Racial Disparities in Utilization of Emergency Medical Services and Related Impact on Poststroke Disability
BACKGROUND: Prompt seeking of emergency medical services (EMS) assistance at stroke onset is critical to minimize poststroke disability. OBJECTIVE: The aim was to study how racial differences in EMS decision-relevant factors and EMS use impact stroke care and disability outcomes. DESIGN: A prospective observational study. PARTICIPANTS: A total of 1168 acute ischemic stroke patients discharged from April 2016 to October 2017 at a safety net hospit…
A Nationwide Population-Based Study on the Association between Land Transport Accident and Peripheral Vestibular Disorders
This case-control study aimed to investigate the association of peripheral vestibular disorders (PVD) with subsequent land transport accidents. Data for this study were obtained from Taiwan's National Health Insurance (NHI) dataset. We retrieved 8704 subjects who were newly found to have land transport accidents as cases. Their diagnosis date was used as their index date. Controls were identified by propensity score matching (one per case, n = 87…
A Nationwide Population-Based Study on the Incidence of Parapharyngeal and Retropharyngeal Abscess—A 10-Year Study
This study aimed to investigate the annual incidence of parapharyngeal and retropharyngeal abscess (PRPA) based on 10-year population-based data. Patients with PRPA were identified from the Taiwan Health Insurance Research Database, a database of all medical claims of a randomly selected, population-representative sample of over two million enrollees of the National Health Insurance system that covers over 99% of Taiwan's citizens. During 2007-20…
A Population-Based Study of Healthcare Resource Utilization in Patients with Mitral Valve Prolapse
This study investigated differences in the utilization of healthcare services between subjects with mitral valve prolapse (MVP) and comparison subjects using data from Taiwan's National Health Insurance population-based database, 138,493 patients with MVP (study group) and 138,493 matched patients without MVP (comparison group). We calculated the utilization of healthcare services in the year 2016 for each study sample. Patients with MVP had more…
Antenatal care satisfaction in a developing country
ANC satisfaction in Nigeria may be enhanced by improving responsiveness to clients, clinical care quality, ensuring equipment availability, optimizing easy access to medicines, and expanding free ANC services
Reducing High-Users’ Visits to the Emergency Department by a Primary Care Intervention for the Uninsured
Reducing avoidable emergency department (ED) visits is an important health system goal. This is a retrospective cohort study of the impact of a primary care intervention including an in-hospital, free, adult clinic for poor uninsured patients on ED visit rates and emergency severity at a nonprofit hospital. We studied adult ED visits during August 16, 2009-August 15, 2011 (preintervention) and August 16, 2011-August 15, 2014 (postintervention). W…
Persisting Racial Disparities in Colonoscopy Screening of Persons with a Family History of Colorectal Cancer
Lower Mortality Among Stroke Patients With Schizophrenia
OBJECTIVE: To examine 90-day mortality among schizophrenia patients who suffered a recent stroke, using population-based data and a retrospective cohort design. Increasing evidence demonstrates a higher cardiovascular risk for schizophrenia patients compared with the general population. There are no data on stroke outcomes among schizophrenia patients. METHODS: Data were derived from the Taiwan National Health Insurance Research Database and Caus…
Cesarean and VBAC rates among immigrant vs. native-born women
Immigrant Vietnamese and Chinese mothers have lower odds of cesarean and higher VBAC odds than native-born Taiwanese, consistent with lower cesarean rates prevailing in their home countries (Vietnam 10.1%; mainland China 20%-50% rural and urban respectively)
Neonatal Outcomes for Immigrant vs. Native-Born Mothers in Taiwan
Quality and Safety of Screening Colonoscopies Performed by Primary Care Physicians With Standby Specialist Support
BACKGROUND: Expanding the population's access to colonoscopy screening can reduce colorectal cancer disparities. Innovative strategies are needed to address the prevailing 50% colonoscopy screening gap, partly attributable to inadequate specialist workforce. This study examined the quality of colonoscopies by primary care physicians (PCPs) with standby specialist support at a licensed ambulatory surgery center. METHODS: Retrospective data on 10,9…
Enhancing the Care Continuum in Rural Areas
Context: Community Health Centers (CHCs) and Critical Access Hospitals (CAHs) play a significant role in providing health services for rural residents across the United States. Purpose: The overall goal of this study was to identify the CAHs that have collaborations with CHCs, as well as to recognize the content of the collaborations and the barriers and facilitators to collaborations. Methods: The target population was CAHs within 60 miles of CH…
Providing Shelter to Nursing Home Evacuees in Disasters
Objectives. We examined nursing home preparedness needs by studying the experiences of nursing homes that sheltered evacuees from Hurricane Katrina. Methods. Five weeks after Hurricane Katrina, and again 15 weeks later, we conducted interviews with administrators of 14 nursing homes that sheltered 458 evacuees in 4 states. Nine weeks after Katrina, we conducted site visits to 4 nursing homes and interviewed 4 administrators and 38 staff members. …
African Americans’ Decisions Not to Evacuate New Orleans Before Hurricane Katrina
Objectives. We examined the psychosocial and personal factors that influenced African Americans’ decision not to evacuate New Orleans, La, before Hurricane Katrina’s landfall. Methods. We conducted 6 focus groups with 53 African Americans from New Orleans who were evacuated to Columbia, SC, within 2 months of Hurricane Katrina. Results. The major themes identified related to participants’ decision to not evacuate were as follows: (1) perceived su…
Do group practices have lower caesarean rates than solo practice obstetric clinics? Evidence from Taiwan
OBJECTIVE: This study examined physicians' propensity for caesarean deliveries at solo versus group practice obstetrics/gynaecology (ob/gyn) clinics in Taiwan. METHOD: We used population-based (National Health Insurance) claims data covering all 253 618 singleton deliveries conducted at ob/gyn clinics, during 2000-02. The dependent variable, delivery mode, was treated as dichotomous [caesarean section (CS) = 1, vaginal delivery (VD) = 0]. The ind…
Impact of a cultural belief about ghost month on delivery mode in Taiwan
Background: Many Chinese believe the lunar month of July, “ghost month” is inauspicious for major surgical procedures. This study hypothesised that caesaren delivery (CS) rates will be significantly lower during lunar July, and higher than normal during June, representing pre-emptive caesaren delivery to avoid delivering in July. Methods: Population based data from Taiwan on all singleton deliveries during 1997–2003 (1 750 862 cases) were subject…
