Actionable policies for high-quality integrated long-term care
Background: The EU funded Project Laurel aims to understand regional differences in integrated long-term care (I-LTC) provisions in Europe, along with the associated support solutions. The Laurel project's primary objective is to develop actionable policies that can effectively respond to the challenges of growing demand, economic constraints, accessibility, affordability, workforce shortages, and the need for systemic reforms in LTC. Integrated long term care services
Background: As Europe faces demographic ageing and a growing number of people with long-term care (LTC) needs, systems must evolve to offer more integrated, person-centred, and sustainable solutions. The EU-funded Laurel project aims to identify regional differences in integrated long-term care (I-LTC) provision, while developing actionable policies to address pressing challenges—such as workforce shortages, affordability, access inequalities, an…
Digital Medicines – Digital tools and data supporting improvements in medication prescribing and therapy in a complex world
Digital tools and data play a key role in achieving more effective and safe prescribing and pharmacotherapy. But when comparing the situation across many different countries, there is still considerably room for improvement when it comes to large-scale implementation of digitally supported care processes or even to the systematic exchange of experience. Promising examples of the integration of digital tools and data into prescribing and pharmacot…
Serendipity in integrated care
Our research team is currently working on the use of AI-enhanced algorithms to predict oxygen needs in patients on ambulatory oxygen therapy. The training of the algorithms, for which rich data sets are required, was a challenge for engineers. The solution proposed was to build an automated version of the 6MWT (named A-6MWT). This serendipitous development is currently promoting the revision of current practices, including a more integrated care …
Actionable policies for high-quality integrated long-term care
Introduction: The EU subsidised LAUREL-project (actionable policies for adopting high-quality integrated long-term care) aims to understand the regional differences in integrated long-term care (I-LTC) provisions in Europe, along with the associated support solutions. LTC services are often funded and organized across various levels and services and pose challenges to care coordination and continuity. However, reforms are underway to integrate th…
Spanish Validation of the “User Reported Measure of Care Coordination” Questionnaire for Older People with Complex, Chronic Conditions
The adapted version of the "User Reported Measure of Care Coordination" into Spanish proved to be a practical tool for use in our daily practice and an efficient instrument for assessment of care coordination in chronic, complex conditions in older people across services and levels of care
Project Integrate
This framework can be used as a stand-alone or combined tool to guide managers to plan and evaluate the care process design of people-centred integrated care services; future work should apply this tool to other settings
Getting the system to invest in integrated care - lessons from the integration stayers
The International Journal of Integrated Care (IJIC) is an online, open-access, peer-reviewed scientific journal that publishes original articles in the field of integrated care on a continuous basis.IJIC has an Impact Factor of 5.120 (2020 JCR, received in June 2021)
Implementation of Home Hospitalization and Early Discharge as an Integrated Care Service
Home Hospitalization has proven efficacy, but its effectiveness and potential as an Integrated Care Service in a real world setting deserves to be explored. Objective: To evaluate implementation and 10 years follow-up of Home Hospitalization and Early Discharge as an Integrated Care Service in an urban healthcare district in Barcelona. Methods: Prospective study with pragmatic assessment. Patients: Surgical and medical acute and exacerbated chronic…

Riding the winner across the finishing line
Background: Better integrated clinical models of care with close cooperation between hospital-based specialists and community practitioners are fundamental to effective chronic disease management. Yet despite integrated care being an international priority for over 20 years, sustained at-scale integration outcomes are still uncommon. Aims and Objectives: To allow clinician researchers with over a decade of service implementation at scale in Europ…
Integrated care services
The four integrated care services showed high potential to enhance health outcomes with cost-containment. Change management, technological approach and legal issues were major factors modulating the success of the deployment. The project generated a business plan to foster service sustainability and health innovation. Deployment strategies require site-specific adaptations
Assessing deployment and growth of integrated care
Integrated Care in one of the six specific priority actions of the European Innovation Partnership on Active and Health Ageing planned for the period 2012-15. The B3 action group on integrated care was established in response to this priority action to assist regions and delivery organisations in addressing the key challenges they have identified in implementing integrated care. At present, the group encompasses more than 150 institutions ranging…
