Mapping the research, policy, and clinical management landscape of post-tuberculosis lung disease
Tuberculosis (TB) can result in debilitating long-term health consequences, most notably post-tuberculosis lung disease (PTLD). However, these conditions remain insufficiently integrated into global policy and routine care, contributing to fragmented care for millions of TB survivors. This scoping review maps the current landscape to identify critical gaps and support the harmonisation of research, policy, and practice. We conducted a scoping rev…
Evaluating the acceptability, feasibility, and implementation fidelity of a multipurpose mobile health (mHealth) app used for Tuberculosis (TB) contact tracing by Ward-Based Outreach Teams (WBOTs) in …
Mobile health (mHealth) technologies are increasingly used to support community-based healthcare. However, their real-world impact often remains unclear. Understanding implementation factors is essential for advancing their use and achieving meaningful health outcomes. We evaluated an mHealth tool (AitaHealth) after customising its modules and workflows for household Tuberculosis (TB) contact tracing and other community-based data collection by c…
Insights on the implementation strategies for a potential new tuberculosis vaccine in South Africa
Several tuberculosis (TB) vaccines intended for adults and adolescents are in late-stage clinical trials, but there is limited research into how a new TB vaccine would be introduced. South Africa is at the forefront of TB vaccine research, with involvement in the clinical trials of leading candidates. We sought to understand what the priorities and implementation approach of a potential new TB vaccine in South Africa would be. We conducted semi-s…
Is global health security worth 0.01% of our gross domestic product
One year on - the long-term impact of Covid-19 pandemic and government restrictions on the health-seeking behaviour, financial security and mental health of TB survivors

Evaluating community knowledge of tuberculosis preventive therapy in rural South Africa
Rural South African community members demonstrated poor TB knowledge. Community members with good knowledge of TB were also aware of TPT. Greater community-level public health education and individual-level counseling efforts are needed to facilitate TPT expansion and implementation
Frequency of tuberculosis symptoms at repeat visits amongst adults attending for HIV care in Gauteng province, South Africa
W4SS symptoms remained relatively common at repeat visits among people without TB in HIV care. Repeat testing of symptomatic patients with Xpert MTB/RIF would have substantial resource implications. Sensitive and specific TB screening tools which are not dependent on symptoms are needed
Feasibility and acceptability of the smart pillbox and medication label with differentiated care to support person-centered tuberculosis care among Ascent trial participants – A multicountry study
Introduction: Digital adherence technologies (DATs) can offer alternative approaches to support tuberculosis treatment medication adherence. Evidence on their feasibility and acceptability in high TB burden settings is limited. We conducted a cross-sectional survey among adults with drug-sensitive tuberculosis (DS-TB), participating in pragmatic cluster-randomized trials for the Adherence Support Coalition to End TB project in Ethiopia (PACTR2020…
Modelling the epidemiological and economic impact of digital adherence technologies with differentiated care for tuberculosis treatment in Ethiopia
BACKGROUND: Digital adherence technologies (DATs) with associated differentiated care are potential tools to improve tuberculosis (TB) treatment outcomes and reduce associated costs for both patients and healthcare providers. However, the balance between epidemiological and economic benefits remains unclear. Here, we used data from the ASCENT trial to estimate the potential long-term epidemiological and economic impact of DAT interventions in Eth…
Burden of tuberculosis in underserved populations in South Africa
Small area analysis of HIV viral load suppression patterns in a high priority district (2012–2016), South Africa
Globally, high viral load (VL) suppression rates are indicators of successful HIV treatment programs. Evaluation of these programmes at lower levels is likely to highlight variations that are masked at the provincial or national levels. This ecological study used routinely collected clinical and surveillance data on the HIV programme from 88 sampled Ekurhuleni wards. Between January 2012 and December 2016, 26 222 HIV VL tests for 2817 patients we…
Acceptability of using the medication monitor and experience of a differentiated care approach for TB treatment adherence among people living with TB in South Africa
Background The introduction of digital adherence technologies (DATs) such as medication monitors in tuberculosis (TB) programmes supports treatment adherence among people with tuberculosis (PWTB). We evaluated the acceptability of using medication monitors (Wisepill evriMED) prompting a stepwise differentiated care approach (DCA), involving short message service (SMS), phone calls, home visits and motivational counselling, among PWTB in South Afr…
A qualitative exploration into the presence of TB stigmatization across three districts in South Africa
TB stigma contributes to suboptimal case finding and care at the community level in South Africa. Interventions to combat stigma, such as community and individual education campaigns on TB treatment and transmission as well as the training of health care workers on stigma and stigmatization are needed to prevent discrimination and protect patient confidentiality
Assessment of facility-based tuberculosis data quality in an integrated HIV/TB database in three South African districts
Background Assessment of data quality is essential to successful monitoring & evaluation of tuberculosis (TB) services. South Africa uses the Three Interlinked Electronic Register (TIER.Net) to monitor TB diagnoses and treatment outcomes. We assessed the quality of routine programmatic data as captured in TIER.Net. Methods We reviewed 277 records from routine data collected for adults who had started TB treatment for drug-sensitive (DS-) TB betwe…
Understanding factors influencing utilization of HIV prevention and treatment services among patients and providers in a heterogeneous setting
Despite advances made in HIV prevention and treatment interventions in South Africa, barriers to their utilization continue to exist. Understanding perspectives from patients and providers of healthcare can shed light on the necessary strategies to enhance uptake of HIV services. A cross-sectional qualitative study was conducted in July 2020 in Ekurhuleni District. Based on HIV prevalence estimates from a national survey, male condom use coverage…
