Changes to autonomous motivation and reductions in amotivation led to positive mental health in overweight male football fans
OBJECTIVE: Do post-program motivational regulations mediate the intervention effect of the SDT-based European Fans in Training (EuroFIT) program on self-esteem, well-being and vitality at 12-months follow-up? METHODS: A causal mediation analysis of data from the randomised controlled trial of the EuroFIT program that recruited 1113 overweight, male football fans in England, Norway, Portugal and the Netherlands. RESULTS: <.001), but not controlled…
An uneven playing field
Sports betting is a growth area for the gambling industry, with football fans a key target of advertising. Men are particularly at risk from gambling harm. The 8-week Football Fans and Betting (FFAB) intervention was designed for delivery in professional football clubs by club community coaches to reduce sports betting and other forms of gambling among men aged 18–55 with a PGSI score of < 15. This paper reports the acceptability and feasibility …
Changes to men’s walking and the contribution of walking to self-reported physical activity in the EuroFIT programme
Objective: Gender-sensitive approaches to increasing men’s physical activity (PA) through sports settings have shown promise across cultural contexts. We examined changes to men’s walking and the contribution of walking towards self-reported PA after participating in the men-only European Fans in Training (EuroFIT) programme before exploring men’s experiences of sustaining walking to explain the quantitative findings. Design: A sequential mixed-m…
Rapid ethnographic appraisal of community concepts of and responses to joint pain in Kilimanjaro, Tanzania
INTRODUCTION: Musculoskeletal disorders, experienced as joint pain, are a significant global health problem, but little is known about how joint pain is categorised and understood in Tanzania. Understanding existing conceptualisations of and responses to joint pain is important to ensure both research and interventions are equitable and avoid biomedical imposition. METHODS: Rapid ethnographic appraisal was conducted in a periurban and rural commu…
‘ … You become a prisoner of your life’
The increased prevalence of non-communicable diseases (NCDs) in recent years has led many Low- and Middle-Income Countries (LMICs), including Tanzania, to develop policies to manage their burden. Musculoskeletal (MSK) conditions, such as arthritis, account for 20% of all years lived with disability in LMICs, but the NCD strategies rarely address them. There is substantial research on the disruption MSK conditions cause to people's lives within Hi…
Increasing HIV testing and linkage to care among men in rural South Africa using conditional financial incentives and a decision support app
Men in sub-Saharan Africa are less likely to accept HIV testing and link to HIV care than women. We conducted a trial to investigate the impact of conditional financial incentives and a decision support application, called EPIC-HIV, on HIV testing and linkage to care. We report the findings of the trial process evaluation to explore whether the interventions were delivered as intended, identify mechanisms of impact and any contextual factors that…
Effect of a Male-Targeted Digital Decision Support Application Aimed at Increasing Linkage to HIV Care Among Men
A more equitable approach to economic evaluation
Capability wellbeing can potentially provide a holistic outcome for health economic evaluation and the capability approach seems promising for African countries. As yet there is no work that has explored the evaluative space needed for health and care decision making at the whole population level and procedures that merely translate existing measures developed in the global north to contexts in the global south risk embedding structural inequalit…
How European Fans in Training (EuroFIT), a lifestyle change program for men delivered in football clubs, achieved its effect

Participation in mass dog vaccination campaigns in Tanzania
Background: Canine rabies causes about 59,000 human deaths each year globally but the disease can be eliminated by sustaining sufficient dog vaccination coverage over several consecutive years. A challenge to achieving high coverage is low participation of dog owners in vaccination campaigns. We explored whether and how previously identified contributory factors to low participation can be addressed through community engagement activities. Method…

DESIGN: = 93) were analysed. MAIN OUTCOME MEASURES: We explored the feasibility and acceptability of program setting and context, recruitment strategies, factors impacting implementation and mechanisms of impact. RESULTS: Recruitment via Australian Football L…
Experiences of multimorbidity in urban and rural Malawi
Multimorbidity (presence of ≥2 long term conditions (LTCs)) is a growing global health challenge, yet we know little about the experiences of those living with multimorbidity in low- and middle-income countries (LMICs). We therefore explore: 1) experiences of men and women living with multimorbidity in urban and rural Malawi including their experiences of burden of treatment and 2) examine the utility of Normalization Process Theory (NPT) and Bur…
Can a conditional financial incentive (CFI) reduce socio-demographic inequalities in home-based HIV testing uptake? A secondary analysis of the Hits clinical trial intervention in rural South Africa
In sub-Saharan Africa, home-based HIV testing interventions are designed to reach sub-populations with low access to HIV testing such as men, younger or less educated people. Combining these interventions with conditional financial incentives (CFI) has been shown to be effective to increase testing uptake. CFI are effective for one-off health behaviour change but whether they operate differentially on different socio-demographic groups is less cl…
Sitting as a moral practice
Amidst public health campaigns urging people to sit less as well as being more physically active, this paper investigates how older adults make sense of their sedentary behaviour. Using an accounts framework focusing on how people rationalise their sitting practices, we analysed data from 44 qualitative interviews with older adults. All interviewees had received information about sedentary behaviour and health, visual feedback on their own object…
Scale-Up and Scale-Out of a Gender-Sensitized Weight Management and Healthy Living Program Delivered to Overweight Men via Professional Sports Clubs
Increasing prevalence of obesity poses challenges for public health. Men have been under-served by weight management programs, highlighting a need for gender-sensitized programs that can be embedded into routine practice or adapted for new settings/populations, to accelerate the process of implementing programs that are successful and cost-effective under research conditions. To address gaps in examples of how to bridge the research to practice g…

