US trends in social isolation, social engagement, and companionship ⎯ nationally and by age, sex, race/ethnicity, family income, and work hours, 2003–2020
Social connectedness is essential for health and longevity, while isolation exacts a heavy toll on individuals and society. We present U.S. social connectedness magnitudes and trends as target phenomena to inform calls for policy-based approaches to promote social health. Using the 2003-2020 American Time Use Survey, this study finds that, nationally, social isolation increased, social engagement with family, friends, and 'others' (roommates, nei…
Understanding how psychosocial factors relate to seeking medical care among older adults using a new model of care seeking
Perinatal Health Outcomes Following a Community Health Worker–Supported Home-Visiting Program in Rochester, New York, 2015–2018
We evaluated the effectiveness of a community health worker–supported home visitor program on perinatal outcomes of 455 at-risk pregnant women with program data merged with electronic medical records from July 2015 through October 2017 in Rochester, New York. Program participants had fewer adverse outcomes than did nonparticipants, including lower rates of preterm birth (12% vs 20%; χ 2 , P = .05) and low birth weight (14% vs 22%; χ 2 , P = .05).…
Least Squared Simulated Errors
Estimation by minimizing the sum of squared residuals is a common method for parameters of regression functions; however, regression functions are not always known or of interest. Maximizing the likelihood function is an alternative if a distribution can be properly specified. However, cases can arise in which a regression function is not known, no additional moment conditions are indicated, and we have a distribution for the random quantities, b…
Understanding Statistical Testing
Statistical hypothesis testing is common in research, but a conventional understanding sometimes leads to mistaken application and misinterpretation. The logic of hypothesis testing presented in this article provides for a clearer understanding, application, and interpretation. Key conclusions are that (a) the magnitude of an estimate on its raw scale (i.e., not calibrated by the standard error) is irrelevant to statistical testing; (b) which sta…
Predictors of Avoiding Medical Care and Reasons for Avoidance Behavior
BACKGROUND: Delayed medical care has negative health and economic consequences; interventions have focused on appraising symptoms, with limited success in reducing delay. OBJECTIVE: To identify predictors of care avoidance and reasons for avoiding care. METHODS: Using the Health Information National Trends Survey (2007), we conducted logistic regressions to identify predictors of avoiding medical visits deemed necessary by the respondents; and, w…
Making Difficult Decisions
Objectives. We investigated how quality of care affects choosing a nursing home. Methods. We examined nursing home choice in California, Ohio, New York, and Texas in 2001, a period before the federal Nursing Home Compare report card was published. Thus, consumers were less able to observe clinical quality or clinical quality was masked. We modeled nursing home choice by estimating a conditional multinomial logit model. Results. In all states, con…
Factors Associated With Emergency Department Use Among the Rural Elderly
CONTEXT: Emergency Department (ED) use among the rural elderly may present a different pattern from the urban elderly, thus requiring different policy initiatives. However, ED use among the rural elderly has seldom been studied and is little understood. PURPOSE: To characterize factors associated with having any versus no ED use among the rural elderly. METHODS: A cross-sectional and observational study of 1,736 Medicare beneficiaries age 65 and …

Improving Risk Adjustment for Medicare Capitated Reimbursement Using Nonlinear Models
OBJECTIVES: This article compares a linear risk-adjusted model of medical expenditures for Medicare patients with a model that explicitly account for skewness in distribution of expenditures. METHODS: A model of expenditures and a model of the square root of expenditures, each expressed as linear combinations of risk adjusters, are estimated using data from the 1992 through 1994 Medicare Current Beneficiary Surveys. Five sets of risk adjusters ar…
