Implementation Determinants of Integrated Tuberculosis and Diabetes Care in South Asian Association for Regional Cooperation (Saarc) Countries
Background: Tuberculosis (TB) and diabetes mellitus (DM) represent a growing syndemic in low- and middle-income countries (LMICs), particularly across South Asia. The bidirectional relationship between these diseases exacerbates health outcomes and increases system burdens. Although the World Health Organization has advocated for integrated management of TB and DM, implementation remains inconsistent across the SAARC region. This systematic revie…
Unlocking the potential of community pharmacies to address hypertension in South Asia
Hypertension is a leading contributor to cardiovascular disease in South Asia, affecting over 40% of adults, of whom most remain undiagnosed or poorly managed. Despite urgent healthcare needs, the already overstretched public primary care systems in low- and middle-income countries, particularly in rapidly growing urban areas, are falling short. Community pharmacies, often the first point of contact for low-income urban residents, represent an op…
Community Health Participatory interventions in the prevention and control of non-communicable diseases including mental health in crisis-affected Low-and Middle-Income Countries – a scoping review
The rising burden of Non-Communicable Diseases (NCDs) requires a comprehensive strategy by integrating community-based interventions - especially in Low- and Middle-Income Countries (LMICs). Over the past decade, researchers have emphasized communities as key agents of change in health systems. While Community Health Participatory (CHP) interventions show promise in NCD management, their application in crisis-affected contexts remains underexplor…
Patients’, caregivers’ and healthcare workers’ perspectives on integrating anxiety care in tuberculosis services in Pakistan
Up to 80% of active tuberculosis (TB) patients experience anxiety, threatening their treatment outcomes. The National TB Control Programme in Pakistan currently only provides mental health services for patients with multi-drug resistant TB, leaving those with drug sensitive TB (DSTB) unsupported. This study explored (1) experiences of anxiety and self-managed coping strategies used by patients with DSTB and their caregivers (2), current practice …
Probability of Stroke in Blacks and Non-Hispanic Whites Treated with ACE-I, ARBs, and Other Antihypertensives
Engagement of coMmunity through Participatory learning and action for cOntrol and preVEntion of type II Diabetes and its Risk factors (Empower-D) - A Protocol for a Cluster Randomised Controlled Trial
The trial is registered with clinicaltrials.gov-NCT06561126. Date of registration: 23rd August 2024
Prevalence and associated factors of parental refusal rates for routine immunisation
The high immunization refusal rate in Peshawar among parents was associated with maternal employment, logistical challenges such as distance immunisation services, associated cost, vaccine hesitancy and safety of vaccines. It is recommended to deploy mobile immunisation units and outreach sessions to improve access, using targeted, evidence-based education to address vaccine safety concerns
Core outcome sets for trials of interventions to prevent and to treat multimorbidity in adults in low and middle-income countries
INTRODUCTION: The burden of multimorbidity is recognised increasingly in low- and middle-income countries (LMICs), creating a strong emphasis on the need for effective evidence-based interventions. Core outcome sets (COS) appropriate for the study of multimorbidity in LMICs do not presently exist. These are required to standardise reporting and contribute to a consistent and cohesive evidence-base to inform policy and practice. We describe the de…
Living with depression and diabetes
Diabetes and depression are both serious health conditions. While their relationship is bidirectional and each condition adversely affects outcomes for the other, they are treated separately. In low and middle income countries, such as Bangladesh and Pakistan, health systems are already stretched and the integration of diabetes and depression care is rarely a priority. Within this context through interviews with patients, healthcare workers and p…
Depression care integration in tuberculosis services
BACKGROUND: The co-occurrence of depression among tuberculosis (TB) patients is a critical issue, contributing to poor treatment outcomes, prolonged hospitalisations and increased healthcare expenses. OBJECTIVE: The objective of this study was to assess the feasibility of delivering a co-designed depression care pathway within TB services in Pakistan. DESIGN: Mixed-method study. SETTING AND PARTICIPANTS: Routine depression screening for TB patien…
Perspectives of health workers engaging in task shifting to deliver health care in low-and-middle-income countries
BACKGROUND: Low- and middle-income countries (LMICs) are experiencing growing demand for healthcare services yet face a persistent shortage in access to specialist health workers (SHWs). Task shifting is an approach used to address this gap in service provision. Specific healthcare tasks are shifted to other, larger cadres of non-specialist health workers (NSHWs), including lay health workers with SHWs potentially taking on supervisory roles. Pre…
Addressing TB multimorbidity in policy and practice
In people with TB, co-existence of long-term conditions (e.g., depression, diabetes and HIV) and risk factors (e.g.,alcohol misuse, malnutrition, and smoking) are associated with increased mortality and poor treatment outcomes including delayed recovery, TB treatment failure and relapse. However, it is unclear as to what extent these comorbidities are addressed in TB policy and practice. Between August and October 2021, we conducted an online cro…
Perceptions of Covid–19-related Risks and Deaths Among Health Care Professionals During Covid-19 Pandemic in Pakistan
COVID-19 is an established challenge to the health care professionals (HCPs) having physical, emotional, and psychological repercussions. We determine the perceptions of Pakistani HCPs regarding the risks and deaths related to COVID-19. A cross-sectional study was conducted among HCPs throughout Pakistan from 16th February 2021 to 6th March 2021 by disseminating an online Google questionnaire via all possible social media platforms including What…
Correlation between maternal and neonatal blood Vitamin D level
In Pakistan, there is limited evidence for the levels and relationship of 25 (OH) Vitamin D (25(OH)D) status in pregnant women and their newborns, while the association between maternal 25(OH)D and newborn anthropometric measurements remains unexplored. Sociodemographic data were collected from 213 pregnant mothers during their visit to a tertiary care hospital at the time of childbirth. Anthropometric measurements were performed on all mothers a…
