mortality (ACM) rates (deaths per 1,000 population) across PEPFAR recipient countries from 2004–2018. As PEPFAR moves into its 3rd decade, this study supplements the existing literature on PEPFAR ‘s overall effectiveness in saving lives by focusing impact estimates on the important subgroups of countries that received different intensities of aid, and provides estimates of impact for differe…
Equity of the HIV epidemic response in 13 African countries
For over 25 years, new programs to attempt to stem the HIV epidemic have been developed in Africa by country governments as well as external donors. These programs and activities have built and operated facilities, trained clinicians, financed drugs and commodities, supported and helped finance government health planning and operations, and contributed in other ways. Who has benefited from this massive mobilization? While some single country and …
National Evaluation of HIV Service Resource Allocation in Tanzania
Using time-driven activity-based costing (TDABC), we examined resource allocation and costs for HIV services throughout Tanzania at patient and facility levels. This national, cross-sectional analysis of 22 health facilities quantified costs and resources associated with 886 patients receiving care for five HIV services: antiretroviral therapy, prevention of mother-to-child transmission, HIV testing and counseling, voluntary medical male circumci…
Between a Rock and a Hard Place
BACKGROUND: Gender inequality and poverty exacerbate the burden of HIV/AIDS among women in Africa. AIDS awareness and educational campaigns have been inadequate in many countries and rates of HIV testing and adherence to condom use remains considerably low, especially among married women. We investigate whether higher HIV knowledge is equally effective in lowering risky behaviors among groups of women with different levels of wealth and agency. M…

Utilization of HIV testing and counselling services by women with disabilities during antenatal care in Uganda
Our findings revealed that women with disabilities are less likely to receive HTC service during ANC and highlighted the need for disability-inclusive HIV and reproductive health services. Government, non-governmental organizations, and other stakeholders should consider funding inclusive campaigns and identifying other mechanisms for disseminating health information and behavioral interventions to women with disabilities
Global surveillance of trends in cancer survival 2000–14 (Concord-3)
BACKGROUND: In 2015, the second cycle of the CONCORD programme established global surveillance of cancer survival as a metric of the effectiveness of health systems and to inform global policy on cancer control. CONCORD-3 updates the worldwide surveillance of cancer survival to 2014. METHODS: CONCORD-3 includes individual records for 37·5 million patients diagnosed with cancer during the 15-year period 2000-14. Data were provided by 322 populatio…
Nations within a nation
BACKGROUND: 18% of the world's population lives in India, and many states of India have populations similar to those of large countries. Action to effectively improve population health in India requires availability of reliable and comprehensive state-level estimates of disease burden and risk factors over time. Such comprehensive estimates have not been available so far for all major diseases and risk factors. Thus, we aimed to estimate the dise…
Costing essential services package provided by a non-governmental organization network in Bangladesh
The health profile of Bangladesh has improved remarkably, yet gaps in delivering quality health care remain. In response to the need for evidence to quantify resources for providing health services in Bangladesh, this study estimates unit costs of providing the essential services package (ESP) in the not-for-profit sector. This study used a stratified sampling approach to select 18 static clinics, which had fixed facilities, from 330 non-profit c…

Modelling the returns on options for improving malaria case management in Ethiopia
BACKGROUND: Diverse opinions have emerged about the best way to scale up malaria interventions. Three controversies seem most important: (1) should the scale-up focus on a broader target of febrile illness (including infectious disease and pneumonia)? (2) should the scale-up feature a single intervention or be targeted to the situation? (3) should scale-up have a preference for one kind of delivery mechanism or another? METHODS: A decision model …
