Improving Comprehensive Care with better information overview: A Randomized Controlled Trial
Background: This study investigates the effect of a digital tool designed to improve information synthesis and workflow efficiency in the context of patients with complex multimorbidity. Approach: Best practice for patients with complex multimorbidity is person-centered, integrated, and proactive care. To provide such care, clinicians need an overview of the patient’s medical history and the person’s needs, wishes, and preferences (patient goals)…
From fragments to overview – Challenges of gathering electronic health data to provide integrated care
Delivery of integrated care (IC) depends on accessing relevant patient information. The goal is for information to follow patient journeys, i.e., "one citizen - one health record". Today electronic solutions mirror the care silos they belong to. As a result, healthcare data is scattered across organizations and Electronic Health Record (EHR) systems. Thus, to create an overview of the patient journey across silos, EHR data from all relevant silos…
Digital presentation of the patient voice in pursuit of person centred, integrated and proactive care
Background: There is an overall agreement that it is important to shift focus from diagnosis-centric to person-centric care, where patients are active participants in healthcare services they receive. However, to support person-centric care for patients with complex long-term needs, the voice of the person must be present also in the electronic health record (EHR). However, the literature on how to design a digital "person voice" which captures "…
Reaching for the Quadruple Aim – How to design effective whole system health service improvement
Aim:In this workshop we will present ideas and discuss how to make health service improvements that reach for the Quadruple Aim.Background:Despite large efforts and many studies, the research community still does not understand how to reach Berwick's and Bodenheimer’s quadruple aims for care (better patient experience, health and function, cost-benefit ratio and professional experience), with any degree of predictability (1). In a recent review o…
Medical pluralism in the aftermath of cancer: Health Seeking Actions and Cancer Patients' Shaping of Trajectories to Healing
Improved treatment methods for cancer are increasing the number of survivals in Norway. In turn, the group of people struggling with late effects after the treatment is growing. Late effects could be physical, psychological or existential conditions caused by treatment or the experience of illness. This qualitative study explores health-seeking actions among nine Norwegian people with cancer, and how they shape their trajectories to healing. Vari…
Engaging and empowering citizens with long-term conditions: How to define personal goals and how to support the goal achievement with technology
The International Journal of Integrated Care (IJIC) is an online, open-access, peer-reviewed scientific journal that publishes original articles in the field of integrated care on a continuous basis.IJIC has an Impact Factor of 5.120 (2020 JCR, received in June 2021)
Policies Make Coherent Care Pathways a Personal Responsibility for Clinicians: A Discourse Analysis of Policy Documents about Coordinators in Hospitals
We suggest that the policies' construction of the 'problem' as a responsibility issue, result in that neither diversity of patients' coordination needs, nor heterogeneity of hospital contexts regarding necessary system support for coordinators, is set on the agenda. Adoption of rhetoric from diagnosis-specific standardized pathways obscures unique challenges in creating coherent pathways for patients with complex needs
Person-centered coordination in hospitals: A review at the crossroads of research and policy
Introduction: Norway has sought to address challenges of discontinuity of care through legislation of a personal coordinator at both primary and speciality levels of care. In this paper, we highlight the "personal care coordinator" (PCC) which was introduced in specialized health care in 2012. The target group is patients with long-term needs of care from different departments and professions, independent of medical condition(s). The stated aim o…
What are the goals of care in individual Patient Pathways? An qualitative analysis of how different health concepts may affect goals for care
Introduction: The individual care process for patients with long-term conditions is ever more a collaborative project between multiple professions, multiple organizations, guidelines, the patient and his/her significant others. This care process is here called the individual Patient Pathway (iPP). Different providers contribute to the iPP with a diverse and complementary set of skills and competencies, and can in many ways be conceptualized as a …
Patients Judgment of Their Trajectories
Objective: to investigate how patients judge the specific aspects of their trajectories through the health care services Method: Data on contacts with a range of health care services for all inhabitants of five municipalities in Central Norway will be used to present patients with their trajectories during 2012 and 2013. A total of 13000 patients with chronic conditions will be asked to comment on and give their judgment of the different contacts…
