The present study addresses the theme of public policies related to accessibility for the deaf in the health area, considering as a structuring axis the use of sign language as a fundamental means of communication to guarantee communication accessibility, comprehensive health actions for this population segment.The aim of this thesis is to historically analyze proposals related to health and accessibility for the deaf in public policies in the city of São Paulo, from 1988 to 2020.To support the understanding of deafness, considering the heterogeneity of its manifestations, we present two main conceptions, the clinicaltherapeutic one, based on the biomedical model, and the socio-anthropological one, with influences from the social model of understanding disabilities.Taking the socio-anthropological conception as a reference for analysis, deafness is guided by linguistic, cultural and identity differences that make the deaf subject interact with the world through the expression of a visual-spatial language, sign language, as first language.The theoretical-methodological reference that guides the study comprises a historical perspective, based on the History of the Present Time, facing the subjects as traces of their historicity for and in the present, allowing to identify political, social and ideological influences in the configuration of public policies, as well as the participation of deaf subjects in these constructions.Sources for the analysis were official and technical documents in the area of health and people with disabilities (federal and in the municipality of São Paulo), as well as reports from National Health Conferences and Municipal Health Conferences in São Paulo, related to other documental and bibliographical sources that allowed to deepen the contextual analyzes related to the themes listed for the research.The analyzes point to the construction of the Brazilian deaf social movement in parallel with the expansion of the social movement of people with disabilities, resulting in the active participation of deaf people in claiming rights and citizenship, including in the area of health.Public policies for people with disabilities in the area of health began to be characterized in an institutionalized way by the State from the end of the 1980s, with advances during the 1990s, but were consolidated in a more robust way only after the 2000s, notably since the enactment of the Viver sem Limite Plan, in 2011, when actions in different areas were contemplated, including health and accessibility.The tendency of consolidation of municipal public policies in São Paulo follows the national movement, contemplating the creation and expansion of specific actions of the municipality, such as the municipal law for the recognition of the Brazilian Sign Language (Libras), the Municipal Program for the Rehabilitation of the Person with Physical and Hearing Disabilities, increasing the reach of the Specialized Rehabilitation Centers (CER) and the implementation of Libras Intermediation Centers (CIL).The Health Conferences proved to be a fundamental instrument to guide the health needs of the deaf population and people with disabilities in general, who, repeatedly, claim for better conditions of accessibility and social participation.Despite the legal advances regarding the conquest of the right to health, public policies emphasize access to hearing technologies and rehabilitation, with a predominance of clinical -therapeutic understanding of deafness, to the detriment of the effectiveness of communication accessibility as a potential for access to care in health and guaranteeing the scope of integrality and equity rights proposed by the Brazilian Public Health System (SUS