Factors associated with low tuberculosis preventive therapy prescription rates among health care workers in rural South Africa
BACKGROUND: Despite extensive rollout of tuberculosis preventive therapy (TPT) in South Africa to reduce the incidence of tuberculosis among people living with HIV (PWH), rates of initiation and completion have remained suboptimal. OBJECTIVE: This study aimed to identify factors associated with low TPT prescription rates among health care workers (HCWs) in rural South Africa. METHODS: A cross-sectional study was conducted using an anonymous 39-it…
Estimating the yield of tuberculosis from key populations to inform targeted interventions in South Africa
INTRODUCTION: Tuberculosis (TB) case finding strategies are recommended to increase yield for TB in key populations. Several key populations are identified in the literature, but techniques for estimating yield and prioritising interventions are needed. METHODS: We conducted a scoping review of existing evidence on TB burden to assess contribution of key populations to the TB epidemic in South Africa. Reports, articles and conference abstracts fr…
They are inconveniencing us’ - exploring how gaps in patient education and patient centred approaches interfere with TB treatment adherence
Limited occupational opportunities, fear of disclosure and stigmatization all contributed to treatment LTFU. Clinicians concurred that poor patient understanding of TB and that biomedical management lacking a psycho-social dimension further exacerbated the poor treatment outcome. TB remains a social disease, the successful management of which hinges on patient-centred care
Observational study of continuity of HIV care following release from correctional facilities in South Africa
The results of our study are the first description of retention in care following correctional facility release from an African setting and indicate high levels of attrition during the transition from correctional facility to community care. Initiating ART within a correctional facility did not impair post-release linkage to care
Knock Knock”
HHCs are supportive of HHCT and felt empowered by receiving TB education that ultimately allowed them to better understand and care for PLTB. HHCs were supportive of screening children for TB at home. Future HHCT activities should consider raising community awareness on the benefits of TB contact tracing at households
Youth Preferences for HIV Testing in South Africa
Is HIV Post-test Counselling Aligned with Universal Test and Treat Goals? A Qualitative Analysis of Counselling Session Content and Delivery in South Africa
Enhancing value and lowering costs of care
While interventions to improve HIV linkage and retention in care exist, none have demonstrated results sufficient to reach UNAIDS 90-90-90 goals. We explored values and costs of seeking clinical care through testing three strategies to improve linkage to care: Point of care CD4 testing alone (POC-CD4), POC-CD4 combined with transportation support and combined with care facilitation. We conducted in-depth interviews with participants and transcrib…

Feasibility and acceptability of a specialist clinical service for HIV-infected mineworkers in South Africa
Occupational settings offer an ideal opportunity to provide preventive health services for HIV-infected workers. A specialized clinic was established in a mining hospital in the Free State, South Africa, with the primary aim of delivering preventive therapy such as isoniazid to those at high risk of tuberculosis (individuals with HIV infection or silicosis), and cotrimoxazole to those at highest risk for opportunistic infections. The clinic desig…
Evaluation of antiretroviral therapy (ART)-related counselling in a workplace-based ART implementation programme, South Africa
Counselling about antiretroviral therapy (ART) is thought important to prepare patients for treatment and enhance adherence. A workplace-based HIV care programme in South Africa instituted a three-step ART counselling protocol with guidelines prompting issues to be covered at each step. We carried out an early evaluation of ART counselling to determine whether patients understood key information about ART, and the perceptions that patients and he…
Establishing a workplace antiretroviral therapy programme in South Africa
Ways to expand access to antiretroviral treatment (ART) in low income settings are being sought. We describe an HIV care programme including ART in an industrial setting in South Africa. The programme uses guidelines derived from local and international best practice. The training component aims to build capacity among health care staff. Nurses and doctors are supported by experienced HIV clinicians through telephone consultation and site visits.…
"That is why I stopped the ART"
Given the reported uncertainty about the existence of HIV disease and the use of traditional medicines while on ART, despite a programme emphasising ART counselling, there is a need to find effective ways to support adherence to ART even if the individual does not accept biomedical concepts of HIV disease or decides to use traditional medicines. Additionally, providers should identify ways to minimize barriers in communication with patients with …
Factors influencing uptake of HIV care and treatment among children in South Africa – a qualitative study of caregivers and clinic staff
Despite antiretroviral therapy rollout in South Africa, fewer children than expected are accessing HIV care services. Our objectives were to describe barriers and facilitators of uptake of HIV care among children. Our study involved six private-sector clinics which provide HIV care free-of-charge in and around Gauteng province, South Africa. In-depth interviews were conducted in July 2008 with 21 caregivers of HIV-infected children attending thes…
Low haemoglobin predicts early mortality among adults starting antiretroviral therapy in an HIV care programme in South Africa
The high mortality rate in the first 3 months underlines the need for earlier HIV diagnosis so that ART can be initiated earlier. Low haemoglobin and low CD4 count are both strong predictors of mortality, and could be used to identify individuals at high risk who might benefit from intensive case management
Contrasting predictors of poor antiretroviral therapy outcomes in two South African HIV programmes
Baseline predictors of poor treatment outcomes were largely unique to each programme, likely reflecting different populations and pathways to HIV care. In the workplace, active promotion of HIV testing may have extended ART to individuals who, without provider initiation, would not have spontaneously sought care. As provider-initiated testing makes ART available to individuals less motivated to seek care, patients may need additional adherence su…
Tuberculosis active case finding
