How do patients expect the mental health service system to act? Testing the WHO responsiveness concept for its appropriateness in mental health care
Decisions and advice about infant feeding
What are women's practices and experiences? And what are pregnant women being told about risks through breastfeeding in an area of high HIV seroprevalence? This work sets out to explore these two principal questions in terms of the implications for women, and for international and national policy guidelines on infant feeding. During a two-year sociological study in KwaZulu-Natal, semi-structured questionnaires, individual and group interviews, an…
Policy
This book sets out to chart a new domain of anthropological enquiry, the anthropology of policy. We ask: how do policies ‘work’ as instruments of governance, and why do they sometimes fail to function as intended? What are the mobilizing metaphors and linguistic devices that cloak policy with the symbols and trappings of political legitimacy? How do policies construct their subjects as objects of power, and what new kinds of subjectivity or ident…
Experiences of breastfeeding and vulnerability among a group of HIV-positive women in Durban, South Africa
While international guidelines are currently being drawn up about HIV and infant feeding practices, and national and regional guidelines are under discussion in South Africa, there have been remarkably few studies that have sought to elicit HIV-positive mothers' experiences of breastfeeding and of paediatric infection. There is an urgent need to document this 'grass roots' knowledge in different sites, and for this data to be used to inform polic…
Reconceptualising issues around HIV & breastfeeding advice
This article is concerned with the dynamics between health care workers and pregnant women, and advice given to women about mother‐to‐child transmission (MTCT) through breastfeeding in KwaZulu‐Natal (KZN). Using ethnographic methods, it explores issues relating to HIV and infant feeding in settings where a period of breastfeeding is expected. The anti‐baby milk action of the 1970s remains an important point of reference, which has profoundly shap…
Le sida et les employées de maison au Kwazulu-Natal en Afrique du Sud
In the post-apartheid South Africa, the importance of gender have been recognized in the spread of HIV. But the author shows that domestic workers of the rural Kwazulu-Natal, who often are expected to care for HIV symptomatic people in their employer's household, enjoy very limited rights and no education about care and about protecting themselves
Confidentiality and HIV status in Kwazulu-Natal, South Africa
This article provides a contextualized comparison and analysis of the former Kwazulu and the new Kwazulu-Natal policy documents on HIV confidentiality, the differing practices within the region, and their implications for support and gender. It is based on interviews with key players in the regional NACOSA (National AIDS Convention of South Africa), and participation in meetings between August and November 1995. The main division is between those…
Women at Risk
AIDS in Africa is a gender, development and rights issue involving power and differential access to resources. The risk situations for women in stressed development contexts of war, destabilisation and displacement, and the many contexts of transactional sex, are poorly understood by policy makers and the medical community. The dominant epidemiological paradigm has focused on female 'prostitutes' in a number of African cities. The limitations of …
The competing discourses of HIV/Aids in sub-Saharan Africa
Thank God I Said No to Aids
In this presentation, which reports on work in progress, I discuss the different discourses and changing representations of the Ugandan official AIDS Control Programme (ACP) and those of the churches. There would appear to be two phases. In the first phase, the differing verbal strategies used by the ACP and the churches and the tensions between them are encapsulated in their respective slogans: Love Carefully of the ACP and Love Faithfully of th…

The Nature of the Right
This volume challenges and extends the definition of right and right-wing discourse as traditionally conceived in male scholarship. The eleven papers share a common perspective: a critique of the ideology of 'natural difference' as the basis for oppression of the dominated group. In a radical feminist analysis, the relation of domination between the sexes is seen as central to the projects of the right, in which the constructions of 'nations', 'r…

Sabra and Shatila
The F family used to live in Tel al-Zaatar. When the camp was destroyed in 1976, Sobhia F lost her husband and one of her sons. She then went to live in Shatila with her remaining children. Today, Sobhia F is still living in Shatila with what is left of her family: her eldest daughter, Wasfia, who has three children, her other three daughters, Khadija, who is 22, Sawson, 12, Zeinab, II, and the only boy who was not killed, Adel, aged 7. Her mothe…
