Disentangling Responsibility
Dementia prevention has become a priority in medical research and public health. Various medical, social and environmental risk factors have been identified; however, medical studies, media discourses and health policies predominantly centre on individual‐level prevention and promote personal responsibility for maintaining cognitive health. This emphasis on individual preventive efforts faces increasing criticism for being ethically problematic, …
Ordinary Prosthetics
Based on fieldwork in Quebec, in a home for 14 older people who were diagnosed with dementia - the Carpe Diem - the central proposal of this article is to suggest "alienation" as a way to rethink dementia care. This is based on the insight that the notion of personhood can become a fallacy of care when understood as continuity with the past and empathy, as is often the case in person-centered care. Alienation here consists of three central elemen…
Aging affordances
Dementia prevention now focuses on individual lifestyle choices as loci of intervention in the hope of delaying or preventing cognitive impairment in aging. Drawing from interviews with dementia experts and middle-aged adults in Canada, we discuss how prevention expectations compete with adults' experiences, showing that enacting prevention is not simple but rather fraught with tensions. Addressing the troubles of prevention, we propose aging aff…
Dilemmas of intervention
Discussions regarding personhood and dementia care are often based on practices of recognition; on notions of being-or not being- 'one of us'. This article provides a short overview of personhood as articulated in dementia care, especially in the assemblage of practices known as 'person-centred care' (PCC), and in post-human approaches that developed following the critique of PCC. This article posits an alternative framework, based on a rereading…
Third age fitness
Since 2006, simple outdoor gyms have been installed on public squares all over Brazil. From the beginning, they were mainly conceived as for use by older people - especially women - within an international movement of 'healthy cities'. Based on an ethnography in Rio de Janeiro, our aim is to show in particular the politico-commercial dimensions of the fitness equipment. By insisting on this way of articulating a material dimension of care, differ…
‘Lost in time like tears in rain’
The notion of ‘person-centred care’, popularised by Tom Kitwood in the latter part of his career, has become an influential moral and therapeutic ideal in the dementia care of older people. It has also nurtured a humanistic and de-medicalising turn to a cluster of affiliated philosophical, artistic, relational, spiritual and ethical approaches to dementia as a shared human condition rather than an isolating disease. However, critical dementia res…
Emerging Technologies for Preventing the 'New' Dementia
Experts' views on the use of mostly digital technologies for dementia prevention are characterized by a simultaneity of "gerontechnological optimism" and skeptical hesitancy. Despite the hope for progress in dementia prevention through preventive technologies, experts also point to the complexity of prevention, the importance of environmental factors and public health policies, and the danger of an excessive focus on individual interventions. Wit…

The turn towards prevention – moral narratives and the vascularization of Alzheimer’s disease
Recently, a major turning point in the field of dementia research has occurred: serious and independent studies have shown that there are risk factors that can be modified in ways that reduce dementia risk. However, the reality behind this hopeful message is much more complex than simply translating it into concrete public health prescriptions regarding medications and lifestyle changes. The objective of this article is twofold: 1. In line with o…

How to fix a broken heart
This article focuses on Canadian stem cell researchers working on therapeutic applications of autologous stem cells for heart disease. Building on the concept of ‘multiverse’ – coined by William James and then further developed by Ernst Bloch – we are interested in the simultaneity of the certain and uncertain, sometimes contradictory arguments articulated by these scientists. In the first part of the article we illustrate some of the factors tha…

The Earlier the Better
Heterotopia and Illness
This article is about older women and the way hypertension is linked to their life in a favela, a 'shantytown', in Rio de Janeiro. Inspired by Foucault, I suggest calling this complex phenomenon 'heterotopic illness'. By calling attention to the importance of place for understanding certain illnesses, the limited usefulness of some public health prevention campaigns is shown. Since hypertension can be considered a 'disease of aging', it will be a…
Neither Body nor Brain
This article compares health promotion attitudes towards prostate cancer and Alzheimer's disease. Our aim is to demonstrate that these two apparently distinct conditions of the aging body - one affecting the male reproductive system, the other primarily the brain - are addressed in similar fashion in recent public health activities because of a growing emphasis on a 'cardiovascular logic'. We suggest that this is a form of reductionism, and argue…
The Multiple Anxieties of Getting Older
Most studies on benzodiazepines emphasize overconsumption and warn of addiction, especially by older adults. This article is about the avoidance of benzodiazepine medications by 'aging' women living in a Brazilian village. This case study helps to support our central concern: to call attention to the ambiguities that exist in discussing these medications, and to stress the importance of a multilayered understanding of effect. We argue that benzod…
The Meaning of "Aging in Place" to Older People
This study illuminates the concept of "aging in place" in terms of functional, symbolic, and emotional attachments and meanings of homes, neighbourhoods, and communities. It investigates how older people understand the meaning of "aging in place," a term widely used in aging policy and research but underexplored with older people themselves. Older people (n = 121), ranging in age from 56 to 92 years, participated in focus groups and interviews in…

Tense Prescriptions? Alzheimer Medications and the Anthropology of Uncertainty
This article discusses the two major groups of Alzheimer medications, which are hotly debated in the specialized literature because of their doubtful efficacy. Examining this issue under the rubric of an `anthropology of uncertainty,' this article seeks to address the question: how do doctors prescribe medications given tensions created by uncertainty? A partial answer is drawn from research conducted in Brazil with local psychogeriatricians, whi…

Inverting compliance, increasing concerns
Why, after 40 years of intensive research, is adherence to treatment still an issue? This paper suggests a possible solution to an apparently unsolvable problem: reconceptualizing adherence. To understand how adherence can affect key personnel in any western health system, this study focuses on community nurses working with older mental health patients in Quebec. When they spoke about adherence, nurses presented an idealized image of the nurse-pa…
Entangled Matters—Alzheimer’s, Interiority, and the ‘Unflattening’ of the World
Lessening the Evils, Online
Virtual communities are an especially rich subject for social scientists studying the dynamic and multifaceted ways that groups negotiate health-related knowledge. What are the forces shaping the health information that virtual community members circulate, evaluate and incorporate? This article explores health information circulating on an international, though mainly North American, email list for people suffering from Parkinson’s disease. The d…
