Methodological challenges in researching activism in action
Civil society engagement around health care and population health improvement is an important driver towards Health for All. Research can improve the effectiveness of health activism by examining the resources, structures and strategies of civil society engagement. However, research to support such engagement faces epistemological and methodological challenges which call for specific research strategies.A four year multi-country study was underta…
Capitalism, imperialism and class
Four recent books dealing with the political economy of global health are reviewed. Drawing on the material covered in these sources it is argued that the concepts of capitalism, imperialism and class (at the national and global levels) are fundamental to a critical public health in the present era of economic globalisation
Governance and policy capacity in health development and implementation in Australia
This article examines policy capacity in the context of Australian health authorities in relation to the challenges confronting policy workers and senior managers in these agencies. The article reports the challenges identified by our informants in relation to political structures, roles and responsibilities of other portfolios and governments, interactions between policy officials and with the minister and the minister's office, whole of governm…
The potential for multi-disciplinary primary health care services to take action on the social determinants of health
The absence of systematic supportive policy, frameworks and structure means that it is hard for PHC services to act on the Commission on the Social Determinants of Health's recommendations. Our study does, however, provide evidence of the potential for PHC services to be more responsive to social determinants given more support and by building alliances with communities and social movements. Further research on the value of community control of P…
Personnel reform in Chinese hospitals
Health care in China is plagued with problems, including barriers to access, maldistribution of resources, inefficiencies and uneven quality. The management of human resources in the health care system has a significant effect on what happens in the clinic and the ward. The management of staff appointments and the remuneration system are two critical areas that remain problematic although they have received government attention. We argue that new…
Factors affecting catastrophic health expenditure and impoverishment from medical expenses in China
OBJECTIVE: To assess the degree to which the Chinese people are protected from catastrophic household expenditure and impoverishment from medical expenses and to explore the health system and structural factors influencing the first of these outcomes. METHODS: Data were derived from the Fourth National Health Service Survey. An analysis of catastrophic health expenditure and impoverishment from medical expenses was undertaken with a sample of 55 …
Negotiating Tensions in Developing Organizational Policy Capacity
This article explores how organizational policy capacity can be developed, drawing on a study conducted in a large human services agency in Australia. Building policy capacity within government agencies is widely acknowledged as important for successfully responding to complex policy problems. The existing literature suggests a range of strategies for building organizational capacity. Findings from interviews with policy workers support the princ…
Revitalising primary healthcare requires an equitable global economic system - now more than ever
The promised revitalisation of primary healthcare (PHC) is happening at a time when the contradictions and unfairness of the global economic system have become clear, suggesting that the current system is unsustainable. In the past two decades, one of the most significant impediments to the implementation of comprehensive PHC has been neoliberal economic policies and their imposition globally. This article questions what will be required for PHC …
Micro macro integration
The idea of micro macro integration (MMI) provides a useful framework for thinking about primary healthcare (PHC) and community development in health (CD). PHC and CD are important strategies for addressing the structural determinants of health. They are each based on a powerful logic and have a significant body of support. However, while exemplary, even inspiring, instances of practice are common, attempts to replicate models of good practice (o…
Technology and globalization
The drivers of institutional change in health care can be traced in part to technology and in some degree to globalisation and to various interactions between the two. Three papers collected under the banner of ‘Health, IT and globalization’ and published in the Y2K issue of Annual review of health social sciences provide substance for reflection regarding the challenge of factoring ‘globalisation’ into our analyses of the processes of institutio…
Policy Contradictions Limiting Hospital Performance in China
In this paper we explore some of the ways in which the current health policy environment in China enables and constrains performance improvement in the teaching hospital sector. We commenced this research with concerns about the quality and efficiency of hospital services. We aimed to estimate the degree to which these problems are due to weaknesses in management training and/or to wider system factors. We collected data from managers in three te…

Is There a Motor‐constancy Mechanism
Two experiments were conducted to investigate the extent to which subjects can compensate for the effects of elbow angle on the mechanical efficiency of the elbow muscle‐joint system. Subjects were found to be able to match movements made in different directions and made from different starting‐points but unable to match isometrically produced torques made under comparable conditions. The results are compatible with the hypothesis that subjects p…
The Influence of Breath Temperature on Assessments of Blood Alcohol Level by Breath Analysis
Simultaneous measurements of breath alcohol and breath temperature were made in 10 human subjects during the course of several expirations. Breath alcohol concentrations were found to increase during expiration (mean 22.6%). This increase was not generally correlated with increase in breath temperature. Based on assumed values for the partition constant, increases in breath alcohol were predicted from the observed changes in breath temperature. T…
Analysis of Visual and Proprioceptive Components of Motor Skill by Means of a Drug
Skilled manual responses depend upon information about the position of the hand which is to be moved. In order to throw light on the way in which CNS depressant drugs impair skill, an experiment was performed to study the effect of nitrous oxide on the perception of hand position by vision and by proprioception. The results show that these two modalities were less efficient in combination than was either separately. The drug increased the variabi…