Men’s reactions to receiving objective feedback on their weight, BMI and other health risk indicators
Undertaking measurements and receiving information on health risk indicators, such as weight or BMI, within the context of behaviour change programmes can enhance motivation for behaviour change when communicated in an empathic and non-stigmatising way, and therefore should be considered as an integral part of interventions. However, providing feedback on health risk may be insufficient to prompt behaviour change in some people and may be detrime…
Can professional football clubs deliver a weight management programme for women
In this evaluation, FFIT for Women was feasible, acceptable and demonstrated potential as a weight loss programme. Our findings suggest the programme has the potential to produce outcomes that are on a par with existing commercial and state-funded offerings

Coz football is what we all have
In this paper we use a social practice approach to explore men's experience of Football Fans in Training (FFIT), a group-based weight management programme for men that harnesses men's symbolic attachment to professional football clubs to engage them in lifestyle change. FFIT is delivered by community coaches in clubs' stadia and is gender-sensitised in relation to context, content and style of delivery. Using a 'toolkit' of concepts from the work…

Total purchasing, community and continuing care
The introduction of total purchasing pilots (TPPs) into the National Health Service (NHS) gave general practitioners (GPs) significant new opportunities to take responsibility for the development of community and continuing care (CCC) services. Based on five case studies of TPPs involved in developing CCC this paper asks three questions: (1) to what extent were the TPP's involvement in CCC informed by an awareness of CCC policy?; (2) were TPPs in…

Do Men's and Women's Accounts of Surviving a Stroke Conform to Frank's Narrative Genres
We compared the illness narratives of 9 male and 9 female United Kingdom stroke survivors using Frank's typologies of illness narratives. Most respondents presented a single dominant narrative genre ("quest memoir," "restitution," "chaos," or a new "despair" genre); none presented quest manifesto or automythology narratives of social action or self-reinvention. We found no gender differences apparent in which genres respondents presented. Stroke …

The Evaluation of Complex Health Policy
The last five years have witnessed the increasing use of evaluated pilot programmes as a way of developing health services policy in the UK. Total purchasing was the first major quasi-market development in the National Health Service to be independently evaluated from the outset. The initiative allowed local, volunteer pilots considerable freedom to implement extensions to existing arrangements for general practitioner budget holding for speciali…
A managed clinical network for cardiac services
The Managed Clinical Network made a difference to ways of working, particularly in breaching traditional boundaries and involving the public, and made modest changes in patient care. However, it required a two-year "set-up" period. Managed clinical networks are complex initiatives with an increasing profile in health care policy. This study suggests that they require energetic leadership and improvements are likely to be slow and incremental

Words matter
Although most people who are overweight or obese accept that it is appropriate for health professionals to discuss weight issues with patients, there is great variation in response to the terms commonly used to describe excess weight. There is no one-size-fits-all approach to discussing weight status: some men and younger people may appreciate a direct approach, whilst others need to be treated more sensitively. It is therefore important that hea…
Can the draw of professional football clubs help promote weight loss in overweight and obese men? A feasibility study of the Football Fans in Training programme delivered through the Scottish Premier …
Objective The UK prevalence of male obesity is among the highest in Europe. Excess weight is associated with ill-health, but while 5-10% weight loss can significantly improve future health, men are reluctant to engage in traditional weight management programmes. This study examines the potential of professional football clubs to engage overweight and obese men in weight loss. Football Fans in Training (FFIT) is a 12 week, gender-sensitised weight…
How personal experiences feature in women's accounts of use of information for decisions about antenatal diagnostic testing for foetal abnormality
Health and Illness in a Connected World
Context: The use of the Internet for peer‐to‐peer connection has been one of its most dramatic and transformational features. Yet this is a new field with no agreement on a theoretical and methodological basis. The scientific base underpinning this activity needs strengthening, especially given the explosion of web resources that feature experiences posted by patients themselves. This review informs a National Institute for Health Research (NIHR)…
Epidemiology of multimorbidity and implications for health care, research, and medical education
Imagined futures
Background Knowledge of disability is considered key information to enable informed antenatal screening decisions by expectant parents. However, little is known about the role of experiential knowledge of disability in decisions to terminate or continue with a pregnancy diagnosed with a fetal abnormality. Objective To explore the role that expectant parents’ experiential knowledge of disabilities and conditions can play in real‐life decisions to …

A gender-sensitised weight loss and healthy living programme for overweight and obese men delivered by Scottish Premier League football clubs (FFIT)
If patient‐reported outcome measures are considered key health‐care quality indicators, who is excluded from participation
Patient‐reported outcome measures have received increasing attention with regard to ensuring quality improvement across the health service. However, there is a risk that people with disabilities and low literacy are systematically excluded from the development of these measures as well as their application in clinical practice. This editorial highlights some of these risks and the potential consequences of exclusion for these groups
Do weight management programmes delivered at professional football clubs attract and engage high risk men? A mixed-methods study
The delivery of a weight management intervention via professional football clubs attracted men at high risk of ill-health. The setting enabled men to join a weight management programme in circumstances that felt 'right' rather than threatening to themselves as men. FFIT is an example of how to facilitate health promotion activities in a way that is consistent with, rather than challenging to, common ideals of masculinity
