Cohort Profile
The Cebu Longitudinal Health and Nutrition Survey (CLHNS) was originally conceptualized as an interdisciplinary study of infant-feeding patterns, particularly the overall sequencing of feeding events (milks and complementary foods), the factors affecting feeding decisions and how feeding patterns affect the infant, mother and household. The idea was to study these topics within as natural a setting as possible and to analyse how infant-feeding de…
Decentralisation and government provision of public goods
While many developing countries have devolved health care responsibilities to local governments in recent years, no study has examined whether decentralisation actually leads to greater health sector allocative efficiency. This paper approaches this question by modeling local government budgeting decisions under decentralisation. The model leads to conclusions not all favourable to decentralisation and produces several testable hypotheses concern…
Changes in access to health care in China, 1989–1997
The post-1979 period in China has seen the implementation of reforms that dismantled much of the Maoist era social welfare system and permitted a significant reallocation of society's resources. The result has been rapid but uneven economic development that has profoundly altered the environment within which consumers make health investment decisions. Many studies report significant and apparently non-random reductions in health care utilization …
Did the distribution of health insurance in China continue to grow less equitable in the nineties? Results from a longitudinal survey
Health-care Facility Choice and the Phenomenon of Bypassing
Health policy-makers in developing countries are often disturbed and to a degree surprised by the phenomenon of the ill travelling past a free or subsidized local public clinic (or other public facility) to get to an alternative source of care at which they often pay a considerable amount for health care. That a person bypasses a facility is almost certainly indicative either of significant problems with the quality of care at the bypassed facili…

Effect of provider characteristics on choice of contraceptive provider
We use surveys of households and health-care facilities conducted in the same area at the same time to determine which characteristics of providers attract users of contraceptives. By using the full-information maximum-likelihood technique to jointly estimate choice of contraceptive method and choice of provider, we avoid self-selection bias. Results support the need for modeling quality and for jointly estimating the choice of contraceptive meth…

The Impact of Population Policies
Lant Pritchett has argued that on the basis of empirical findings variations in fertility desires account for most of the variation in fertility rates across countries and that access to contraception contributes at best a significant but numerically small (2-3%) independent effect. Pritchett concluded that changing fertility desires and increased contraceptive access cause some trivial fertility reductions. Pritchetts empirical analysis is belie…

Nutrition, lactation, and birth spacing in filipino women
The Cebu Longitudinal Health and Nutrition Survey is used to examine the roles of women’s nutrition and infant feeding in determining time from birth to menses and time from menses to conception. The analysis sample includes 2,648 Filipino women followed for 24 months postpartum. Recently devised statistical estimation techniques to control for unobserved heterogeneity and endogeneity are employed in estimating a two-state hazard model. Low body …
Influences on the Extent of Breast-Feeding
There has been much controversy about the impact of the health sector and the infant food industry on breast-feeding behavior. This study links causal factors to breast-feeding decisions, using a longitudinal survey of more than 3,000 Filipino mother-infant pairs. Most factors decreasing the likelihood that mothers will breast-feed seem to be related to family economics. Delivery in a private hospital, urban residence, high income, absence of spo…
Breast-feeding and Diarrheal Morbidity
This study used a unique longitudinal survey of more than 3000 mother-infant pairs observed from pregnancy through infancy. The sample is representative of infants from the Cebu region of the Philippines. The sequencing of breast-feeding and diarrheal morbidity events was carefully examined in a longitudinal analysis which allowed for the examination of age-specific effects of feeding patterns. Because the work controlled for a wide range of envi…

Prenatal care and pregnancy outcome in Cebu, Philippines
Breastfeeding trends in the Philippines, 1973 and 1983
This paper examines comparable national surveys of breastfeeding from the Philippines carried out in 1973 and 1983. The probability of breastfeeding at selected infant ages is estimated, using the weighted life table. The conclusions are that a 5 per cent decline in the proportion of infants ever breast-fed occurred during the referenced period, and that median length of breastfeeding remained essentially the same
The Effect of Economic Factors on Contraceptive Choice in Jamaica and Thailand
Child Spacing in the Philippines
In this study the effects of characteristics current at the beginning of the birth interval on the time elapsing before the next birth are examined. The child-spacing process is viewed as affected by time-varying and fixed characteristics of the community, household, and individual. In the model on which the empirical research is based sociological and economic antecedents are used. The study is based on data from two panels of the Bicol Multipur…

Breastfeeding Patterns and Determinants in the Near East
Few detailed studies of the determinants of breastfeeding behaviour have been carried out, especially in the Middle East. We provide a comparative investigation of patterns and determinants of breastfeeding in four Middle East countries (Egypt, Jordan, Tunisia and Yemen) based on WFS data. Differences in factors influencing ever-breastfeeding, continuing breastfeeding from very short to moderate durations, and extending breastfeeding into the sec…

The Determinants of Breast-Feeding in Sri Lanka
Breast-feeding is the focus of rapidly growing interest. Research on the determinants of breast-feeding is only beginning. The research in this paper is based on World Fertility Survey data for Sri Lanka. We develop what we believe to be an appropriate probit model and find that there are significant socioeconomic factors that influence breast-feeding, in addition to the demographic factors focused upon in the literature. Moreover, some of them h…

How Much Child Support Can Absent Fathers Pay
A variety of empirical studies show that custodial mothers and children are worse off after separation and divorce than absent fathers. Many of these mothers and children live in poverty and many more are above the poverty line but below median family income. Since 1974 the U.S. has had a federal‐state program of Child Support Enforcement, but absent fathers still have a poor record of paying child support. To estimate how much money absent fathe…

The Efficiency of Local School Finance
The most prevalent arguments for increased state and federal aid to education are concerned with equity criteria. Relatively little has been written or discussed concerning the efficiency aspects of aid to education. This paper uses a median voter decision model and certain assumptions about the functional form of demand functions to obtain an estimate of the efficient level of expenditure for each of 104 school districts in New York State. The a…

Microdata estimation of school expenditure levels
Part 1
This review relates to 2 articles in Medical Anthropology (Greiner et al. 1981; Gussler and Briesmeister 1980) which infer a large decline in the extent of breastfeeding and focus on a limited set of myriad factors which are believed to affect lactation extent and duration. They concentrate on the insufficient milk syndrome and use attitudinal information to a large extent. The assumption of the lage universal breastfeeding decline is questioned …

Commenting on the authors paper Determinants of Breastfeeding in Sri Smith reanalyzed the World Fertility Survey (WFS) data for Sri Lanka using different estimation procedures variables and sample sizes. The results agree with respect to the impacts of parity education and residence on breastfeeding duration. Smith stated that the only variables for which results differed were working away from home contraceptive use and mothers age. He did howev…
